Face-to-face vs Online Hypnotherapy for the treatment of Irritable Bowel Syndrome (FORTITUDE)

Het prikkelbare darmsyndroom (PDS) is één van de meest voorkomende oorzaken van langdurige buikklachten. De onderliggende oorzaak van PDS is nog niet helemaal duidelijk, waardoor de behandeling vaak moeilijk is. Een van de behandelingen die wel effectief is gebleken voor PDS is hypnotherapie. Bij deze behandeling wordt via diepe ontspanning door een therapeut de focus gelegd op een gezond lichaam. Conventionele hypnotherapie heeft nadelen: de patiënt moet meerdere keren een afspraak maken met een hypnotherapeut, dit kost tijd en ook geld. Online hypnotherapie lijkt in dit opzicht aantrekkelijk en kan ook de drempel verlagen voor de patiënt om deze behandeling te ondergaan. Het doel van dit onderzoek is de werkzaamheid van hypnotherapie (in de vorm van online hypnotherapie en hypnotherapie gegeven door een therapeut) te vergelijken. Verder zal er gekeken worden naar langetermijneffecten en kosten van de behandelingen.

Samenvatting van de aanvraag

DOEL/VRAAGSTELLING 1. Om de kosteneffectiviteit van face-to-face hypnotherapie vs. online hypnotherapie te vergelijken in patiënten met prikkelbare darmsyndroom (PDS). 2. Om de effectiviteit van hypnotherapie (ongeacht de vorm waarin het wordt toegediend) te bevestigen vs. online psychoeducatie bij patiënten met PDS HYPOTHESE 1. Online hypnotherapie heeft vergelijkbare (niet-inferieure) werkzaamheid t.o.v. face-to-face hypnotherapie, gegeven door een hypnotherapeut, maar is kosteneffectiever. 2. Hypnotherapie (in welke vorm dan ook) is superieur aan online psychoeducatie. STUDIE OPZET Gerandomiseerde studie met drie armen (psychoeducatie, face-to-face hypnotherapie, online hypnotherapie) gedurende 12 weken met een follow-up van 6 maanden. STUDIEPOPULATIE Patiënten met PDS zonder ernstige psychiatrische ziekten. INTERVENTIE EN VERGELIJKING Hypnotherapie (face-to-face en online) wordt aangeboden in een reeks van 6 individuele, twee-wekelijkse sessies. Psychoeducatie bestaat uit meerdere onderdelen die gedurende 12 weken beschikbaar worden gesteld. UITKOMSTMATEN Primaire uitkomstmaten: responspercentage 4 weken na beëindigen van de behandeling gemeten aan de hand van de dagelijkse buikpijnscore en adequate verlichting van de algemene symptomen (volgens FDA/EMA definitie). Secundaire uitkomstmaten: ernst van symptomen (IBS-SSS), kosteneffectiviteit, bijwerkingen, therapietrouw, kwaliteit van leven. SAMPLE SIZE BEREKENING/DATA-ANALYSE De berekening van de groepsgrootte is gebaseerd op een non-inferioriteitsdesign, met een verwacht responspercentage van respectievelijk 60% en 50% in de face-to-face hypnotherapie en de online hypnotherapie groep. De non-inferioriteitsmarge werd gedefinieerd als 10%. Met een correctie voor drop-out resulteert dit in een totale steekproefomvang van 243. Data analyse zal worden verrichten volgens het per protocol principe. KOSTENEFFECTIVITEITSANALYSE / BUDGET IMPACT ANALYSE Kosteneffectiviteitsanalyse en kostenutiliteitsanalyse zal worden uitgevoerd vanuit maatschappelijk en gezondheidszorg perspectief volgens Nederlandse richtlijnen over een tijdspan van 6 maanden met extrapolatie van de data naar 12 maanden. TIJDPAD Sept. 2018-Febr. 2019 (6 mnd): Aanvraag ethische goedkeuring. Design van IT-infrastructuur. Febr. 2019-Febr. 2022 (36 mnd): Inclusieperiode. Mrt. 2022-Aug. 2022 (6 mnd): Einde follow-up. Analyse en publicatie van de resultaten. OBJECTIVES 1. To examine whether the effectiveness of online hypnotherapy is non-inferior compared to individual face-to-face hypnotherapy delivered by a therapist and more cost saving. 2. To re-establish the efficacy of gut-directed hypnotherapy (regardless of the form in which it is delivered) vs. psychoeducation administered online for the treatment of IBS. HYPOTHESIS: 1. Hypnotherapy delivered in the form of online exercises has comparable (non-inferior) efficacy to hypnotherapy exercises delivered by a hypnotherapist but is more cost-effective. 2. Hypnotherapy (in either form) is superior to online administered psychoeducation in IBS patients. STUDY DESIGN: Randomized controlled trial with three arms (psychoeducation, face-to-face hypnotherapy, online hypnotherapy) for 12 weeks with a follow-up of 6 months. STUDY POPULATION Patients with IBS aged 16-65 years without severe psychiatric disease. INTERVENTION AND COMPARISON Hypnotherapy (face-to-face and online) will be delivered in a series of 6 individual, bi-weekly sessions. Psychoeducation will be available for 12 weeks. OUTCOME MEASURES Primary co-primary outcome measures: abdominal pain response rate and adequate relief of global symptoms 4 weeks following completion of the treatment period (according to FDA/EMA endpoints). Secondary outcomes measures: symptom severity (IBS-SSS), cost-effectiveness, side effects, adherence to therapy, quality of life. SAMPLE SIZE CALCULATION/DATA ANALYSIS Sample size calculation is based on a non-inferiority design, with an anticipated response rate of 60% and 50% in the face-to-face hypnotherapy and the online hypnotherapy, respectively. The non-inferiority margin was defined as 10%. Appling an additional correction for drop-outs results in an overall sample size of 243. The primary data analysis will be performed per protocol. COST-EFFECTIVENESS ANALYSIS/ BUDGET IMPACT ANALYSIS Cost-effectiveness analysis and cost-utility analysis will be performed from a societal and healthcare perspective according to Dutch guidelines with a time horizon of 6 months with extrapolation of results to 12 months.
