HeAlthy Ageing eco-system for peopLe with dementia

The world is facing similar challenges in respect to the ageing population and shortages of care personnel, with dementia as the largest and one of the most expensive challenges. Across Europe and beyond, various dementia friendly eco-systems are in place within municipalities with AAL services to empower clients, informal carers and care professionals. With the HAAL project, The Netherlands, Taiwan, and Italy combine their strength, co-create, evaluate and share their experiences in supporting dementia care by state-of-the-art AAL bundles. Primary end-users (people with dementia (PwD) from MCI to late stages), secondary- (formal and informal carers, including social care), and tertiary- (management, payers/buyers) will be involved from the start of the project in co-design, business modelling, responsible innovation and evaluation in the later phases of the project. Over 80 end-users will be involved in co-creation, meaningful try-outs and formative evaluations. In the final evaluation, 90 end-users will be involved in 3 pilots (30 per country), consisting of triads of PwD and their (in)formal carers. Unique to HAAL is a that a careful selection of International AAL services can support dementia care over the various stages of dementia. HAAL provides a complete package of AAL-technology for people with dementia through all stages in a patient journey starting from the stage of Mild Cognitive Impairment (MCI, global deterioration scale, GDS stage 3) till the phase of severe dementia (GDS stage 7). Within the project, multiple existing AAL technologies - that are ready for upscaling - will be tried-out, implemented, and tested, such as solutions from the following AAL projects: RESILIENT, FreeWalker, eWare, Rosetta, i-evAALution, 2PCS & FearLess and also non-AAL project services that have just been introduced within AAL from Taiwan on rehabilitation, exergaming, monitoring and sensor networks. Adaptable to each stage, AAL based care technology that cannot be used anymore in a certain stage will be left out, e.g., due to decreasing cognitive functioning, while other AAL services will be introduced that better support the situation at that stage. During the whole dementia process there is one distribution channel, one contact and helpdesk for the total package that supports integrated care over the dementia client journey. The company - leading commercial partner in each country - is pro-active towards the customer - in practice mainly the informal care - in adapting the package to the current situation. Unsupervised and supervised Machine Learning (ML) models will be analysed to create descriptive and predictive models to perform behaviour and health analysis, using the data from the middleware under certain privacy restrictions and also under restrictions for customer protection. De wereld staat voor vergelijkbare uitdagingen met betrekking tot de vergrijzing en het tekort aan zorgpersoneel, met dementie als de grootste en een van de duurste uitdaging. In Europa en daarbuiten zijn er binnen gemeenten verschillende dementievriendelijke ecosystemen ontstaan met AAL-diensten om cliënten, informele zorgverleners en zorgprofessionals te ondersteunen. Met het HAAL-project combineren Nederland, Taiwan en Italië hun krachten, co-creëren, evalueren en delen ze hun ervaringen bij het ondersteunen van dementiezorg door middel van bewezen effectieve AAL-bundels. Primaire eindgebruikers (mensen met dementie (PwD) van MCI tot late stadia), secundaire (formele en informele zorgverleners, inclusief sociale zorg) en tertiaire (management, financiers / kopers) zullen vanaf de start van het project worden betrokken in co-design, business modelling, verantwoorde innovatie en evaluatie in de latere fasen van het project. Meer dan 80 eindgebruikers worden betrokken bij co-creatie, zinvolle try-outs en formatieve evaluaties. Bij de eindevaluatie worden 90 eindgebruikers betrokken in 3 pilots (30 per land), bestaande uit triades van PwD en hun (in) formele verzorgers. Uniek aan HAAL is dat een zorgvuldige selectie van Internationale AAL-diensten de dementiezorg in de verschillende stadia van dementie kan ondersteunen. HAAL biedt een compleet pakket van AAL-technologieen voor mensen met dementie in alle stadia van hun client journey, beginnend vanaf het stadium van milde cognitieve stoornissen (MCI, globale verslechteringsschaal, GDS-stadium 3) tot de fase van ernstige dementie (GDS-stadium 7) . Binnen het project worden meerdere bestaande AAL-technologieën - die klaar zijn voor opschaling - uitgeprobeerd, geïmplementeerd en getest, zoals oplossingen uit de volgende AAL-projecten: RESILIENT, FreeWalker, eWare, Rosetta, i-evAALution, 2PCS & FearLess en ook niet-AAL-projectdiensten die zojuist vanuit Taiwan binnen AAL zijn geïntroduceerd op het gebied van rehabilitatie, exergaming, monitoring en sensornetwerken.
