Wondinfecties na operaties komen regelmatig voor en leiden tot pijn, ongemak en ziektelast voor de patiënt. De behandeling van wondinfecties is belastend en bestaat uit frequente reiniging, bijvoorbeeld door het uitspoelen van de wond met water en vervolgens verbinden met gazen of door het gebruik van negatieve druktherapie. Er is een nieuwe behandeling die negatieve druktherapie combineert met een vloeistof-instillatiesysteem. Bij deze behandeling wordt geïnfecteerd weefsel in de wond automatisch losgeweekt. Het zou de wondzorg voor zowel de patiënt als zorgpersoneel minder belastend maken, maar het is nog onduidelijk of deze behandeling beter is dan de huidige wondzorg.

In dit onderzoek bestuderen we daarom of negatieve druktherapie, gecombineerd met een vloeistof-instillatiesysteem, leidt tot betere uitkomsten dan standaard wondzorg (standaard verbandmaterialen of standaard negatieve druktherapie zonder vloeistof-instillatiesysteem) bij wondinfecties na operaties.

Samenvatting van de aanvraag

SAMENVATTING NEDERLANDS VRAAGSTELLING: Leidt negatieve druktherapie (NDT) met een vloeistof-instillatiesysteem tot een snellere wondgenezing dan standaard wondzorg bij postoperatieve geïnfecteerde wonden? HYPOTHESE: Het gebruik van NDT met een vloeistof-instillatiesysteem leidt tot een snellere wondgenezing bij postoperatieve geïnfecteerde wonden dan standaard wondzorg. STUDIE OPZET: Patiënten worden geloot voor één van de beide behandelopties. Dit onderzoek zal in meerdere ziekenhuizen plaatsvinden. STUDIEPOPULATIE: Volwassen patiënten met een postoperatieve geïnfecteerde wond. INTERVENTIE: NDT met een vloeistof-instillatiesysteem. Een schuimverband wordt in de wond gebracht en daarop wordt een pomp aangesloten. Deze pomp zuigt continu lucht en zorgt voor afvoer van overtollig wondvocht naar een opvangbeker. Hierdoor ontstaat een negatieve druk in de wond (125 mmHg). Dezelfde pomp brengt ook iedere 2,5 – 3,5uur een hoeveelheid vloeistof in de wond om de wond te reinigen. Dit blijft vervolgens 15 minuten in de wond. Na deze 15 minuten gaat de pomp weer actief lucht wegzuigen. Op deze manier wordt de wond tussentijds gespoeld en geweekt waardoor de infectie sneller zou moeten verdwijnen. De pomp blijft 24 uur per dag aangesloten en het verband zal 1x per 2 á 3 dagen verwisseld worden. GEBRUIKELIJKE ZORG/VERGELIJKING: Standaard wondzorg. De geïnfecteerde wond wordt bedekt met een verbandmateriaal of met NDT zonder een spoelsysteem. Dit kan bijvoorbeeld bestaan uit alginaat-, hydrofiber en/of siliconenverbanden óf een combinatie van verbandmaterialen. Om de infectie te bestrijden wordt er een ontsmettingsmiddel toegevoegd, bijvoorbeeld jodium UITKOMSTMATEN: De belangrijkste uitkomstmaat is tijd tot complete wondgenezing. Overige uitkomstmaten zijn pijn, opnameduur; heropnames; extra operaties; kwaliteit van leven; tevredenheid van patiënten en zorgverleners; duur en frequentie verbandwisselingen; hoeveelheid thuiszorg, kosten en het verschil in tijd tot complete wondgenezing binnen subgroepen (operatief gesloten vs. niet operatief gesloten). SAMPLE SIZE BEREKENING/DATA-ANALYSE: 201 patiënten per groep, in totaal 402 patiënten om een 14-dagen snellere wondgenezing aan te tonen. KOSTENEFFECTIVITEITSANALYSE/ BUDGET IMPACT ANALYSE Een kosteneffectiviteits (KEA)- en kosten utiliteitsanalyse (KUA) worden uitgevoerd. Het primaire eindpunt voor de KEA zijn de kosten die bespaard worden per dag snellere wondgenezing. Het primaire eindpunt zijn de kosten per extra quality adjusted life year (QALY). Met een budget impact model zullen de financiële gevolgen worden vastgesteld. TIJDPAD: De studie duurt 4 jaar; 30 maanden werving van patiënten; 12 maanden follow-up; 6 maanden data-analyse en verslag. SUMMARY ENGLISH RESEARCH QUESTION: Does the use of Negative Pressure Wound Therapy with Instillation (NPWTi) lead to a faster wound healing compared to standard wound care in patients with a postoperative wound infection (POWI)? HYPOTHESIS: Time to complete wound healing in patients with a POWI who receive NPWTi will be shorter than in patients receiving standard wound care (i.e. NPWT and/or conventional dressings). STUDY DESIGN: A multicenter randomized controlled trial. Patients will be randomized to (1) NPWTi or (2) standard wound care. STUDY POPULATION: Patients with a POWI INTERVENTION: NPWTi. The wound will be covered with an open-cell foam and an occlusive drape. Every 2.5-3.5 hours (depending on the bioburden of the wound), the wound bed will be automatically soaked with 0.9% normal saline for 15 minutes, followed by a negative pressure cycle at 125 mmHg. The foam plus occlusive drape application will remain in place for 2 to 3 days. USUAL CARE/COMPARISON: Standard wound care. This involves the use of NPWT and/or conventional dressings. NPWT involves the same treatment procedure as NPWTi, except without the topical delivery of a wound treatment solution. Conventional dressings are gauze-based or occlusive dressings and will be used until the wound is completely healed. OUTCOME MEASURES: Primary outcome: Time to complete wound healing. Secondary outcomes: pain; length of stay; readmissions; additional surgical procedures; quality of life; patient-and professional satisfaction; duration and frequency of total wound care, amount of homecare; costs; time to complete wound healing (subgroup: surgically closed wounds vs. healing by secondary intention) SAMPLE SIZE/ DATA-ANALYSIS: 201 patients per group, totaling 402 patients to detect a 14-day quicker wound healing. COST-EFFECTIVINESS ANALYSES/ BUDGET IMPACT ANALYSIS: The economic evaluation will be performed as a cost effectiveness and cost utility analysis. The primary outcome in the CEA will be costs per day shorter wound healing. The primary outcome in the CUA will be costs per QALY. A budget impact model will be used to estimate the financial consequences. TIME SCHEDULE: The study will take 4 years; patient recruitment 30 months; follow-up 12 months; data analysis and report 6 months.
