In-vitro generated leukemia-reactive T cells for the treatment of patients with refractory B cell malignancies

Stamceltransplantatie met stamcellen van een gezonde donor is een belangrijk onderdeel van de behandeling van patiënten met leukemie of lymfeklierkanker. Bij deze vorm van transplantatie worden bloedcelvormende stamcellen van de patiënt vervangen door donorcellen. Het is gebleken dat de afweercellen (T cellen) van de donor een belangrijke rol spelen in het anti-tumor effect van de transplantatie omdat ze in staat zijn om de tumorcellen van de patiënt als vreemd te herkennen op eenzelfde manier zoals dit gebeurt tijdens een afweerreactie na bijvoorbeeld een virale infectie. In dit project zullen patiënten met bepaalde vormen van leukemie (chronische lymfatische leukemie of acute lymfatische leukemie) die niet reageren op andere therapie, behandeld worden met in het laboratorium gegenereerde tumor-specifieke T cellen.

Samenvatting van de aanvraag

Allogeneic stem cell transplantation (alloSCT) has been successfully applied in the treatment of patients with hematological malignancies. Patients with persistent or recurrent disease after the transplantation can be treated with adoptive cellular immunotherapy using donor lymphocyte infusions (DLI) from their stem cell donors. This treatment has been found to be highly effective in the majority of patients with relapsed chronic myeloid leukemia (CML) in chronic phase. However, in patients with chronic lymphocytic leukemia (CLL) or in patients with more rapidly progressing diseases like acute myeloid leukemia (AML), acute lymphoblastic leukemia (ALL) and CML in accelerated phase or blast crisis, remissions were achieved in only 10-30% of the cases. This is probably due to the limited immunogenicity of these leukemic cells, and the lack of specificity of the donor T cells resulting in concurrent development of graft-versus-host disease (GvHD). These limitations can be overcome by the in-vitro generation of leukemia-reactive T cells and the adoptive transfer of these cells into patients with relapsed leukemia after alloSCT. Although the potential clinical efficacy of this approach has been demonstrated previously by us and others, new strategies are needed for the efficient and reproducible induction and selection of leukemia-reactive T cells with a good probability to survive in-vivo and to generate persistent immunological memory. The aim of this study is to in-vitro induce leukemia-reactive T cells from the naïve donor T cell repertoire by stimulation with in-vitro generated leukemic antigen presenting cells (APC) of the patient and to select the responding T cells by isolation based on the cell surface expression of the T cell activation marker CD137. A phase I/II clinical trial will be conducted to explore the safety and possible efficacy of the adoptive transfer of these in-vitro generated leukemia-reactive T cells to patients suffering from the B cell malignancies acute lymphoblastic leukemia (ALL) or chronic lymphocytic leukemia (CLL) who are refractory to conventional therapies. These patients will be transplanted with CD34 selected stem cells from an allogeneic donor followed by the application of donor-derived leukemia-reactive T cells at 6-9 weeks after the transplantation.
