Cost-effectiveness of early fixation versus conservative therapy of multiple, simple rib fractures (FixCon); a multicenter Randomized Controlled Trial

Vroege fixatie versus conservatieve behandeling van meerdere, enkelvoudig gebroken ribben (FixCon); een multicenter onderzoek.

Gebroken ribben komen vaak voor en kunnen ernstige gevolgen hebben. Als meerdere ribben gebroken zijn kan de borstkas minder goed werken en doet het pijn. Hierdoor kunnen patiënten problemen krijgen met ademhalen. Van oudsher werden patiënten zonder operatie behandeld, tegenwoordig wordt steeds vaker geopereerd. In dit onderzoek wordt vergeleken welke van deze behandelingen de beste resultaten geeft. Dit onderzoek is opgezet door het Erasmus MC, afdeling Traumachirurgie en wordt uitgevoerd in verschillende Nederlandse ziekenhuizen. Er zijn 180 deelnemers nodig.

Gedurende een jaar worden de deelnemers lichamelijk onderzocht, hun longfunctie wordt gemeten en deelnemers wordt verzocht vragenlijsten in te vullen over de gezondheid en herstel.

Samenvatting van de aanvraag

DOEL / VRAAGSTELLING Is operatieve ribfixatie kosten-effectiever dan niet-operatieve behandeling van volwassen patiënten met meerdere enkelvoudige ribfracturen na stomp trauma? HYPOTHESE Operatieve ribfixatie is kosten-effectiever, vooral door reductie van pneumonie, opnameduur en werkverzuim. STUDIE OPZET Multicenter gerandomiseerde studie met kosten-effectiviteits- en kosten-utiliteitsanalyse. STUDIEPOPULATIE / DATABRONNEN Volwassenen met drie of meer enkelvoudige ribfracturen. INTERVENTIE Open reductie en interne fixatie van gebroken ribben met het Matrix Rib Systeem. Indien nodig worden pijn- en inhalatiemedicatie, zuurstoftoediening of mechanische ventilatie en longfysiotherapie gegeven. GEBRUIKELIJKE ZORG / VERGELIJKING Niet-operatieve behandeling gericht op complicatiepreventie en comfort, bestaande uit pijn- en inhalatiemedicatie, zuurstoftoediening of mechanische ventilatie en longfysiotherapie. UITKOMSTMATEN Primaire uitkomstmaat is pneumonie binnen 30 dagen. Secundaire uitkomstmaten zijn mechanische ventilatie, longfunctie, opnameduur, complicaties en mortaliteit, kwaliteit van leven (EQ-5D), zorggebruik en directe in indirecte medische kosten. Patiënten worden 12 maanden gevolgd. Kosten per voorkomen pneumonie en per gewonnen QALY worden bepaald. SAMPLE SIZE BEREKENING / DATA-ANALYSE N=180 (90 per group), gebaseerd op een verwacht percentage pneumonie van 35% bij niet-operatieve behandeling en 15% bij operatieve behandeling, een alpha van 0.5 (2-zijdig), 80% power en 25% verwachte loss to follow-up en mortaliteit. Statistische analyse op intention-to-treat basis zal bestaan uit standard univariate en multivariate toetsen en berekening van de kosten per voorkomen pneumonie en kosten per gewonnen QALY. KOSTENEFFECTIVITEITSANALYSE / BUDGET IMPACT ANALYSE De economische evaluatie wordt uitgevoerd vanuit een maatschappelijk perspectief. Kosten-effectiviteit (o.b.v. pneumonie) en kosten-utiliteit (o.b.v. EQ-5D) zullen bepaald worden. Onzekerheid zal geschat worden geschat d.m.v. bootstrapping en gepresenteerd worden in ‘cost-effectiveness planes’ en ‘acceptability curves’. Sensitiviteitsanalyses zullen uitgevoerd worden voor onzekere parameters. Bij de budget impact analyse zullen Nederlandse epidemiologische data en Markov modellen gebruikt worden. Het maatschappelijk, gezondheidszorg- en verzekeraarsperspectief wordt meegenomen. TIJDPAD Maand 0-3: voorbereiding Maand 1-30: inclusie Maande 1-42: follow-up Maand 42-48: analyse en implementatie RESEARCH QUESTION Is operative rib fixation more cost-effective than nonoperative treatment of multiple simple rib fractures after blunt trauma in adults? HYPOTHESIS Operative rib fixation is more cost-effective, mainly due to less pneumoniae, hospital stay, and work absence. STUDY DESIGN Multicenter randomized controlled trial with cost-effectiveness and cost-utility analysis. STUDY POPULATION / DATASETS Adults who sustained three or more simple rib fractures. INTERVENTION Open reduction and internal fixaton of fractured ribs with the Matrix Rib System. If needed, pain and inhalation medication, oxygen therapy or mechanical ventilation, and pulmonary physical therapy will be given. USUAL CARE / COMPARISON Nonoperative treatment, aimed at complication prevention and comfort, consisting of pain and inhalation medication, oxygen therapy or mechanical ventilation if needed, and pulmonary physical therapy. OUTCOME MEASURES The primary outcome measure is pneumonia within 30 days. Secondary outcome measures are mechanical ventilation, pulmonary function, hospital stay, complications and mortality, quality of life (EQ-5D, health care consumption, and direct and indirect medical costs. Patients are followed for 12 months. Costs per pneumonia prevented and per QALY gained will be determined. SAMPLE SIZE CALCULATION / DATA ANALYSIS N=180 (90 per group), based on expected pneumonia rates of 35% in nonoperatively treated patients and 15% in operated patients, an alpha of 0.05 (2-sided), 80% power, and 25% expected loss to follow-up and mortality. Statistical analysis based on intention-to-treat will include standard univariate and multivariable tests and calculation of costs per pneumonia prevented and costs per QALY gained. COST-EFFECTIVENESS ANALYSIS / BUDGET IMPACT ANALYSIS The economic evaluation will be conducted from a societal perspective. Coste-effectiveness (pneumonia within 30 days) and cost-utility (EQ-5D) will be determined. Uncertainty will be estimated using bootstrapping. Cost-effectiveness planes and acceptability curves will be presented. Sensitivity analyses will be conducted on uncertain parameters. The budget impact analysis will use Dutch epidemiological data and Markov models. The societal, health care, and insurer perspectives will be considered in the analysis.
