Improved pharmacotherapy for Dutch preschool children with asthma-like symptoms by the innovative breath test.

Astmaklachten (benauwdheid, hoesten en piepen/zagen) komen bij 40% van alle jonge kinderen onder de 6 jaar voor. Hierbij geldt dat eenderde deel echt astma heeft en tweederde deel groeit over de klachten heen en is klachtenvrij bij 5-6 jaar (ook wel ‘viral wheeze’ of ‘episodisch piepen’ genoemd). Tot voor kort was er geen diagnostische test die kinderen met astma kan onderscheiden van ‘viral wheeze’ wat leidt tot onderdiagnostiek/onderbehandeling van astma en overbehandeling van ‘viral wheeze’. In een eerder onderzoek is de ademtest voor een vroege astmadiagnose ontwikkeld.

Onderzoek

In dit vervolgonderzoek wordt de gezondheidswinst en de vermindering van zorgkosten door de toepassing van de ademtest bepaald: 220 kinderen met astmaklachten van 2-3 jaar oud worden meerdere jaren gevolgd.

Verwachte uitkomst

De invoering van de ademtest in Nederland betekent niet alleen een betere behandeling van jonge kinderen, maar ook een grote jaarlijkse kostenbesparing. De ademtest voor kinderen is ook gunstig voor ouders en behandelaars.

Samenvatting van de aanvraag

Problem definition: Asthma-like symptoms like breathlessness, coughing, and wheeze are present in 40% of all children below 6 yrs. A third part of these children really have asthma, 2/3 part is growing out of symptoms at 5-6 yrs of age (also called ‘viral wheeze’). Currently there is no diagnostic test, which can discriminate between children with asthma and viral wheeze. This leads to underdiagnosis/undertreatment of asthma, and overtreatment of children with viral wheeze. We recently demonstrated that a breath test is able to diagnose asthma at an early age. Objective: to assess the total gain in health, correct pharmacotherapy, and the savings of costs of care by using the diagnostic breath test. Study design: a multi-centre RCT during 4 years. Study population: 220 children aged 2-3 yrs with asthma-like symptoms. Diagnostic intervention and comparator: at baseline in the intervention group, the breath test is performed and the results will be given tot the parents/treating doctor. Children with an asthma diagnosis will receive asthma medication, whereas asthma drugs will be stopped and referrals to specialist care avoided as much as possible in children with viral wheeze. Outcome measures: % well controlled disease (primary outcome), exacerbations, healthcare use and costs (secondary outcomes). Sample size: 220 children are necessary to detect a 20% difference in % well controlled disease between the intervention and usual care group with an alpha of 0.05 and a power of 80%. Data-analysis: comparison of outcome measures between the intervention and control group (Chi-square test, unpaired t-test or Kruskal-Wallis test). Economic evaluation: A cost-effectiveness analysis will be performed from a societal and healthcare perspective with a time horizon of 2 years. Relevance: The breath test is a reliable, fast, safe, and non-invasive diagnostic test for improved, personalized treatment and better outcome in this large group of preschool children.
