Walk and talk: Effectiviteit van een nieuwe intensieve behandeling voor PTSS

Ernstige schokkende gebeurtenissen laten niemand onberoerd. De meesten herstellen na enkele dagen. Bij sommigen gaat het niet over en ontwikkelen zich klachten die we kennen als PTSS. Vele vormen van psychotherapie zijn ontwikkeld waar toch maar 35% voldoende op reageert. In dit project wordt een revolutionaire en nieuwe behandeling onderzocht die intensiever en persoonlijker is. De behandeling is traumagericht en wordt aangeboden terwijl de patiënten loopt. Met persoonlijke elementen via foto's en muziek wordt op een interactieve manier via het lopen de intensiteit vergroot. Uit de stoel dus! In dit project wordt de (kosten)effectiviteit van deze behandeling onderzocht. Hypothese is dat minder sessies nodig zijn en meer patiënten kunnen profiteren. Dit levert effectievere zorg en ook kostenbesparing op. Doelgroep is patiënten met PTSS door trauma in de werksituatie. Werving via Centrum ’45 en GGZ Drenthe.

Samenvatting van de aanvraag

SAMENVATTING -DOELSTELLING De posttraumatische stress-stoornis (PTSS) is een psychische stoornis met grote gevolgen op het gebied van gezondheid en kosten. Nationale en internationale richtlijnen bevelen traumagerichte psychotherapieën aan als eerste keus behandeling. Traumagerichte psychotherapie is bewezen effectief en doelmatig, maar non-respons loopt binnen sommige PTSS populaties op tot tweederde van de behandelde groep. Wij beogen de doelmatigheid van een nieuwe interventie, hoogintensieve bewegingsafhankelijke psychotherapie, te beoordelen ten opzichte van conventionele traumagerichte psychotherapie in de behandeling van PTSS. -HYPOTHESE Hoogintensieve, bewegingsafhankelijke multi-modulaire psychotherapie vergroot het herstel en de snelheid van herstel, bevordert therapietrouw en zal daarom qua doelmatigheid superieur zijn aan gebruikelijke therapie. -STUDIE OPZET Multicenter, enkelblinde, gerandomiseerde gecontroleerde studie met parallelle groepen die de doelmatigheid van de nieuwe therapie vergelijkt met standaardzorg in een superioriteitsdesign. De experimentele groep ontvangt 10 wekelijkse sessies en de controlegroep 16 wekelijkse sessies. De primaire uitkomstvariabele wordt wekelijks gemeten. Overige assessments vinden plaats voor en na behandeling, na 3 en 6 maanden. -STUDIE POPULATIE Patiënten met werkgerelateerde PTSS, geworven bij Centrum ’45 en GGZ Drenthe. -INTERVENTIE De experimentele conditie bestaat uit 7 sessies geprotocolleerde interactieve bewegingsafhankelijke multi-modulaire psychotherapie, genaamd 3MDR, gevolgd door 3 sessies traumagerichte psychotherapie. -USUAL CARE / VERGELIJKING De controleconditie bestaat uit 16 sessies geprotocolleerde standaardzorg, zijnde traumagerichte psychotherapie (TF-PT) in de vorm van traumagerichte cognitieve gedragstherapie of eye movement desensitization & reprocessing therapie. -UITKOMSTVARIABELEN Zelfgerapporteerde PTSS (primaire uitkomstvariabele), PTSS gemeten met klinisch interview, gezondheidsgerelateerde kwaliteit van leven en kosten geassocieerd met psychische stoornissen, patiëntbeoordelingen, vermijding, depressieve symptomen en algemeen functioneren. -STEEKPROEFGROOTTE Het verwachte verschil in effectgrootte tussen de condities is gebaseerd op de systematische review en wordt geschat op een middelgroot effect. Een powerberekening laat zien dat er 78 deelnemers nodig zijn om met 80% kans een middelgroot effect te detecteren met 25% dropout. SUMMARY -OBJECTIVE Posttraumatic Stress Disorder (PTSD) is a mental disorder with a major health and economic burden on patients, their relatives and society. National and international guidelines recommend trauma-focused psychotherapies as first-line treatments. Trauma-focused psychotherapies have proven to be efficacious and cost-effective, but two thirds of selected PTSD populations do not benefit sufficiently. We aim to assess the cost-effectiveness of a new intervention, high intensive motion-assisted psychotherapy, against conventional trauma-focused psychotherapy in the treatment of PTSD. -HYPOTHESIS Motion-assisted multi-modular exposure-based psychotherapy is expected to augment recovery processes and speed of recovery, will promote compliance and will therefore be superior in cost-effectiveness compared to usual care. -STUDY DESIGN Multicenter, single blind, parallel group randomized controlled trial comparing the cost-effectiveness of the new intervention to usual care in a superiority design. The experimental group receives 10 weekly sessions whereas the control group receives 16 weekly sessions. The primary outcome will be assessed weekly. Other assessments will take place pre- and post-treatment, after 3 and 6 months. -STUDY POPULATION Patients with work-related PTSD, recruited at Centrum ’45 and Mental Health Care Drenthe. -INTERVENTION The experimental condition consists of 7 sessions of manualized interactive motion-assisted multi-modular exposure-based psychotherapy, called 3MDR, followed by 3 sessions of trauma-focused psychotherapy. -USUAL CARE/COMPARISON The control condition consists of 16 sessions of manualized usual care, in the form of trauma-focused psychotherapy (TF-PT) consisting of either trauma-focused cognitive behavioral therapy or eye movement desensitization and reprocessing therapy. -OUTCOME MEASURES Self-reported PTSD (primary outcome), clinician-rated PTSD, health-related quality of life and costs associated with psychiatric illness, patient evaluations, avoidance, depressive symptoms and general functioning. -SAMPLE SIZE The expected difference in effect size between conditions is based on the systematic review and estimated at a medium effect. A power calculation shows that for an 80% chance of detecting a medium effect with 25% dropout, 78 subjects will be needed.
