Hartcentrum innoveert en excelleert

Het St. Antonius Ziekenhuis is al decennia lang een topreferent centrum voor behandeling van hartaandoeningen. Het Hartcentrum innoveert en excelleert onder andere in niet-coronaire interventies en gepersonaliseerde anti-trombotische therapie. Hierbij is nationale en internationale samenwerking essentieel. Met behulp van deze TZO-subsidie kan de gespecialiseerde zorg verder worden versterkt en structureel geborgd worden. Daarnaast zal onderzoek worden verricht naar het verbeteren van hartinterventies en het voorkomen van trombo-embolische- en bloedingscomplicaties.

Kosteneffectiviteit en maatschappelijke meerwaarde

De focus ligt hierbij niet alleen op de effectiviteit en veiligheid van behandelingen, maar ook op het onderzoeken van de kosteneffectiviteit en de maatschappelijke meerwaarde. Dit laatste wordt onderzocht door het bepalen van patiëntgerelateerde uitkomstmaten. Uiteindelijk zullen de resultaten worden geïmplementeerd in de dagelijkse praktijk en zal de opgedane kennis worden gedeeld met patiënten en zorgprofessionals.


Inleiding onderzoeken

Om voorop te blijven lopen, is het Hartcentrum gespecialiseerd in het doen van onderzoek naar nieuwe, verbeterde behandelingen. Hiervoor initieert het Hartcentrum jaarlijks nieuw, vaak baanbrekend onderzoek dat meestal direct toepasbaar is op de patiëntenzorg. Het Hartcentrum werkt ook samen met universitaire ziekenhuizen.

Het Hartcentrum onderzoekt hoe hart- en vaatziekten zo veilig mogelijk behandeld kunnen worden met zo min mogelijk belasting voor de patiënt. Een voorbeeld hiervan is de vervanging van “hartkleppen via de lies” (een kathetherbehandeling, waarbij de patiënt niet meer geopereerd en onder narcose hoeft). Door deze veiligere, minder belastende behandeling, is de herstelperiode korter en kunnen kwetsbare patiënten toch behandeld worden. We pogen deze behandelingen nog effectiever en veiliger te maken door complicaties tijdens en na de behandeling te voorkomen. De meest voorkomende en meest ernstige complicaties tijdens deze ingreep zijn bloedingen en stolsels (bloedpropjes die o.a. kunnen leiden tot een herseninfarct). Om deze complicaties te voorkomen is het nodig dat de patiënt de juiste bloedverdunners tijdens en na de ingreep krijgt. Tot nu toe is niet bekend welke bloedverdunners het beste zijn, en wij zijn van plan dit te onderzoeken.

In lijn met deze ambitie, beoogt het Hartcentrum vijf nieuwe onderzoeken uit te voeren, waarbij nieuwe, minimaal belastende, ingrepen voor patiënten nog meer op maat gesneden worden (personalised medicine), effectiever en veiliger gemaakt worden, door het verder terugdringen van bloedingen en stollingscomplicaties.

Onderzoek 1: POPULAR- LAAO

Prof. Lucas Boersma beoogt het minimaliseren van het risico op bloedstolsels in patiënten met hartboezemfibrilleren, door afsluiting van het linker hartoor –als bron van stolsels- te combineren met een geoptimaliseerde antistollingstherapie.

Onderzoek 2: PAUSE-TAVI

Dr. Jur ten Berg test een geoptimaliseerde antistollingstherapie uit bij patiënten met een kathetergestuurde vervanging van een hartklep, om bloedstolsels en bloedingen te voorkomen.

Onderzoek 3: GUIDE-T-AVR

Dr. Patrick Klein test uit hoe het maken en bestuderen van een CT-scan voorafgaand aan een hartklepvervanging bijdraagt tot een juiste selectie voor transcatheter- of chirurgische aortaklepvervanging waardoor de kans op bloeding, stolsels en herseninfarcten kan verminderen.

Onderzoek 4: CTEPH-OAC

Dr. Marco Post onderzoekt wat de beste antistollingstherapie is rond het dotteren van de longslagader bij patiënten met een verhoogde bloeddruk in de longslagader, veroorzaakt door bloedstolsels.

