Bij patiënten met overgewicht of obesitas verandert de verdeling en uitscheiding van geneesmiddelen.

Onderzoek

In dit onderzoek werden bij (morbide) obese (BMI > 40 kg/m2) en non-obese mensen (BMI 18-25 kg/m2) concentraties in het bloed gemeten van drie verschillende antibiotica (vancomycine, tobramycine, gentamicine). Deze antibiotica worden gebruikt bij ernstige, levensbedreigende infecties.

Resultaten

Deze studie heeft kunnen vaststellen hoe precies de uitscheiding en verdeling van deze antibiotica toeneemt bij patiënten met verschillende mate van overgewicht. Op basis van deze resultaten zijn voor deze antibiotica doseeradviezen voor obese patiënten opgesteld die in de praktijk kunnen worden gebruikt. Hierdoor kunnen deze antibiotica veiliger en effectiever worden toegepast bij (morbide) obese patiënten.


Samenvatting bij start

Bij ernstige, levensbedreigende infecties is het belangrijk dat de patiënt zo snel mogelijk een effectieve, maar niet te hoge, dosis antibiotica krijgt.
Het is bekend dat bij patiënten met ernstig overgewicht de verdeling en uitscheiding van geneesmiddelen verandert. Hoe dit zit bij veel gebruikte antibiotica zoals vancomycine, tobramycine en gentamicine, die per infuus worden gegeven, is nog onduidelijk.
Daarom wordt in dit onderzoek onderzocht hoe deze antibiotica zich precies gedragen in het lichaam van patiënten met ernstig overgewicht door concentraties van de antibiotica te meten in bloed bij patiënten met ernstig overgewicht die een maagverkleiningsoperatie ondergaan.

De resultaten worden vergeleken met patiënten zonder overgewicht. Met deze gegevens worden doseerrichtlijnen opgesteld die door artsen en apothekers gebruikt kunnen worden bij de behandeling van ernstige infecties bij patiënten met overgewicht.

Samenvatting van de aanvraag

Timely, adequate and optimal treatment of infectious diseases is essential for the survival of patients with bacterial infections (Surviving Sepsis campaign). Specifically in (morbid) obese patients information on optimal dosage of antibiotics is scarce. This is an urgent problem since the prevalence of obesity is increasing worldwide, and particularly because obesity is known to influence different pharmacokinetic parameters such as clearance and distribution volume. While the influence of obesity on the pharmacokinetics of antibiotics has only been sparsely investigated, adequate treatment with antibiotics of obese individuals is being jeopardized. For severe life-threatening infections, vancomycin, gentamicin and tobramycin are important antibiotics, of which the serum concentrations are known to be related to both efficacy and toxicity measures. There are to date no specific guidelines available for clinical practice on how to adjust the dose of these antibiotics in (morbid) obesity, for several reasons: 1) the antibiotics have mostly been investigated in a less heavy population, 2) different dose frequencies have been investigated or 3) outdated target concentrations have been used. As a consequence, the chances that target concentrations will be rapidly reached in obese patients are small. It is clear that there is an urgent need for evidence based dose guidelines for these specific patients that result in the desired exposure with optimal efficacy and minimal toxicity. Therefore, the goal of this study is to develop dose guidelines for vancomycin, gentamicin and tobramycin in (morbid) obese patients by characterizing the pharmacokinetics of these drugs in this patient population, based on the most recent insights in target concentrations. The information on pharmacokinetics will be implemented in relevant guidelines and in software to aid clinicians to determine the optimal dose for each individual patient in daily care.
