Cognitive deficits in brain tumor patients after neurosurgery: incidence, severity and prediction of outcome

Geheugen-, concentratie- en planningsproblemen komen bij een aanzienlijk (maar onbekend) aantal patiënten met een primaire hersentumor voor. Deze zogenaamde cognitieve problemen kunnen een negatieve invloed hebben op de kwaliteit van leven van patiënten.
Een belangrijk doel van dit project is het onderzoeken van het vóórkomen en de ernst van cognitieve problemen bij hersentumorpatiënten voor en na een hersenoperatie. Daarbij wordt gekeken wat de invloed is op het dagelijks functioneren. Ook wordt onderzocht welke factoren de cognitieve problemen, of juist het herstel, na een hersenoperatie voorspellen. Om deze vragen te beantwoorden, worden bij patiënten op onze afdeling direct voor, en 3 en 12 maanden na de operatie, neuropsychologische tests en vragenlijsten afgenomen.
Met deze kennis kunnen we patiënten beter informeren over de gevolgen van een operatie op cognitief functioneren na de operatie en mogelijk ook aanwijzingen vinden om in de toekomst de kans op cognitieve problemen te verkleinen.

Eindverslag

Mensen met een primaire hersentumor (meningeoom of glioom) hebben last van geheugen-, concentratie- en planningsproblemen. Deze zogenaamde cognitieve problemen kunnen een negatieve invloed hebben op de kwaliteit van leven van patiënten. In dit project werd het cognitief functioneren van 261 patiënten met een meningeoom en 285 patiënten met een glioom onderzocht met gecomputeriseerde tests, die voor operatie en 3 en 12 maanden erna afgenomen werden binnen de standaard klinische neuro-oncologische zorg. De resultaten laten zien dat cognitieve stoornissen bij veel patiënten, van direct voor operatie tot 3 en 12 maanden daarna, voorkomen. Veranderingen in cognitieve prestaties van voor tot na operatie zijn het grootst en meest frequent voor aandacht, cognitieve flexibiliteit, en psychomotore snelheid. Daarbij werd gevonden dat ¬lagere preoperatieve cognitieve prestaties bij patiënten met een meningeoom of hooggradig glioom, en -in mindere mate- sociodemografische factoren (hogere leeftijd, vrouwelijk geslacht, en een lager opleidingsniveau bij patiënten met een meningeoom, en vrouwelijk geslacht bij patiënten met een hooggradig glioom) het risico op cognitieve stoornissen na de operatie verhogen. Deze kennis draagt bij aan de informatie¬voorziening aan patiënten, naasten, en clinici in een vroeg stadium van de behandeling. Daarnaast benadrukken de bevindingen het belang van het herkennen, onderzoeken, en actief omgaan met cognitieve stoornissen in de klinische neuro-oncologische behandeling van patiënten met primaire hersentumor.

Een belangrijk doel van dit project is het onderzoeken van het vóórkomen en de ernst van cognitieve problemen bij hersentumorpatiënten voor en na een hersenoperatie. Daarbij wordt gekeken wat de invloed is op het dagelijks functioneren. Ook wordt onderzocht welke factoren de cognitieve problemen, of juist het herstel, na een hersenoperatie voorspellen. Het liefst doen we dat met voorspellers die al vroeg bekend zijn. Om deze vragen te beantwoorden, worden bij patiënten op onze afdeling direct voor, en 3, 12 en 24 maanden na, de operatie neuropsychologische tests (de korte, gecomputeriseerde testbatterij CNS Vital Signs) en vragenlijsten afgenomen. Met de vragenlijsten doen we ook meer kennis op over vermoeidheid, werk status en beperkingen, sociale integratie/participate en de relatie van deze uitkomstmaten met cognitief functioneren (over tijd).
Met de kennis die we in dit project opdoen kunnen we patiënten beter informeren over de gevolgen op cognitief functioneren na de operatie en mogelijk ook aanwijzingen vinden om in de toekomst de kans op cognitieve en gerelateerde problemen te verkleinen.

Samenvatting van de aanvraag

BACKGROUND Deficits in cognitive functions are common in patients with primary brain tumors. These cognitive deficits can be very subtle, and easily go undetected on routine clinical examination. However, they are often very disruptive for a person’s quality of life, preventing return to a normal social and professional life. Preservation of cognitive functioning is an important outcome measure in treatment of glioma surgery, and essential for quality of life. At present, unfortunately, it is largely unknown how surgery affects cognition. A better understanding of the variables that predict the impact of surgery on cognition is of significant importance not only to patients and their families, but also to neurosurgeons. It provides neurosurgeons with evidence-based information about possible individual risk of surgery which will steer clinical decision making and enables to inform patients better about the consequences of surgery on long-term cognitive functioning. OBJECTIVES (1) To describe the incidence and severity of cognitive impairments in patients with gliomas and meningiomas before, and 3 and 12 months after surgical treatment. (2) To develop models based on presurgical sociodemographic, clinical, imaging, and (neuro)psychological variables that predict cognitive functioning one year after surgery. (3) To increase knowledge on fatigue, work status, work limitations, and community integration, and their relationship with cognition (over time) in order to improve care for patients with primary brain tumors. STUDY DESIGN Cognitive test data have been prospectively collected with a short computerized test battery (CNS VS) and several questionnaires on patient-reported psychological functioning. These data were collected immediately before, and 3 months after neurosurgery in patients with gliomas and meningiomas. For the proposed project, a 12-months assessment will be added to collect data on this clinically very relevant time point. OUTCOME MEASURES Computerized and paper-and-pencil cognitive tests, measuring aspects of memory, attention, speed of information processing and executive functioning, and questionnaires on self-reported cognitive functioning, anxiety, and depression have been administered prior to surgery, and 3 months after. In addition to these measures, questionnaires on fatigue, work status, work ability, work limitations, and participation will be administered at the 12-months assessment. STUDY POPULATION/DATA-SET Adult patients with supratentorial gliomas and meningiomas undergoing resective surgery in the St. Elisabeth Hospital, Tilburg. SAMPLE SIZE Pre-surgical (neuro)psychological test and questionnaire data were obtained from 200 patients with gliomas, and 170 with meningiomas. Data from the 3-months assessment are available from 150 patients with gliomas, and 120 patients with meningiomas. It is projected to collect further data from 100 patients with gliomas and 100 with meningiomas for a 12-months follow-up assessment. DATA ANALYSIS Analyses will be carried out on (neuro)psychological test and self-report questionnaire data, collected prior to, and 3 and 12 months after surgery. Statistical methods include: independent one-sample t-tests, analysis of (co)variance or linear mixed models, reliable change index, and multiple linear and logistic regression analyses. Machine learning techniques will be applied for developing the prediction models, with the aims of 1) cross-validation and replication of the regression analyses; 2) improving the classification performance of the prognostic models; 3) store these models in rule-bases that can be exploited for clinical decision support. The Alan Turing Institute Almere (ATIA) will contribute its tools and expertise to this end. Sociodemographic, clinical, imaging, and (baseline) (neuro)psychological variables will be included in the analyses as possible predictors or confounders, depending on the type of analysis. COLLABORATION/CONNECTION Tilburg University, Department of Cognitive Neuropsychology. The Alan Turing Institute Almere (ATIA) will contribute its tools and expertise of machine learning techniques. TIME SCHEDULE Start project: May, 2015; 12-months data collection: May 2015 – November 2016. Analyses and manuscript writing: May 2015 – November 2019.
