De multidisciplinaire behandeling van het complexe rectumcarcinoom (Catharina Ziekenhuis)

Behandeling op maat

Patiënten met vergevorderde en agressieve vormen van endeldarmkanker hebben behandeling op maat nodig. Dat geldt ook voor patiënten bij wie een standaardbehandeling niet haalbaar is, zoals kwetsbare ouderen of mensen die tegelijkertijd lijden aan andere ziekten. Het Catharina Ziekenhuis Eindhoven is gespecialiseerd in de complexe behandeling van endeldarmkanker.

Prognose verbeteren

Met de TZO-subsidie kan het Catharina Ziekenhuis deze zorg blijven aanbieden en verbeteren. Er worden 4 innovatieve onderzoeken gestart, waarbij het doel is om de prognose van de meest agressieve vorm van endeldarmkanker te verbeteren. Bij kwetsbare patiënten gaat het erom de behandeling minder ingrijpend te maken, zonder concessies te doen aan de kans op genezing. Met de subsidie richt het Catharina Ziekenhuis een landelijk netwerk op; gericht op onderwijs, onderzoek en het delen van kennis. Zo zetten zij zich niet alleen in voor optimale zorg en onderzoek in het Catharina Ziekenhuis, maar ook voor de rest van Nederland.


Inleiding onderzoeken

Wij voeren vernieuwende behandelingen in met behulp van 4 onderzoeksvoorstellen, bedoeld voor patiënten met “complexe endeldarmkanker”. Hiermee wordt voortgeborduurd op ons eerdere wetenschappelijk onderzoek. Tezamen gaan deze 4 onderzoeksvoorstellen nieuw licht werpen op de behandeling van complexe endeldarmkanker. Wij zijn ervan overtuigd dat deze onderzoeken gaan leiden tot een verbetering van de kwaliteit van zorg. Niet alleen voor de patiënt met complexe endeldarmkanker, maar ook voor de patiënt met “gewone” endeldarmkanker.

Wij doen dit wetenschappelijk onderzoek samen met andere gespecialiseerde ziekenhuizen in Nederland. Daarvoor hebben wij met die ziekenhuizen afspraken gemaakt over hoe de zorg voor complexe endeldarmkanker er in Nederland uit moet komen te zien. We gaan ervoor zorgen dat de behandeling overal gelijk is en we kijken bij elkaar mee. We verwachten dat deze ontwikkelingen al heel snel tot een enorme verbetering van de kwaliteit van zorg voor complexe endeldarmkanker gaan zorgen.

Onderzoek 1: Contact X-Ray brachytherapie

In dit onderzoek wordt Contact X-Ray brachytherapie (CXB) onderzocht. Dit is een techniek waarbij tijdens een poliklinisch onderzoek van de endeldarm kortdurend een hoge dosis bestraling op de endeldarmtumor kan worden gegeven. In dit project willen wij onderzoeken of de kans op het volledig verdwijnen van de tumor groter wordt als CXB wordt toegevoegd aan de normale voorbehandeling met bestraling en chemotabletten. Wij verwachten dat er veel meer patiënten zullen zijn die uiteindelijk geen operatie nodig hebben.

Onderzoek 2: behandeling oudere en kwetsbare patiënten

In dit onderzoek gaat het om oudere en kwetsbare patiënten. Voor hen is een operatie vaak te risicovol, maar niets doen leidt ook tot grote ellende. In dit onderzoek willen wij deze kwetsbare patiënten behandelen met dezelfde combinatie van bestraling en chemotabletten als in onderzoek 1, maar nu duurt de behandeling 1 week langer. Daarna krijgen de patiënten ook nog eens CXB. Dit lijkt een verzwaring van de behandeling in deze kwetsbare groep, maar het doel van deze behandeling is juist dat een operatie hierna niet meer nodig is. Wij verwachten zelfs dat de meeste patiënten zonder operatie genezen kunnen worden. Als dit bij de ouderen en kwetsbare patiënten lukt, is de kans groot dat ook jongere en fitte patiënten de voorkeur geven aan deze behandeling.

Onderzoek 3: inductie chemotherapie

In dit onderzoek gaat het om patiënten met de meest agressieve vormen van “lokaal voortgeschreden endeldarmkanker”. Het risico dat de ziekte terugkomt, op dezelfde plaats, of in de vorm van uitzaaiingen, is bij deze patiënten groot. Er zijn aanwijzingen dat de kansen beter worden als deze patiënten voor de behandeling met bestraling en chemotabletten eerst een aantal kuren chemotherapie via de bloedbaan krijgen. Dit heet inductie chemotherapie. Deze combinatiebehandeling is niet standaard. Met dit onderzoek willen wij uitzoeken of de resultaten van deze combinatiebehandeling inderdaad zo veelbelovend zijn als gedacht. Zo ja, dan moet deze behandeling standaard worden.