Onderzoek 5: POPULAR-GUIDE-PCI

Dr. Jur ten Berg (lijn 5) onderzoekt of een genetische test gebruikt kan worden voor het optimaliseren van antistollingstherapie voor patiënten die een stent (buisje) geplaatst krijgen om een dichtgeslibd bloedvat open te houden.
Dit onderzoek betreft een vervolg op project Cost-effectiveness of CYP2C19 genotype guided treatment with antiplatelet drugs in patients with ST-segment-elevation myocardial infarction undergoing immediate percutaneous coronary intervention with stent implantation: optimization of treatment

In alle onderzoekslijnen wordt kritisch gekeken naar hoe deze interventies bijdragen aan een verbetering in klinische uitkomstmaten, patiënt gerapporteerde uitkomstmaten en kostenbeheersing.

Samenvatting van de aanvraag

In the Netherlands, the St. Antonius hospital has had a prominent position in the treatment of patients with heart disease for more than 60 years. The heart center consists of the departments of cardiology and cardiothoracic surgery and offers (top referral) care for the full range of cardiac diseases. Patients are therefore referred to St. Antonius from all over the Netherlands. There is national recognition for this top referral care function, but even outside the country borders there is recognition for the role that the Heart Center plays in innovation and development of care for heart patients Within this TZO application, five research proposals will be enclosed for this purpose, with the common theme “optimization of interventions with special attention to prevention of thrombosis (including stroke) and bleeding” in patients who undergo a cardiological or cardiac surgery procedure. The aim of this is 1) to investigate the effectiveness and societal added value (cost-effectiveness) of promising new interventions, 2) to optimize existing interventions by comparing different treatment options, 3) to translate research results into clinical practice in the largest possible manner (supra) -regional) treatment population, in order to continue, strengthen and sustainably guarantee the top specialist care within the Heart Center. This requires a number of improvements in the underlying care, research, and R&D infrastructure. Briefly stated - partly due to the Topzorg 1 grant - all components are present, but these can be coordinated even better and used more structurally. In the area of care, accessing a common database for the Heart Center is crucial. In addition, outcome measures (PROMs) reported by the patient are entered into all care processes of the heart center and included in this common database. With this, half-yearly longitudinal and cross-sectional analyzes will be performed on clinical outcome measures, PROMs, and underlying healthcare costs. Based on identified areas for improvement, improvement teams will cyclically optimize care processes. If these improvements cannot be implemented directly in the workplace due to, for example, process optimizations, these points for improvement can give rise to new investigations as possible "hotspots for innovations". In the field of research, the five thematically related lines of research offer an excellent opportunity to strengthen the common research infrastructure. To this end, best practices and research tools in the field of data management, clinical trial coordination, and collaboration with academic centers are shared and selected to achieve more uniformity in design, execution and analysis of all lines of research within the Heart Center. In the field of education and training, the above objectives, best practices and results will be included in education programs, to train new talents in accordance with this vision, and to be shared with colleagues through inter-collegial consultation, among other things. Finally, the semi-continuous (cost) effectiveness analyzes of regular and experimental care within the heart center will be used to measure project progress and Heart center performance and, where necessary, to adjust policy accordingly. These outcome measures make it easier to select care with proven added value for the individual patient, both for research and for regular care purposes. These outcome measures also make a crucial contribution to perpetuating our top referral care. By supplying a "comprehensive business case" for a care product with proven added value to insurers, obtaining or maintaining adequate reimbursement is the most promising. To this end, a dialogue will be set up with insurers during the project in order to ensure that the lines of research optimally match the conditions used for reimbursement processes. In addition, better-informed choices can (and should) be made to restrict patients' access to care that is not considered effective for them. With this, the balance between optimum quality versus efficiency can be monitored. Patients and patient organizations will be closely involved in all parts and phases. A patient panel - consisting of patients and representatives of the affiliated patient associations - will provide input on the design and implementation of the research lines, among others in the field of information provision, PROMs questionnaires, informed consent letters, and shared decision making. In addition, patients in this panel will be involved in discussions about the integrated vision and policy implementation to implement it, in a joint effort to strengthen the top referral care within the Heart Center but to keep it affordable.
