Prenatale diagnostiek: MLPA en/of karyotypering

Zwangeren met een hoog risico op een kind met Down syndroom (leeftijd, uitslag triple test, eerder kind met Down syndroom) kunnen een vruchtwaterpunctie laten verrichten. Dit onderzoek vindt plaats rond 15 weken zwangerschapsduur. Het vruchtwater bevat cellen van het kind die onderzocht worden op de aanwezigheid van afwijkingen in het aantal chromosomen. Het normale aantal is 46, een kind met Down syndroom heeft er 47. Het duurt ca. 3 weken voordat de utslag van dit onderzoek, het karyogram, bekend is. Sinds 2003 bestaat er een andere techniek, MLPA, die al binnen een paar dagen uitsluitsel geeft. In Leiden is de bruikbaarheid van MLPA in de praktijk onderzocht. De conclusie van het onderzoek luidt dat MLPA net zo betrouwbaar is, kosten bespaart en de voorkeur heeft van zeker de helft van de zwangeren.

Zwangeren met een hoog risico op een kind met Down syndroom kunnen een vruchtwaterpunctie laten verrichten. Dit onderzoek vindt plaats rond 15 weken zwangerschapsduur. In het vruchtwater wordt in de cellen van het kind beoordeeld hoeveel chromosomen aanwezig zijn. Het normale aantal is 46, een kind met Down syndroom heeft een extra choromosoom 21. De uitsalg van dit onderzoek, het karyogram, duurt 3 weken. Nieuwe moleculair biologische technieken, MLPA, hebben het mogelijk gemaakt alleen naar chromosoom 21 onderzoek te doen. Deze uitslag is na 2-4 dagen bekend. Dit onderzoek heeft aangetoond dat dat in dfe klinische praktijk goed mogelijk is, bovendien geeft het een kostenreductie van 433$. Zeker de helft van de zwangeren heeft voorkeur voor deze methode van onderzoek. De kwaliteit van leven blijft gelijk. Samenvattend is de conclusie dat MLPA het karyogram kan vervangen bij het vruchtwateronderzoek.

Samenvatting van de aanvraag

Background 1. For the past 30 years karyotyping is the gold standard in prenatal diagnosis for the detection of chromosomal aberrations in the fetus, in particular trisomy 21 (Down syndrome). 2. The main indications for traditional karyotyping (TKT) in the Netherlands are advanced maternal age and increased risk based on prenatal screening tests (PNS). 3. The annual numbers of maternal age-based invasive procedures for TKT are decreasing from 8803 (1999) to 7921 (2003). In contrast, the numbers of PNS are increasing from 15.000 (2003) to 80.000 (2005). This is due to a shift of the maternal age distribution, with a relative increase of elderly pregnant women making use of PNS, an increased demand of pregnant women, augmented by a change in government policy allowing PNS (combined test, triple test) for risk estimation of Down syndrome (DS). 4. MLPA (multiplex ligation-dependent probe amplification) is a new molecular genetic technique in prenatal diagnosis using amniotic fluid. It is a potential alternative test for detecting aneuploidies. 5. Compared to TKT, MLPA has 4 potential advantages: (1) the result is known in 2 days instead of 3 weeks, (2) the procedure is considerably less labour intensive, (3) the test requires less amniotic fluid (2-4 ml instead of 20 ml) (4) MLPA is suitable for high throughput testing. Previous preclinical evidence suggests equivalence of MLPA and TKT regarding test performance (accuracy in detection of common occurring chromosome aberrations). Research questions 1. The present study will establish the suggested equivalent preclinical diagnostic accuracy of MLPA in a clinical setting. 2. Psychological impact, costs, unexpected findings and patient’s preferences will be evaluated. Outcomes are calculated for the two main indications, advanced maternal age and increased risk after PNS. Study design 1. The principal design is a national prospective study. A cohort of an anticipated 6000 women is subjected to both MLPA and TKT (gold standard). MLPA by definition precedes TKT, but laboratory personnel involved in TKT will be blinded from MLPA outcome. 2. A sample size of 4500 amniocentesis suffices to establish equivalence accepting minimal inferiority of MLPA. Outcome measures 1. Primary outcome is expressed in standard measures of diagnostic accuracy. 2. Anxiety and distress as secondary outcome will be measured by available standardized questionnaires (HADS, PPC, IES, MOS-SF) which also have been used for similar studies (PhD Thesis Muller, 2006 in press; screening for irregular erythrocyte antibodies in pregnancy]. 3. Cost as secondary outcome will focus on medical costs (including implementation and quality control). If relevant the cost-effectiveness of TKT over MLPA will be calculated. 4. Using a commercially available kit (i.e. P095), trisomy 13, 18, 21 and sex chromosome abnormalities will be detected. Firstly, we will focus on detecting trisomy 21 and secondly, chromosomes 13, 18, X and Y. The latter can be of uncertain clinical relevance and cause counselling difficulties. A panel will assess these findings and will judge whether they are clinically relevant, irrelevant or of uncertain relevance. This might result in excluding/including probes from/into the MLPA aneuploidy kit in future testing. Differences in emerging unexpected or incomprehensible findings will be described qualitatively taking advantage of a recent study on this issue [Van Zwieten, 2004]. 5. A standard patient preference study (so called discrete choice evaluation) will allow quantification of the balance of all (dis)advantages of either test [Ryan, 2005].
