Prognostic factors in TBI patients with DAI: a retrospective cohort study

Dit onderzoek is een samenwerking tussen het ETZ en het UMC Groningen.
Bij diffuus axonale schade (DAI) zijn de zenuwuitlopers (axonen) in de hersenen beschadigd na
een ernstig ongeval. De prognose is nog onduidelijk.

Literatuur

Patiënten mét DAI na een ongeval hebben een ongunstigere uitkomst. Maar in het algemeen heeft
52% van de patiënten met DAI een gunstige uitkomst. Bij elke stijging in graad o.b.v. de MRI-scan
wordt de kans op een ongunstige uitkomst 3 keer groter.

Retrospectief

Van 2008-2014 werden 177 patiënten met DAI gediagnostiseerd (ETZ en UMCG), 51% heeft een
gunstige uitkomst. Het herstel van bewustzijn ≤7 dagen, de pupilreactie en Marshall score (een CT
beoordeling) hebben een relatie met de uitkomst. 62% heeft een goede kwaliteit van leven, de
gradering van DAI bleek hiervoor een voorspeller.
37% had een DAI graad 1, 25% DAI graad 2 en 38% DAI graad 3. Deze gradering werd met 7
andere methodes vergeleken, 1 had een minder goede en 6 hadden een betere relatie met de
uitkomst.

Eindverslag

Dit onderzoek werd uitgevoerd door het level 1 traumacentrum van het ETZ en de afdeling neurologie van het UMC Groningen (UMCG).
Diffuus axonale schade is een vorm van hersenletsel die ontstaat na ernstige ongevallen. Hierbij is er sprake van beschadiging van de zenuwuitlopers (axonen). De ernst van de axonale schade wordt aangegeven in een gradering. De betekenis voor de prognose van de patiënt is nog onduidelijk.
Literatuur onderzoek
Patiënten met axonale schade na een ernstig ongeval, hebben een ongunstigere uitkomst dan patiënten zonder deze schade. Maar in het algemeen heeft 52% van de patiënten met axonale schade een gunstige uitkomst. Bij elke toename in gradering o.b.v. de MRI-scan wordt de kans op een ongunstige uitkomst drie keer zo groot.
Retrospectief onderzoek
In het ETZ en UMCG werd tussen 2008 en 2014 bij 177 patiënten met diffuus axonale schade vastgesteld. Bij 134 patiënten hebben we op lange termijn een functionele uitkomst achterhaald, 51% van deze patiënten heeft een gunstige uitkomst op de lange termijn. Het herstel van bewustzijn ≤7 dagen, de pupilreactie en Marshall score (een CT beoordeling) hebben een relatie met de uitkomst. 86 patiënten hebben de vragen over de kwaliteit van leven beantwoord, 62% van deze patiënten heeft een goede kwaliteit van leven. Graad 3 diffuus axonale schade bleek een voorspeller voor een slechte kwaliteit van leven.
De meest gebruikte MRI scoringsmethode werd vergelen met 7 alternatieve methodes, waarvan er 6 een betere relatie met de uitkomst op lange termijn bleken te hebben.

Samenvatting van de aanvraag

Traumatic brain injury (TBI) is characterized by a complex and heterogeneous pathology. Brain damage may occur from different primary lesions, such as hematomas, cortical contusions, and diffuse axonal injury (DAI). DAI is often not visible in acute CT scans of the brain but in MRI, DAI is often present in severe TBI patients. The axonal injury results from the stretching and deformation of the brain tissue caused by acceleration-deceleration forces. DAI visualised on MRI can be graded into 3 stages. Grade 1 is defined as a pattern of traumatic lesions in the lobar white matter, whereas in stage 2 and 3, lesions are depicted in the corpus callosum and brainstem, respectively. Whereas stage 3 is often associated with a poor outcome, the outcome in stage 1 and stage 2 has been reported with conflicting results. However, the staging is only related to the location of the lesions and not to the severity. A study taking number and volume of DAI lesions into account found an association of both measures with outcome. It is of interest whether number and volume of lesions together with location of DAI lesions and standard available clinical properties result in a better prognostic value of the MRI. For this study we will identify all brain trauma patients of the past 5 years from our trauma databases with a MRI within the first 3 months after trauma of the brain. We will collect basic clinical data, treatment data, and outcome data using the extended Glasgow Outcome Scale (GOSE). The aim of this study is to collect data of potential prognostic variables. We will score the MRI using the classical staging but also by scoring the number and volume of the DAI lesions and by scoring the location of the DAI lesions. The patients will be subdivided in a group with DAI lesions only, and a group with DAI and other lesions like for example cerebral contusion. This research will be the first step to a MRI and clinical data based prognostic model which subsequently will be investigated in a prospective study. This research will be conducted at the trauma centre Brabant in collaboration with the department of neurology of the university hospital Groningen.
