Models of fatal and non-fatal outcome measurement in the trauma population

Om de kwaliteit van zorg te kunnen meten en vergelijken, wordt gebruik gemaakt van internationale voorspellende modellen. De klassieke uitkomstmaat die hierbij gebruikt wordt, is ziekenhuissterfte. Op basis van patiënt- en ongevalkenmerken worden de overlevingskansen binnen een patiëntengroep voorspeld.

Alternatieve modellen en uitkomstmaten

Uit eerder onderzoek blijkt dat met behulp van deze bestaande modellen de overleving niet voor alle patiëntencategorieën goed te voorspellen is. In dit project worden bestaande modellen gevalideerd en alternatieven ontwikkeld. De laatste jaren is de sterfte binnen de traumapopulatie sterk afgenomen. Dit vraagt naast sterfte ook om andere uitkomstmaten, zoals kwaliteit van leven en invaliditeit. In dit project worden op basis van een grote dataverzameling voorspellende modellen ontwikkeld die gebaseerd zijn op die andere uitkomsten.

Verwachte resultaten

Door voorspelde en werkelijke uitkomst te vergelijken kan vervolgens de kwaliteit van de traumazorg voor grote groepen gemeten en vergeleken worden.

Eindverslag

De traumaregistratie is een belangrijk middel voor het evalueren van kwaliteit van traumazorg. Het verouderde model dat in Nederland gebruikt wordt en de steeds ouder wordende trauma populatie zorgen voor vertekende getallen bij deze evaluatie. Dit project ontwikkelde en valideerde modellen waar comorbiditeit en leeftijd geïncludeerd zijn voor meer accurate voorspellingen. Het gepresenteerde model kan in de toekomst gebruikt worden voor betere evaluatie van kwaliteit van traumazorg. Daarnaast is het gebruik van multiple imputatie techniek om ontbrekende waarden in te vullen geoorloofd, zodat alle verzamelde gegevens daadwerkelijk meegenomen kunnen worden in de analyses. Overleving blijft een belangrijke uitkomst, maar veel patiënten ervaren fysieke en psychologische problemen na trauma. Dit project heeft een model ontwikkeld om kwaliteit van zorg te meten en vergelijken op basis van gezondheidsstatus. Ook is er een score kaart ontwikkeld voor psychologische gevolgen na trauma. Het bepalen van de gezondheidsstatus van de patiënt voorafgaand aan het ongeval is essentieel voor nauwkeurige voorspelling van het functioneren na trauma. De modellen kunnen gebaseerd worden op simpele klinische karakteristieken en kunnen in de toekomst helpen om de zorg nog beter op patiënten toe te spitsen. Ook kunnen ze helpen bij de communicatie tussen arts en patiënt.

In de (landelijke) traumaregistratie worden alle patiënten vastgelegd die als gevolg van een ongeval worden opgenomen in een ziekenhuis. Verschillende kenmerken van patiënt, ongeval, letsel en opname worden geregistreerd. De klassieke uitkomstmaat in de traumachirurgie is ziekenhuissterfte. Op basis van patiënt- en ongevalskenmerken uit de traumaregistratie kunnen de overlevingskansen binnen een patiëntengroep voorspeld worden. Door deze met de werkelijke overlevingscijfers te vergelijken, kan de kwaliteit van zorg geëvalueerd worden.

Ook wordt in dit project onderzocht wat het effect is van ontbrekende waarden in de traumaregistratie en hoe hier het beste mee om kan worden gegaan.

Door voorspelde en werkelijke uitkomst, zowel fataal als niet-fataal, te vergelijken kan vervolgens de kwaliteit van de traumazorg voor grote groepen gemeten en vergeleken worden.

Samenvatting van de aanvraag

OBJECTIVE: The major aim of this project is to validate and develop models for predicting fatal and non-fatal outcome after trauma. Specific questions of this project are: - What is the effect of multiple imputation in the Dutch trauma registry? - Which mortality prediction model is valid, reliable, and accurate for complex trauma patients? - What is a valid, reliable and accurate non-fatal outcome prediction model in a trauma population? - How are different non-fatal outcome measures related to each other? - What is a valid, reliable and accurate model for integrated fatal and non-fatal risk profiles? - How to use an integrated fatal and non-fatal prediction model in quality assessment of trauma care? STUDY DESIGN: A. prediction models for fatal outcome The first part is a retrospective analysis of the Trauma Registry Brabant in which we will 1. develop and validate mortality prediction models for complex patients: - severely injured patients - elderly trauma patients For this part of the prediction models we will use data from available registrations. 2. investigate the patterns of missing values in our trauma registry and the effect of Multiple Imputation in a trauma mortality prediction model. B. The second part is the developing of an overall outcome model including non fatal outcome using prospective collected data in a 24 months follow-up study. Non fatal outcome data will be collected in a longitudinal cohort study in Brabant. This study is described in detail in the proposal “Prevalence, recovery patterns and risk factors of non-fatal outcome and costs after trauma; a prospective follow up study” STUDY POPULATION/DATASET: In the Trauma Registry Brabant all twelve hospitals in the county Brabant participate. Physicians assisted by data managers perform a registration of all admitted trauma patients, including both those admitted immediately and secondary referrals. Patients who die in the emergency department are also included in this registry. Each year about 12.000 admissions are included in the registry A. We will include four years of registration data (2010-2013, N=46.000) for the fatal outcome models. B. For the non-fatal outcome models we will use the data collected in a prospective study in which all admitted trauma patients in the region Brabant who survive. Inclusion period will be one year and expected number of patients is 8.000. OUTCOME MEASURES: A. in-hospital mortality B. non-fatal outcome measures. Non-fatal outcome measures are: health related quality of life; functional outcome; psychological outcome; societal outcome; direct and indirect costs DATA ANALYSIS: A. fatal outcome models: - To develop accurate mortality prediction models for severely injured patients and for the elderly trauma patients stepwise multiple logistic regression methods will be used. - Multiple imputation: we will investigate the patterns of the missing physiological data and the association with auxiliary variables in the Trauma Registry Brabant. The model developed in another trauma registry will be performed and compared to a complete-case analysis. Draws time series plots (convergence) and autocorrelation plots (independence) of the mean and variance of physiologic variables will be generated. - One of the alternatives for Injury Severity Score, the Trauma Mortality Prediction Model (TMPM) is validated in the Brabant Trauma Registry data. The performance of the TMPM will be expressed in discrimination and calibration. B. Non-fatal outcome models: Correlation between the different non-fatal outcome measures will be calculated with Spearman's rho test.Predictors for non-fatal outcome are assessed using stepwise multiple regression models. The performance of the models will be assessed in terms of calibration and discrimination. The internal validity of the final model will be tested. The models will be externally validated in an external cohort. The role and effect of MI will be investigated. COLLABORATION/CONNECTION: The study will be performed in the St Elisabeth hospital, in collaboration with the Department of Public Health of Erasmus MC. This study will be a collaboration of the 12 hospitals participating in the Network Emergency Care Brabant.