Onderzoek 4: lokaal recidief endeldarmkanker

In dit onderzoek gaat het om patiënten waarbij de endeldarmtumor is teruggekomen, in het gebied waar die oorspronkelijk ook zat. Dit heet “lokaal recidief endeldarmkanker”. Deze patiënten hebben slechte vooruitzichten en de kwaliteit van leven is vaak al ernstig aangetast. Alleen het volledig verwijderen van al het tumorweefsel kan voor genezing zorgen, maar veel patiënten komen niet aan een operatie. Bij degenen die wel geopereerd kunnen worden, lukt het maar bij 60% van de patiënten om de tumor helemaal te verwijderen. De afgelopen jaren heeft onderzoek uit het Catharina Ziekenhuis aangetoond dat ook hier de combinatiebehandeling van inductie chemotherapie en
daarna bestraling met chemotabletten zinvol kan zijn. Doordat de tumor dan nog kleiner wordt gemaakt, wordt de kans dat al het tumorweefsel kan worden verwijderd groter. Of dat ook leidt tot betere kansen voor de patiënt en of de bijwerkingen van de chemotherapie niet te hevig zijn wordt onderzocht in een grote studie. Daaraan doen alle gespecialiseerde ziekenhuizen van Nederland mee, maar ook een aantal gerenommeerde buitenlandse ziekenhuizen.

Samenvatting van de aanvraag

The Catharina Cancer Institute (CCI) is an experienced centre for treatment of complex rectal cancer. It plays a leading role nationally and internationally concerning the organisation of care, scientific research, knowledge transfer, training and innovation. The main objective of this TZO program is to augment the quality of care for patients with complex oncological conditions, specifically, those with complex rectal cancer, which is our top specialist care and research endeavour. To achieve this, the CCI’s existing multidisciplinary approach to its core elements of care, research and education will be enhanced, structurally anchored in multiple care pathways, and made sustainable whilst reinforcing the international position of the CCI. Altogether, we aim to provide improved and patient-specific treatment to patients with rectal cancer, thereby leading to improved quality of life and affordable care. Consequently, the four research proposals which constitute this application revolve around improving the top specialist care provided by the CCI to such patients. Each proposal will involve multidisciplinary collaboration between multiple healthcare centres and entail the training of future generations of rectal cancer specialists whilst providing the framework required to address further research questions on the clinical management of the pertinent conditions. In Research Proposal 1 we will investigate the utility of contact X-ray brachytherapy (CXB) in successfully treating patients who present with locally advanced rectal cancer. Specifically, we aim to investigate whether such a treatment can increase the probability of these patients being eligible for a “watch and wait” (W&W) organ-preservation programme, thereby circumventing potentially unnecessary and debilitating surgery-related complications and sequelae. In doing so, we seek to demonstrate that neoadjuvant chemoradiotherapy in combination with a CXB boost is superior to treatment with only neoadjuvant chemoradiotherapy in terms of organ preservation paving the way for future reform. Research Proposal 2 encompasses a particularly vulnerable group of rectal cancer patients: the elderly and frail. For these patients, surgical intervention is either impossible or extremely risky. Surgical intervention in in this context has a substantially higher risk of resulting in debilitating post-operative morbidity or mortality, leading to drastic reductions in patients’ quality of life. To improve the quality of care provided to these patients, we will therefore explore the possibility of combining CXB with radiotherapy by means of external radiation beams to investigate whether this innovative treatment has a durable, organ-saving and potentially curative effect. In concrete terms, we will assess whether addition of an endocavitary boost with CXB and intensified treatment with CRT or SCRT, increases the chance for cure, achieves durable control of local tumour growth, and preservation of both the rectum and anus. Furthermore, the scope of the project extends beyond elderly patients to those who present with the most advanced stages of locally advanced rectal cancer (LARC). These patients have an elevated risk of experiencing local tumour relapse and distant metastases associated with incomplete surgical resections. However, despite the dismal prognosis, there are positive indications that the addition of induction chemotherapy (ICT) to neo-adjuvant treatment can considerably increase the success ratio. Therefore, in Research Proposal 3, patients with the most extensive stages of locally advanced rectal cancer (LARC) will be treated with a combination of ICT and neo-adjuvant therapy. Given that this combination treatment is not yet considered 'standard of care', we seek to illustrate that this approach can make a significant contribution to improved response, surgical clear resection margins, oncological outcomes, survival chances, quality of life, morbidity and cost-effectiveness in order to construct a convincing case to alter the current definition of standard of care. Research Proposal 4 focuses specifically on investigating locally recurrent rectal cancer (LRRC) patients. These patients have an extremely poor prognosis and an abysmal quality of life. In such cases, only a complete resection of tumour tissue in its entirety can provide a realistic chance for cure. However, currently only 60% of patients are successful. The CCI has gathered first-of-its-kind evidence that addition of ICT to the current treatment regimen for LRRC patients increases the success rates of complete tumour resections. This project will thus test the hypothesis that the addition of ICT to the current treatment regimen, together with the centralisation of care processes, increases desirable oncological outcome for LRRC patients, thereby strengthening the case for its inclusion as standard of care.
