Nieuwe technologieën en beslissingsondersteuning voor verbetering van patiëntwaarde bij 3 veelvoorkomende hartaandoeningen (Catharina Ziekenhuis)

Innovaties en beslissingsondersteuning

Preventie en optimale therapiekeuze leiden tot betere uitkomsten en lagere kosten bij hartpatiënten. Het specialistisch hartcentrum van het Catharina Ziekenhuis wil dit bereiken door technologische innovaties (zoals tele-monitoring en eHealth) samen met geavanceerde beslissingsondersteuning toe te passen bij patiënten met 3 veelvoorkomende hartziekten:

  1.  Verstopte kransslagaderen
    Ruggenmergstimulatie zal worden toegepast voor de behandeling van complexe vormen van kransslagaderlijden. Daarnaast zal beslissingsondersteuning in de ambulance bij patiënten met acute afsluitingen van de doorbloeding van het hart worden onderzocht.
  2. Vernauwde aortaklep
    Vernauwde aortakleppen komen vaak voor bij mensen op hoge leeftijd. Vaak hebben zij ook andere lichamelijke of cognitieve beperkingen. Dit project beoogt betere geriatrische screening en betere bepaling van de ernst van de vernauwing te ontwikkelen.
  3. Boezemfibrilleren
    Dit project richt zich op betere screening op risicofactoren (zoals slaapapneu en levensstijl) en de toepassing van artificial intelligence-analyses (AI) om preventie en optimale therapiekeuze te bevorderen.

Inleiding onderzoeken

Hartaandoeningen vormen de belangrijkste oorzaak van sterfte en veroorzaken ook voor heel veel mensen een grote ziektelast. Het onderzoek door het CHC richt zich al jaren op deze veelvoorkomende aandoeningen en de onderzoeksvoorstellen hebben dan ook betrekking op 3 veelvoorkomende hartaandoeningen, die nog steeds in hoge mate bijdragen aan vroegtijdig overlijden, verlies van kwaliteit van leven en hoge zorgkosten:
1) complexe kransslagader aandoeningen, 2) hartkleplijden en 3) hartritmestoornissen.

Onderzoek 1: complexe kransslagaderaandoeningen

In dit onderzoek worden complexe kransslagaderaandoeningen onderzocht, die een grote impact hebben op de kwaliteit van leven en investeringen in zorgmiddelen vergen. Het onderzoek vindt plaats bij twee belangrijke groepen patiënten, namelijk patiënten met therapieresistente pijn op de borst (RAP) en patiënten met een hoog risico op een hartinfarct (ACS). Voor beide situaties zijn er nieuwe innovatieve en veelbelovende opties voor vroege diagnose en invasieve behandeling beschikbaar, maar er is nog weinig objectief bewijs of het ook effectief is. De RAP-studie is een dubbelblinde, placebogecontroleerde, gerandomiseerde studie om de prestatie en werkzaamheid van ruggenmergstimulatie bij RAP te onderzoeken. In de ACS-studie wordt de waarde vastgesteld van een nieuwe pre-ziekenhuisrisicobeoordeling om triage en vroege behandeling te verbeteren en tegelijkertijd de zorgkosten te verlagen.


Onderzoek 2: vernauwing van de aortaklep

In dit onderzoek wordt aortaklepstenose onderzocht. Vernauwing van de aortaklep is bijna altijd progressief en leidt uiteindelijk tot klachten en sterk verhoogde kans op vroegtijdig overlijden waardoor een ingrijpende operatie onafwendbaar is. Onlangs is aangetoond dat een TAVI (transcatheter aortic valve implantation) ingreep een goed innovatief alternatief is voor openhartchirurgie. In dit project is het doel om de TAVI-behandeloptie beter in te bedden
in de zorg, een beter beeld te krijgen van de pre-interventie patiënt-specifieke data op basis van innovatieve beeldvorming en kunstmatige intelligentie (AI) om de behandeling te personaliseren en daarmee te optimaliseren voor de specifieke patiënt. Daarnaast gaan we samen met verwijscentra in de regio een nieuwe manier ontwikkelen om patiënten voor tijdens en na de behandeling beter te monitoren, zodat de hele “TAVI-zorgketen” kan worden verbeterd en patiënten dichter bij huis optimale zorg ontvangen.

