Clinical implementation of advanced MRI techniques for localization and monitoring of sensorimotor and cognitive functions in patients with brain tumors

Veel patiënten met een hersentumor worden in het dagelijkse leven beperkt door problemen met geheugen, planning of concentratie. Er is weinig bekend over de oorzaak van deze zogenaamde cognitieve stoornissen. Zowel vanuit het perspectief van de patiënt als van de neurochirurg is het belangrijk om meer kennis op dit gebied te verkrijgen.

Functionele MRI (fMRI)

Moderne beeldvormende technieken maken het in principe mogelijk om cognitieve functies te meten voorafgaand aan een operatie. Hierbij wordt typisch een netwerk van verschillende hersengebieden zichtbaar gemaakt. Een belangrijk doel van dit project is om te onderzoeken of deze informatie relevant is voor patiënt en neurochirurg, en of het kan leiden tot verbetering in patiënteninformatie en chirurgische strategie. Daarnaast willen we fMRI geschikt maken voor dagelijks gebruik in het ziekenhuis.

Werkwijze

Hersentumorpatiënten in het St Elisabeth Ziekenhuis zullen gevraagd worden deel te nemen aan het onderzoek. Hierbij worden fMRI en neuropsychologische onderzoeken rondom operatie verricht.

Eindverslag

Hersentumor patiënten hebben vaak last van concentratie problemen en vermoeidheid, zowel voor als na een operatie. De neurobiologische oorzaak van deze klachten is tot op heden onbekend. In dit project is daarom de hersenactivatie van patiënten gemeten met functionele MRI ('fMRI'), om zo een beter inzicht te verkrijgen. We weten uit eerdere studies dat cognitieve prestaties sterk gerelateerd zijn aan twee netwerken van hersengebieden: het ’Central Executive Netwerk’ (CEN), dat de taak zelf uitvoert, en het ‘Default Mode Netwerk’ (DMN), die concentratie en taakuitvoering juist kan verstoren. Onze studies tonen aan dat hersentumor patiënten een te hoge DMN activatie hebben tijdens taakuitvoering. Ook blijkt zelf-gerapporteerde vermoeidheid gecorreleerd te zijn aan te hoge activatie van het DMN. Verminderde prestaties op een cognitieve taak lijken ook gecorreleerd aan verminderde reorganisatie van het CEN ten opzichte van rust, mogelijk ook een gevolg van verstoring door het te actieve DMN. De verhoogde activatie van het DMN is drie maanden na operatie nog onveranderd.

Samenvatting van de aanvraag

RATIONALE Patients with brain tumours frequently suffer from cognitive impairments that have a negative influence on their daily functioning and quality of life. To date, unfortunately, we know very little about the causes of these impairments or the influence of (surgical) treatment. There is an undisputed clinical need to acquire this knowledge to improve patient counselling and optimize surgical treatment. Available clinical tools for this purpose are insufficient. Better options are conceptually available in the form of non-invasive functional neuroimaging techniques. These techniques can improve surgical planning and potentially monitor cognitive functions in brain tumour patients, thereby offering patients insight in the nature of their impairments. Whereas neuroscientific researchers have embraced functional neuroimaging techniques as important tools for brain mapping, they are rarely used by neurosurgeons because of a lack of standardized protocols and the need for expert knowledge. OBJECTIVES (1) Development of robust functional imaging protocols adapted for routine surgical planning in brain tumour patients. We aim to eliminate the need for expert knowledge (via standardized protocols and a documented workflow) and make these techniques applicable for neurosurgeons and neuroradiologists. (2) Elucidation of neural correlates of cognitive processing and development of new methods to localize and monitor cognitive functions in brain tumour patients, to supplement current methods for motor and language functions. STUDY DESIGN / POPULATION / OUTCOME MEASURES Our project is structured into a first phase with retrospective studies (A,B), and a second phase with prospective studies (C,D). Each of four studies (A-D) is described separately. (A) OPTIMIZE EXISTING TECHNIQUES The project will start with retrospective analyses of functional neuroimaging data from 150 brain tumour patients that have already been acquired in our hospital for clinical purposes, but have not been scientifically analysed. The standard imaging protocol includes two functional MRI scans (motor and language), one Fiber Tractography (FT) scan, one resting state fMRI scan (RS-fMRI), and anatomical scans for surgical planning. The data will be used to improve functional imaging methods in brain tumour patients. Automatized procedures will be developed for data processing and identification of critical motor and language areas and associated fiber tracts (B) INTRODUCE NEW TECHNIQUES RS-fMRI is a relatively new MRI technique to assess functional brain networks without subjects having to perform a specific task. Although generally considered a promising technique, there is no agreement in the neuroimaging community about the best approach to acquire and analyse RS-fMRI data. There is a clear need for validation and evaluation of its clinical effectiveness. The first aim of this retrospective study is to develop a robust and reliable RS-fMRI analysis by comparing different methods available. Feasibility of identifying motor and language networks, and of distinguishing functional from dysfunctional brain tissue is investigated. The best methods will be selected for use in the prospective studies (C & D). Second important aim is to explore RS-fMRI measures that correlate with the level of cognitive functioning in brain tumour patients, as determined with neuropsychological tests. We specifically focus on two brain networks: the default mode network (DMN) and the executive-control network. (C) ESTABLISH FINAL PROTOCOL The clinical scan protocol will be expanded with one or two fMRI tasks that target cognitive functioning. Tasks will first be evaluated against neuropsychological tests in cognitively impaired tumour patients, and the most sensitive tasks will be modified for fMRI, and validated in patients. All the fMRI paradigms will be optimized for sensitivity and efficiency and are integrated with the other scans into a comprehensive final clinical functional imaging protocol. The current clinical workflow from MR scanner to operating room, and risks to quality of the service, will be mapped in detail and will be improved, documented and validated. A reference data set is obtained from a group of healthy volunteers, with the new protocol and workflow, to establish baseline data for comparison to brain tumour patients (D). (D) IMPLEMENT PROTOCOL & CLINICAL RESEARCH In a prospective 3 year study, 100 brain tumour patients will be examined prior to and three months after surgery with the newly established clinical imaging protocol. We will study correlations between functional network activity and cognitive performance as measured with neuropsychological tests. Specifically, we aim to find imaging variables or characteristics of brain-network behaviour that predict cognitive behaviour in patients prior to and after surgery. A start will be made with a database aimed at mapping risk regions in the brain for neurosurgeons and scientists.
