In moderne IVF is het ontdooien en plaatsen van eerder ingevroren embryo’s van steeds groter belang; door betere laboratoriumtechnieken worden veel meer embryo’s ingevroren en meer kinderen worden geboren uit een ‘cryo-embryo’.
Het moment van ontdooien en plaatsen in de baarmoeder wordt getimed aan de hand van de eisprong. De eisprong wordt in het ziekenhuis of thuis gemonitord. In het ziekenhuis gebeurt dit m.b.v. echo’s en opwekking van de eisprong door middel van een hormooninjectie (hCG). Bij thuismonitoring wordt de eisprong door de vrouw zelf met LH testen in de urine gedetecteerd waardoor minder belasting voor haar werk en privé leven.

Thuismonitoring wordt toegepast in Nederland waarbij de zwangerschapscijfers vergelijkbaar lijken met ziekenhuis monitoring. Het doel van deze studie is om te achterhalen of thuismonitoring niet leidt tot een lagere kans op zwangerschap bij cryo-embryoplaatsing in vergelijking met ziekenhuismonitoring en wat de tevredenheid van de vrouw is.

Samenvatting van de aanvraag

Samenvatting DOEL/VRAAGSTELLING: Bepalen van de kosten-effectiviteit van thuis- versus ziekenhuismonitoring om het juiste moment van plaatsen van een cryo embryo te bepalen. HYPOTHESE: Thuismonitoring van de ovulatie is niet inferieur echomonitoring in het ziekenhuis waarbij de ovulatie wordt opgewekt met een injectie. STUDIE OPZET: Multicentrum, non-inferioriteits RCT met een kosten-effectiviteitsanalyse. STUDIEPOPULATIE/DATABRONNEN: Vrouwen die een cryo cyclus ondergaan. INTERVENTIE: Jet moment van de ovulatie wordt door de vrouw zelf thuis met LH testen in de urine gedetecteerd (thuismonitoring met urine LH testen). GEBRUIKELIJKE ZORG /VERGELIJKING: Ziekenhuis monitoring van de cyclus m.b.v. echo’s en opwekking van de ovulatie door middel van een hormooninjectie (hCG). UITKOMSTMATEN: Doorgaande zwangerschap per vrouw na plaatsing van één cryo-embryo. SAMPLE SIZE BEREKENING/DATA-ANALYSE: we verwachten 12% kans op een doorgaande zwangerschap bij ziekenhuis monitoring per vrouw per gestarte cyclus. Om te evalueren of thuismonitoring geen lagere kans op zwangerschap geeft (4% non inferioriteits marge), zijn 1390 vrouwen nodig (power 80%, alfa fout 0.05). We moeten 1464 vrouwen randomiseren in verband met de verwachte ‘lost to follow up’ van 5%. KOSTENEFFECTIVITEITSANALYSE/ BUDGET IMPACT ANALYSE: Als thuismonitoring kosten-effectief is, dan levert dit een jaarlijkse kostenbesparing van 3 tot 4 miljoen Euro op. TIJDPAD: Totaal 60 maanden waarvan 12 maanden voorbereiding, 36 maanden inclusie, 10 maanden follow up, 2 maanden data analyse en rapportage. 
Summary OBJECTIVE(S)/RESEARCH QUESTION(S): Assessing cost-effectiveness of home based monitoring with urinary LH tests (true natural cycle frozen embryo transfer, true NC-FET) versus hospital controlled monitoring with repeated ultrasound monitoring and hCG trigger (modified NC-FET) to time frozen embryo transfer in IVF. HYPOTHESIS: True NC-FET is non-inferior to modified NC-FET in ongoing pregnancy rate after one FET cycle. STUDY DESIGN: Multicentre, non-inferiority RCT with a cost-effectiveness analysis. STUDY POPULATION(S)/DATASETS: Women that have undergone IVF/ICSI who are scheduled for frozen embryo transfers. INTERVENTION: True natural cycle FET with home-based monitoring of ovulation with urinary LH tests USUAL CARE /COMPARISON: Modified NC-FET with repeated ultrasound monitoring of the dominant follicle followed by hCG trigger. OUTCOME MEASURES: Ongoing pregnancy per woman after a single FET cycle. SAMPLE SIZE CALCULATION/DATA ANALYSIS: We expect a 12% ongoing pregnancy rate per started cycle per woman after modified NC FET. To evaluate whether true NC-FET does not result in a decrease of ongoing pregnancy rate of 4% (non-inferiority design), we need 1390 patients with a power of 80% and an alpha error of 0.05. Anticipating 5% lost to follow up we need to randomise 1464 women. COST-EFFECTIVENESS ANALYSIS/ BUDGET IMPACT ANALYSIS: If home-based monitoring is cost-effective, an annual cost reduction of 3 to 4 million Euro can be achieved. TIME SCHEDULE: A total of 60 months, consisting of 12 months preparation, 36 months for recruitment, 10 months for follow up and 2 months data analysis and reporting.