Onderzoek 3: zorg voor patiënten met atriumfibrilleren

In dit onderzoek zal de zorg voor patiënten met atriumfibrilleren (AF) worden onderzocht. AF is de meest voorkomende ritmestoornis en is geassocieerd met hogere morbiditeit, zoals beroerte, dementie, en sterfte. Op dit moment bestaan er geen zorgprogramma’s voor screening op AF en de onderliggende risicofactoren. In dit project wordt een nieuwe vorm van zorg voor primaire en secundaire preventie van AF ontwikkeld door implementatie van innovatieve, niet-invasieve technologieën. Dit gebeurt door het gebruik van relatief goedkope en simpele wearables, zoals een horloge die gericht is op de ritmestoornis zelf, maar ook op de onderliggende risicofactoren, zoals slaap-apneu. Daarnaast wordt informatie verkregen over levensstijl, en klachten via patiënt self-reporting. Tenslotte wordt een nieuwe vorm van beslissingsondersteuning voor ritme-behandeling voor boezemfibrilleren opgezet, zodat een betere ‘first time right’ keuze gemaakt kan worden uit het brede palet behandel-opties. Analyse van bovenstaande data, maar ook van de bekende aanvullende diagnostiek kan op deze manier leiden tot optimale therapie-keuze, waardoor betere uitkomsten en vermindering van zorgkosten.

Samenvatting van de aanvraag

The main objective of the Catharina Heart Centre’s (CHC) TZO program is to augment the quality of care for cardiac patients with complex coronary disease, aortic valve stenosis, and complex arrythmias. These three represent our top specialist care and research endeavours. To achieve this, our existing multidisciplinary approach to its core elements of care, research and education will be enhanced, structurally anchored in care pathways covering the full cycle of care in CHC’s referral network and made sustainable whilst reinforcing the international position of the CHC. Altogether, we therefore aim to provide improved and patient-specific treatment through implementation of technological innovations, data analytics, and value-based healthcare (VBHC) principles. This will result in valuable decision supporting information for the treating physician and for the patient through which a more personalized treatment will be possible. To realise this, the existing multidisciplinary approach on the triad of care, research and education will be reinforced, embedded in regional care pathways, and made future proof by building a sustainable data infrastructure. By sharing our experiences and expertise, other care centres can also implement our way-of-working, leading to better quality of life for patients and manageable expenses for the care system, a balance which in our vision should always be at the basis of choices made in healthcare. Cardiac diseases are a major cause of death and lead to high disease burden for many. Our projects focus on the most common heart diseases. Each proposal will involve multidisciplinary collaboration between multiple healthcare centres using the value-based healthcare theory as leading principles, entail the training of future generations of cardiological specialists whilst providing the framework required to address further research questions on the clinical management of the pertinent conditions and contribute to the development of an advanced data-infrastructure and PDCA-cycle based on outcomes that matter most to patients both within CHC and in collaboration with referring cardiologists. In Research Proposal 1 we will investigate complex coronary conditions, which have a major impact on quality of life and require investment of care means. We will investigate two important groups of patients, namely patients with persisting angina (refractory angina pectoris, RAP) and patients that have a high risk for acute coronary syndrome (ACS). For both these situations there are new innovative and promising early diagnosis and invasive treatment options available, but there is yet little objective proof of their efficacy. The RAP study is a double-blinded, placebo-controlled, randomized study to investigate the performance and efficacy of Spinal Cord Stimulation to overcome RAP. In the ACS study we will ascertain the value of a new pre-hospital risk assessment to improve triage and early treatment while reducing cost of care. In Research Proposal 2 we investigate aortic valve stenosis, which is almost always progressive and represents a high risk for an early death. Therefore, drastic invasive open-heart surgery is unavoidable. Recently, however, it has been shown that transcatheter aortic valve implantation (TAVI) is a suitable innovative alternative to surgery. The goal of our project is to better embed the TAVI-procedure in the care pathways and to improve our understanding of pre-intervention patient-specific data obtained with innovative medical imaging and artificial intelligence (AI). This will result in an optimal patient-centred treatment. Additionally, we will work with referring centres to progress on patient monitoring via e.g. wearables that will improve the entire “TAVI chain of care” and enable optimal close-to-home treatment for patients with less necessary hospital visits. In Research Proposal 3 we study atrial fibrillation (AF), a common abnormal heart rhythm, which is associated with a higher chance for comorbidities like stroke, dementia, and mortality. There are currently no screening possibilities that take the underlying risk factors for AF into account. In this project we will develop a new form of primary and secondary preventative care, by implementing innovative, non-invasive wearable technology, which is broadly transferable to all caregivers involved in this patient group and the AI-based analysis of the generated data. The wearable technology (e.g. a watch) will measure heart rhythm directly but will also monitor underlying risk factors like sleep apnoea and general lifestyle and gathering of complaints via patient self-reporting. Physicians and specialised nurses will be trained to coach patient in the use of this new technology. Additionally, based on the AI-analysis of the data, supplemented by a new form of decision-support will be set up to ascertain a ‘first time right’ choice for treatment of AF, improving care results while lowering cost of care.
