Dementie terug in de gemeenschap; een studie naar kwaliteit van leven van mensen met dementie

Zorggroep Elde Maasduinen heeft een nieuw verpleeghuis gebouwd waarin mensen met dementie vrij kunnen bewegen, anderen kunnen ontmoeten en hun eigen leefpatroon kunnen behouden binnen de lokale gemeenschap.
Doel
Het doel van dit project is het monitoren van activiteiten en veranderingen bij mensen met dementie, familieleden, verplegend personeel, vrijwilligers en mensen uit de lokale gemeenschap door middel van een procesevaluatie en een voor-na ontwerp, in vergelijking met andere locaties waar deze veranderingen niet zijn doorgevoerd. nog niet geïmplementeerd.
Aanpak
Er worden gemengde methoden gebruikt, waaronder een analyse van activiteitenpatronen van personen met dementie via individuele tags en sensoren, observaties, analyse van zorggegevens uit individuele zorgplannen en enquêtes.
Resultaten en aanbevelingen
De resultaten van dit onderzoek hebben een dieper inzicht verschaft in de interactiepatronen tussen personen met dementie en hun omgeving en hoe deze interacties verband houden met de gezondheid en kwaliteit van leven van mensen met dementie en de sociale cohesie in de lokale gemeenschap.
Dit onderzoek liet zien dat het maken van de omslag naar meer vrijheid voor bewoners met dementie complex is en wordt beïnvloed door factoren op proces-, organisatie- en omgevingsniveau. Enkel meer vrijheid bieden aan mensen met dementie is onvoldoende om de gewenste omslag naar een dementievriendelijke omgeving te maken.
De omslag vraagt:
- een investering zodat medewerkers gefaciliteerd worden om hun werk op een andere wijze in te richten;
- een duidelijke visie van de zorgorganisatie;
- dat de verschillende externe stakeholders het concept begrijpen, ondersteunen en delen;
- dat ruimtes worden gecreëerd waar mensen met dementie en buurtbewoners elkaar fysiek kunnen ontmoeten.
Dit onderzoek heeft inzicht gegeven in de complexe processen van organisatieverandering in de langdurige zorg, waarvan andere zorgorganisaties kunnen leren.

Samenvatting van de aanvraag
Park Vossenberg is a long-term care organization that is going to rebuild the facilities and redesign the surrounding park area to make it possible that residents with dementia freely use the outdoor park area. There will be no gates and people from the surrounding residential area will also use the park. Our research project will accompany this change in care for people with dementia. Elderly people in long-term care facilities have complex care needs that often derive from multiple health problems. This is especially true for persons with dementia. Due to their health problems, persons with dementia in long-term care facilities often live in closed units and are often not allowed to move around freely inside and outside the facility without supervision. This impacts their possibilities for social interaction and participation in the community, and limits their possibilities for physical activity. Consequently, leading to a lower quality of life. At this moment long-term care in the Netherlands is being restructured. Long-term care is to be provided mainly at home and in a large part by informal caregivers. Yet, due to the organization of care for persons with dementia over the last decades in homes for the elderly and in closed units in long-term care facilities, persons with dementia have largely disappeared from society. People in the community often don’t know how to interact with persons with dementia and how to help them. With the estimated increase in persons that will suffer from dementia in the near future and the current policy that elderly persons with dementia stay at home in their own environment, this poses important challenges to Dutch society and the way we provide care to our loved ones with dementia. Our research accompanies changes in a care organization, Maasduinen–location Park Vossenberg, which is in the process of integrating a long-term care facility into the community by creating a new living environment for persons with dementia who need long-term care, including a park that can be entered freely by both persons with dementia and people in the local community. The changes of the organization focus on 1) social cohesion, 2) making your own choices as a person with dementia, and 3) increasing quality of life. With the new living environment Maasduinen aims to connect clients, family members, informal caregivers, nursing staff, volunteers and all other people in the local community, and to bring long-term care back in the heart of the community. The purpose of this project is to monitor activities and changes in persons with dementia, family members, nursing staff, volunteers and people from the local community through a process evaluation and a before-after design with comparison to other locations where these changes have not been implemented yet. Mixed methods are used, including an analysis of activity patterns of persons with dementia through individual tags and sensors, observations, analysis of care data from individual care plans, and surveys. The results of this study will provide a deeper understanding of interaction patterns between persons with dementia and their environment and how these interactions are related to health and quality of life of people with dementia and social cohesion in the local community. In addition, both in the policies of Maasduinen and in our research project ethical issues will be taken into account, related to the use of monitoring devices and safety related outcomes. Secondary outcomes concern new methods for monitoring social interaction patterns using monitoring devices.
