Aanleiding

In 2030 is alvleesklierkanker, na longkanker, de belangrijkste doodsoorzaak door kanker. Een operatie gevolgd door chemotherapie is de standaardbehandeling voor patiënten met opereerbare alvleesklierkanker.

In Nederland leven deze patiënten gemiddeld maar 1 jaar na diagnose. Veel patiënten krijgen een operatie die achteraf zinloos lijkt en komen niet toe aan chemotherapie waarmee ze langer hadden kunnen leven.

Doel

De PREOPANC-2 studie onderzoekt of voorbehandeling met FOLFIRINOX chemotherapie gevolgd door een operatie beter is dan de standaard van een operatie gevolgd door chemotherapie bij patiënten met opereerbare alvleesklierkanker.

We verwachten dat meer patiënten hierdoor chemotherapie krijgen en daardoor langer leven met een betere kwaliteit van leven. Patiënten met nog niet zichtbare uitzaaiingen die snel uitgroeien blijft een zinloze operatie bespaard. Verder verwachten we dat voorbehandeling met FOLFIRINOX niet alleen kosteneffectief maar zelfs kostenbesparend is.

Samenvatting van de aanvraag

DOEL/VRAAGSTELLING De PREOPANC-2 trial onderzoekt of neoadjuvante chemotherapie met FOLFIRINOX gevolgd door herstadiëring en resectie superieur is aan de huidige praktijk van resectie gevolgd door adjuvante gemcitabine bij patiënten met (borderline) resectabel pancreascarcinoom. HYPOTHESE Neoadjuvante chemotherapie met FOLFIRINOX gevolgd door herstadiëring en resectie resulteert in meer quality-adjusted life years (QALYs) en is kosteneffectief. STUDIE OPZET De PREOPANC-2 trial is een fase III gerandomiseerde, gecontroleerde multicenter studie. STUDIEPOPULATIE/DATABRONNEN Patiënten met (borderline) resectabel pancreascarcinoom. INTERVENTIE Neoadjuvant FOLFIRINOX chemotherapie bestaande uit de combinatie 5-fluorouracil, irinotecan, oxaliplatin en leucovorin (8 kuren van in totaal 16 weken) gevolgd door herstadiëring en resectie. GEBRUIKELIJKE ZORG /VERGELIJKING Resectie gevolgd door adjuvant gemcitabine (6 kuren van in totaal 24 weken). UITKOMSTMATEN De primaire uitkomstmaat is overleving. Secundaire uitkomstmaten zijn toxiciteit, postoperatieve complicaties, resectie percentage, R0 resectie percentage, ziekte-vrije overleving, QALY’s, (in)directe kosten, kosteneffectiviteitsratio, budget impact. Aanvullende uitkomsten in de interventie arm zijn radiologische en pathologische respons. SAMPLE SIZE BEREKENING/DATA-ANALYSE In totaal zijn 260 patiënten nodig. Aannames hiervoor zijn een mediane overleving van 12 maanden in de standaard arm, een hazard ratio van 0.65, een alfa fout van 0.05 en een power van 80%. KOSTENEFFECTIVITEITSANALYSE/ BUDGET IMPACT ANALYSE Neoadjuvant FOLFIRINOX wordt economisch geëvalueerd vanuit maatschappelijk perspectief met kosteneffectiviteit en kostenutiliteit analyses. Kosteneffectiviteit ratio’s worden berekend als de extra kosten per extra QALY. Met een budget impact Markov model worden de financiële gevolgen van neoadjuvant FOLFIRINOX bij patiënten met (borderline) resectabel pancreascarcinoom vastgesteld vanuit het perspectief van de zorgaanbieder, zorgverzekeraar en overheid. De hypothese is dat neoadjuvant FOLFIRINOX kosteneffectief of kosten sparend is. TIJDPAD De patiënten inclusieperiode duurt 36 maanden. Met een follow-up duur van 12 maanden zal de PREOPANC-2 trial in totaal 4 jaar duren. RESEARCH QUESTION The PREOPANC-2 investigates whether neoadjuvant chemotherapy with FOLFIRINOX followed by restaging and resection is superior to the current practice of upfront resection followed by adjuvant gemcitabine in patients with (borderline) resectable pancreatic ductal adenocarcinoma (PDAC). HYPOTHESIS Neoadjuvant chemotherapy with FOLFIRINOX followed by restaging and resection results in more quality-adjusted life years (QALYs) and is cost-effective. STUDY DESIGN The PREOPANC-2 trial is a phase III randomized controlled multicenter trial. STUDY POPULATION Patients with (borderline) resectable PDAC. INTERVENTION Neoadjuvant FOLFIRINOX (8 cycles during a total of 16 weeks) followed by restaging and resection. USUAL CARE/COMPARISON Upfront resection followed by adjuvant gemcitabine (6 cycles during a total of 24 weeks). OUTCOME MEASURES The primary outcome is overall survival (OS). Secondary outcomes are toxicity, postoperative complications, resection percentage, R0 resection percentage, disease-free survival (DFS), QALYs, cost-effectiveness ratio, and budget impact. Outcomes evaluated only for the intervention arm are radiological and pathological response. SAMPLE SIZE In total 260 patients are required. Assumptions are a median OS of 12 months in the standard arm, a hazard ratio of 0.65, an alpha error of 0.05, and a power of 80%. COST-EFFECTIVENESS ANALYSIS / BUDGET IMPACT ANALYSIS Neoadjuvant FOLFIRINOX is economically evaluated from the societal perspective with cost-effectiveness and cost-utility analyses. Incremental cost-effectiveness ratios are calculated as the additional costs per additional QALY. A budget impact Markov model estimates the financial consequences of the neoadjuvant chemotherapy approach in patients with (borderline) resectable PDAC from the perspective of the health care provider, the health insurance company and the government. The hypothesis is that neoadjuvant chemotherapy is cost-effective or cost saving. TIME SCHEDULE Patient accrual will take 36 months. With a follow-up of 12 months after inclusion of the last patient, the PREOPANC-2 trial will take 4 years to complete.
