Clinical introduction online and real-time MRI guided prostate cancer radiotherapy

In het UMC Utrecht is sinds kort een nieuwe bestralingstechniek voor de behandeling van prostaatkanker. De behandeling met het nieuwe apparaat (Elekta Unity) is een combinatie van een bestralingstoestel en een diagnostische MRI-scan. De Unity maakt het mogelijk om tijdens de bestraling met behulp van MRI-beelden exact te bepalen waar de prostaat zich bevindt. Hierdoor kan de patiënt met prostaatkanker nauwkeuriger worden bestraald en blijft het gezonde weefsel beter gespaard. Deze manier van bestralen zal naar verwachting leiden tot nog minder bijwerkingen dan de huidige behandelingen voor prostaatkanker. Door de nauwkeurigheid van de Unity kan een grotere hoeveelheid straling per keer worden gegeven. Dit betekent dat patiënten minder vaak naar het ziekenhuis moeten voor een behandeling. Het is de verwachting dat de bestraling uiteindelijk in één week gegeven kan worden waardoor de behandeling ook goedkoper zal zijn.

Samenvatting van de aanvraag

Prostate cancer is the most common cancer in men worldwide; survival rates are high due to the typically non-aggressive nature of disease and effective treatments. Radical treatments such as surgery and radiotherapy often cause toxicity and long term side effects. With modern day radiotherapy a substantial amount of normal tissue around the prostate is exposed to radiation and this frequently leads to bowel and urinary complaints. Surgery, even minimally invasive robotic procedures are regularly associated with incontinence and impotency. The new MRI-Linac system developed at the UMC Utrecht, in collaboration with Elekta and Philips, provides radiation delivery under high precision MRI-visualisation. The concept of seeing while you treat enables reduction of treatment uncertainty margins around the prostate and toxicity is lowered as less healthy tissue is exposed to radiation. The entire treatment will be performed through several outpatient hospital visits; this benefits patient comfort and cost-efficiency. The non-invasive MRI-linac holds the promise to become standard of care for prostate cancer treatment. It is estimated that on an annual basis 5000 patients in the Netherlands may be candidates for this new treatment modality in several highly specialised prostate cancer centres.
