The implementation of a guideline on the management of the lumbosacral radicular syndrome among general practitioners, physiotherapists, radiologists, neurologists, neurologic surgeons and orthopedic surgeons: a shared care project in two hospitals

Klachten bij het lumbosacraal radiculair syndroom (o.a. uitstralende pijn naar één been) verdwijnen vaak binnen zes weken spontaan. De richtlijnen raden daarom zes weken te wachten alvorens nader onderzoek in het ziekenhuis te laten doen. Mede onder druk van de patiënten en de soms lange wachtlijsten in het ziekenhuis verwijzen huisartsen vaak eerder. Dit leidt tot onnodig gebruik van (dure) ziekenhuiszorg. TNO Kwaliteit van Leven onderzocht het effect van het maken van duidelijke afspraken tussen huisartsen, neurologen, radiologen en fysiotherapeuten in de regio’s Eindhoven en Helmond. Afgesproken werd patiënten niet vervroegd door te sturen. Waren de klachten na zes weken niet over, dan kon de patiënt via een versnelde verwijzing en dankzij aangepaste organisatie binnen het ziekenhuis binnen een week in het ziekenhuis terecht en werd binnen drie weken een diagnose gesteld. Na het invoeren van deze afspraken liep het percentage onnodig verwezen patiënten terug van 15% naar 8%.

Objective: a) To implement the national General Practitioners (GP) guideline on Lumbosacral Radicular Syndrome (LRS) in one region, including two hospitals (specialists) and all GPs (± 200) and physiotherapists (± 250). b) To determine the effects of the implementation. Design: In a pretest-posttest design the effects of implementing the guideline on unnecessary referrals, diagnostic procedures,admission time and costs are measured. GPs and physiotherapists are interviewed on obstacles in adhering to the LRS-guideline. Study populations: GPs, physiotherapists, radiologists, neurologists, neurological surgeons and orthopaedic surgeons. Patients visiting the GP with symptoms of a LRS. Intervention. The implementation of the national LRS-guideline. The care process in primary care and hospital is redesigned for LRS-patients by introducing a fast track procedure: if the GP adheres to the guideline (i.e. conservative management/no referral in the first six weeks) in turn the hospital guarantees a priority consultation with the neurologist and priority for MRI after six weeks, if still necessary (when symptoms remain or worsen). Implementation activities: Aimed at adoption: Brochures, newsletters, visits to local GP and physiotherapist groups. Aimed at implementation: focus interviews with GPs and physiotherapists on obstacles in adhering to the guideline. Afterward strategies will be developed that are tailored to the critical obstacles found (e.g. training, patient brochure, information campaign to the public). Outcome measures: The effectiveness will be measured in terms of 1) Reduction of percentage of patients who are referred within six weeks with no indication. 2) Reduction in average waiting time for first consultation with the neurologist .3) Reduction of duration of total diagnostic procedures. The effectiveness will be related to the difference in costs before and after the implementation of the guideline. Time schedule: November 2004 - April 2005: redesigning process in hospital, strategies aimed at positive adoption among GPs and physiotherapists. March-May 2005: focusinterviews with GPs and physiotherapists, developing strategies tailored to determinants found. July 2005: official start fast track procedure. January- May 2005 pretest; January-May 2006 first posttest; Januari-May 2007 second posttest. Doelstelling: a) Implementatie van de NHG-standaard Lumbosacraal Radiculair Syndroom (LRS) in een regio met twee ziekenhuizen onder alle huisartsen (± 200) en fysiotherapeuten (± 250). b) Onderzoek naar de effecten van de implementatie. Opzet: Via een pretest-posttest design worden de effecten van de implementatie van de standaard gemeten wat betreft onnodige verwijzingen, toegangstijd tot en diagnostisch proces op de polikliniek en de kosten. Huisartsen en fysiotherapeuten worden geïnterviewd over belemmerende en bevorderende factoren bij het werken met de standaard. Populatie: huisartsen, fysiotherapeuten, radiologen, neurologen, neurochirurgen, orthopedisch chirurgen. Patiënten met LRS-symptomen die de huisarts bezoeken. Interventie: Implementatie van de NHG-standaard over LRS. Invoering van een ´fast track´ procedure in het ziekenhuis: als de huisarts/fysiotherapeut de eerste zes weken een conservatief beleid voert conform de standaard, kan de patiënt daarna versneld terecht bij de neuroloog en voor een MRI, indien nog nodig (als de klachten niet zijn verminderd). Implementatie activiteiten: Ter bevordering van een positieve adoptie bij de huisartsen/fysiotherapeuten: brochure, nieuwsbrieven, bezoek aan de HAGRO en fysiotherapeuten. Ter bevordering van de implementatie: focusgroepinterviews met huisartsen/fysiotherapeuten over belemmerende en bevorderende factoren bij het werken met de standaard. Vervolgens worden strategieën ontworpen die hierop ingrijpen (bijv. aanvullend protocol, training in gespreksvaardigheden, patiëntenfolder, publiekscampagne). Uitkomstmaten: Reductie van het percentage onterecht verwezen patiënten binnen zes weken. Reductie van de gemiddelde toegangstijd tot de polikliniek en reductie van gemiddelde doorlooptijd op de polikliniek. De effectiviteit zal worden gerelateerd aan het verschil in kosten voor en na de implementatie van de richtlijn. Tijdsplanning: november 2004-april 2005: introductie ´fast track´ procedure in ziekenhuis; strategieën ter bevordering van de adoptie bij de huisartsen/fysiotherapeuten. Maart-mei 2005: focusgroepinterviews met huisartsen/fysiotherapeuten, ontwikkeling invoerstrategieën gericht op belemmerende en bevorderende factoren. Juli 2005: officiële start ´fast track´ procedure. Januari-mei 2005 pretest; januari-mei 2006 eerste posttest; januari-mei 2007 tweede posttest.
