IDENTIFICATION OF MULTIPLE POTENTIAL TRIGGERS IN SARCOIDOSIS A strategy towards prevention and new treatments

Sarcoïdose, ook wel de ziekte van Besnier-Boeck genoemd, is een ontstekingsziekte van met name de lymfklieren en longen. Dit kan onder andere benauwdheidsklachten veroorzaken. De oorzaak van sarcoïdose is nog niet bekend en er is geen behandeling die deze ziekte kan genezen. Er zijn aanwijzingen dat bepaalde bacteriën en metalen een rol spelen in het veroorzaken van sarcoïdose. In dit project zal bij alle nieuwe patiënten worden nagaan of hun afweersysteem reageert op deze mogelijke oorzaken. Dit onderzoek is nog niet eerder routinematig bij sarcoïdose patiënten verricht. Het doel is om nieuwe subgroepen binnen de groep patiënten te identificeren. Wanneer bijvoorbeeld een bacterie een rol speelt in het ontstaan van sarcoïdose, kan antibiotica getest gaan worden als nieuwe behandelmethode. Bij patiënten waar metalen een rol spelen kan verdere ziekteprogressie worden voorkomen door de expositie hieraan te vermijden.

Binnen dit project werd gekeken of er verschillende oorzaken aan te wijzen zijn voor de ziekte sarcoïdose, een inflammatoire aandoening waarbij meerdere organen betrokken kunnen zijn. Door middel van bloedtesten is gekeken of patiënten met sarcoïdose overgevoelig reageren tegen bepaalde metalen zoals beryllium of bacterien met een inflammatoire reactie tot gevolg. Deze informatie kan ons helpen om bijvoorbeeld betere preventie toe te passen of antibiotische therapie te testen als alternatief voor middelen zoals prednison.

Een belangrijke conclusie is dat we binnen de groep van patiënten met sarcoïdose inderdaad patiënten kunnen identificeren met een overgevoeligheid tegen metalen zoals beryllium. Binnen de groep van sarcoïdose patiënten blijkt dat slechts een klein percentage te zijn, namelijk minder dan 2%. Echter, voor de betreffende patiënten is het relevant omdat gekeken kan worden of de expositie aan beryllium vermeden kan worden. De relevantie voor de andere metalen wordt op dit moment nog onderzocht.

Een andere voorlopige conclusie is dat obv onze resultaten bacteriën zoals mycobacteriën en propionibacteriën geen belangrijke rol lijken te spelen in het ontstaan van sarcoïdose in Nederlandse patiënten. Echter, het kan zo zijn dat de gebruikte detectiemethode middels een bloedtest in het onderzoek niet gevoelig genoeg is. In een vervolgonderzoek willen we gaan proberen om de mogelijk betrokken bacteriën aan te tonen in de granulomen van patiënten met de ziekte sarcoïdose. Indien dit lukt is het mogelijk om in deze specifieke groep van sarcoïdose patiënten het effect van antibiotische therapie te testen in de behandeling als alternatief voor de immuunsuppressieve middelen zoals prednison welke nu gebruikt worden.

Binnen dit project wordt gekeken of verschillende oorzaken aan te wijzen zijn voor de ziekte Sarcoïdose (vroeger de ziekte van Besnier-Boeck genoemd). Dmv bloedtesten wordt gekeken of patienten overgevoelig zijn voor bepaalde metalen of bacterien. Met deze informatie kunnen we betere preventie toepassen of antibiotica gaan gebruiken bij de behandeling van deze ziekte.

Samenvatting van de aanvraag

OBJECTIVE: Sarcoidosis is a systemic disorder of unknown etiology. Many potential organic/inorganic substances or microorganisms have been suggested to cause sarcoidosis, such as beryllium, aluminum, zirconium, mycobacteria and propionibacteria. Routine testing for possible triggers in sarcoidosis is not daily clinical practice. However, published data suggest that possible triggers could be identified in 74% of sarcoidosis patients. Confirmation of these data in Dutch sarcoidosis patients can lead the way for randomized controlled trials in distinct subgroups of patients assessing the efficacy of antimycobacterial or antipropionibacterial treatment. If sensitization for metals in sarcoidosis patients can be determined, avoiding exposure is an important extra therapeutic option in treating their disease. HYPOTHESIS: We hypothesize that inorganic substances such as aluminum, beryllium and zirconium as well as microorganisms such as mycobacteria and propionibacteria are antigenic triggers in Dutch sarcoidosis patients. STUDY DESIGN: Prospective cohort study STUDY POPULATION/DATASET: Patients with biopsy proven sarcoidosis. Patients presenting on the outpatient clinic with problems other than interstitial lung disease will serve as a control group. INTERVENTION: All patients will be asked to donate blood for this study. A single vein puncture will be performed. OUTCOME MEASURES: 1) The prevalence of sensitization against mKatG, RP-35, beryllium, aluminum and zirconium will be determined in sarcoidosis patients and non-sarcoidosis patients 2) Clinical Outcome Status in Sarcoidosis patients after 4 years of follow up will be defined 3) A comparison will be made between the classical lymphocyte proliferation test (LPT) and interferon gamma release assay (IGRA) to detect beryllium, aluminum and zirconium SAMPLE SIZE: A total of 200 sarcoidosis patients will be included. This is the amount of new sarcoidosis patients seen annually on our outpatient clinic. After one year, the inclusion will be stopped. In order to maximize the power to detect differences in sensitization, three controls will be recruited for each case (n=600). DATA ANALYSIS: The prevalence of sensitization against zirconium, aluminum, beryllium, mKatG and RP35 will be investigated. When comparing both lymphocyte proliferation test and interferon gamma release assay a student’s T-test will be used. A p value < 0.05 is considered significant. TIME SCHEDULE: 4 years
