Wat is staar?
Bij staar is de ooglens troebel geworden. Dit kan worden behandeld met een staaroperatie, waarbij de troebele lens wordt vervangen door een heldere kunstlens.
Onderzoek
In dit onderzoek werd de torische kunstlens bij staaroperaties onderzocht. Dit is een lens die naast bijziendheid of verziendheid ook een cilinderafwijking kan corrigeren. Deze lens werd vergeleken met een standaard kunstlens die alleen bijziendheid of verziendheid en géén cilinder afwijking kan corrigeren.
Resultaten
De resultaten laten zien dat patiënten met torische IOLs even tevreden zijn over de postoperatieve kwaliteit van zien als patiënten met monofocale IOLs. Patiënten met torische IOLs hebben wel een significant betere ongecorrigeerde visus bij vertezien, een lager refractie astigmatisme en gebruiken minder frequent een bril bij vertezien. De kosten van torische IOL zijn hoger in vergelijking met monofocale IOL. Hiermee concludeert u dat het gebruik van torische IOLs niet kosteneffectief vergeleken is met monofocale IOLs.

Samenvatting van de aanvraag
Doel/ vraagstelling: Een recente innovatie op het gebied van staaroperaties is de introductie van torische intraoculaire lenzen (IOLs), die postoperatief astigmatisme kunnen verminderen of zelfs voorkomen. Deze lenzen geven patiënten met een substantieel astigmatisme de mogelijkheid om een optimale visus voor afstand te bereiken zonder bril. Een goede leesvisus kan dan bereikt worden met een goedkope leesbril. De huidige werkwijze, het implanteren van niet-torische IOLs bij astigmate patiënten, leidt tot het gebruik van dure multi- of bifocale brillen met cilinder correctie om veraf en dichtbij scherp te kunnen zien. Een torische IOL implantatie kost ongeveer €400 meer dan een niet-torische IOL, waardoor implementatie van deze torische IOLs in de cataract chirurgie zal leiden tot een kostentoename van ongeveer €14.000.000. Voordat deze torische IOLs kunnen worden geïmplementeerd in de reguliere gezondheidszorg moeten de effectiviteit en kosteneffectiviteit worden bewezen in een gerandomiseerde studie. Studie opzet: Gerandomiseerde klinische studie. Studiepopulatie: De studiepopulatie bestaat uit 160 patiënten met cornea astigmatisme die een cataract operatie moeten ondergaan. Interventie: Cataract chirurgie met implantatie van een AcrySof torische IOL (model SN60TT) of een AcrySof monofocale IOL (model SN60WF). Deze twee lenzen hebben een identiek design, behalve dat de AcrySof torische lens is uitgerust met een cilindrische correctie. Uitkomstmaten: De primaire uitkomstmaat is de kwaliteit van vertezien zonder correctie. De secundaire uitkomstmaten zijn ongecorrigeerde visus (veraf), frequentie van het dragen van een bril voor vertezien, generieke gezondheidsgerelateerde kwaliteit van leven, complicaties na de staaroperatie en kosten postoperatieve bril en torische IOL implantatie (kosteneffectiviteit). Sample size berekening/data analyse: Power analyse toonde aan dat met betrekking tot de parameter kwaliteit van vertezien voor een verwachte score van goed tot uitstekend bij 40% in de monofocale groep en 70% in de torische groep, 80 patiënten in beide groepen nodig zijn. Voor de data analyse worden puntschatters en 95% betrouwbaarheidsintervallen berekend. Economische evaluatie: De economische evaluatie wordt uitgevoerd vanuit een maatschappelijk en gezondheidszorgperspectief met een tijdshorizon van 6 maanden. Tijdpad: Patiënten worden geïncludeerd gedurende de eerste 15 maanden van de studie, de follow-up bedraagt 6 maanden en data-analyse zal plaatsvinden in de laatste 3 maanden van de studie. Objective(s)/research question(s): Recent innovations in cataract surgery consist of the introduction of toric intraocular lenses (IOLs) that can correct postoperative astigmatism. It offers the opportunity for patients with substantial astigmatism to achieve optimal distance vision without using spectacles. Good reading vision may subsequently be achieved with low-cost reading glasses. The current practice of non-toric IOL implantation in astigmatic patients warrants the use of expensive bifocal or multifocal spectacles with cylinder correction to achieve good distance and near vision. The toric IOL costs about €400 more than a non-toric IOL and it is estimated that implementation of toric IOLs in cataract surgery could thus lead to a considerable increase in costs of about €14.000.000. Before these new toric IOLs can be implemented in the regular health care system the efficacy and cost-effectiveness have to be proven in a randomized clinical trial. Study design: Randomized clinical trial. Study population/datasets: The study population will consist of 160 patients with corneal astigmatism who require cataract surgery. Intervention: Cataract surgery with implantation of an AcrySof toric IOL (model SN60TT) or an AcrySof monofocal IOL (model SN60WF). These IOLs are identical in design except for the addition of a cylinder correction in the AcrySof toric IOL. Outcome measures: The primary outcome measure is the quality of distance vision without correction. The secondary outcome measures are uncorrected distance visual acuity, frequency of spectacle wear for distance vision, generic health related quality of life, complications and costs of postoperative spectacles and toric IOL implantation (cost-effectiveness). Sample size calculation/data analysis: Power analysis for the outcome measure quality of distance vision with expected scores good to excellent in 40% of patients with monofocal IOLs and 70% with toric IOLs showed that 80 patients need to be included in both the experimental and the control group. Data analysis will be performed using point estimates and 95% confidence intervals. Economic evaluation: An economic evaluation will be performed from both a societal and health care perspective with a time horizon of 6 months. Time schedule: Patients will be included during the first 15 months of the study. After a follow-up of 6 months, data analysis will be performed in the final 3 months.
