Multicentre randomized clinical trial in resuscitation of preterm infants at birth with intact umbilical cord: The Aeration, Breathing and then Clamping (ABC) Study

Artsen van de Nederlandse centra voor veel te vroeg geborenen gaan onderzoek doen naar het beste moment van doorknippen van de navelstreng (afnavelen).
Veel te vroeg geborenen zijn bij geboorte het meest kwetsbaar en lopen risico op orgaanschade. De meesten hebben bij opvang bij geboorte hulp bij de ademhaling nodig om te overleven. Daarom worden ze snel afgenaveld zodat ze direct naar de reanimatietafel kunnen worden gebracht voor beademing. Het is echter gebleken dat het gunstig is voor de bloedsomloop van de baby wanneer gewacht wordt met afnavelen totdat de ademhaling goed op gang is gekomen. In het Leids Universitair Medisch Centrum is nu een reanimatietafel ontwikkeld die het mogelijk maakt om baby’s alle acute zorg te verlenen zonder ze eerst af te navelen.
De komende 2 jaar zal deze tafel gebruikt worden om te onderzoeken of eerst opvang en dan afnavelen bij veel te vroeg geborenen leidt tot betere klinische uitkomsten en minder zorgkosten dan eerst afnavelen en daarna opvang.

Samenvatting van de aanvraag

DOEL / VRAAGSTELLING Wat is het effect van afnavelen na cardiopulmonale stabilisatie (physiology based cord clamping (PBCC)) in plaats van afnavelen op basis van tijd (time-based cord clamping (TBCC)) op intacte overleving en zorgkosten bij premature pasgeborenen. HYPOTHESE PBCC bij premature pasgeborenen leidt tot een toename in intacte overleving (overleving tot ontslag zonder cerebrale schade en/of necrotizerende enterocolitis) en een afname in zorgkosten wanneer vergeleken met standaardzorg (TBCC). STUDIE OPZET Multicenter gerandomiseerd klinisch onderzoek STUDIEPOPULATIE / DATABRONNEN Pasgeborenen < 30 weken gestatieduur INTERVENTIE PBCC: De pasgeborene wordt gestabiliseerd aan de navelstreng en pas afgenaveld op het moment dat de pasgeborene cardiopulmonaal stabiel is. Hiervoor wordt een speciaal daarvoor ontworpen reanimatietafel gebruikt (Concord) waarbij het mogelijk is om de pasgeborene zeer dicht bij moeder aan de navelstreng te behandelen en te stabiliseren. GEBRUIKELIJKE ZORG / VERGELIJKING Premature pasgeborenen worden bij geboorte eerst afgenaveld alvorens ze naar de standaard reanimatietafel worden gebracht voor stabilisatie. Afnavelen is op tijd gebaseerd en wordt meestal 30-60 seconden uitgesteld, tenzij de conditie van de pasgeborene uitstel van behandeling niet toestaat. In dat geval wordt binnen enkele seconden afgenaveld. UITKOMSTMATEN Intacte overleving (overleving zonder cerebrale schade en/of necrotizerende enterocolitis), prematuriteit gerelateerde morbiditeit, (her- )opnamedagen, gebruik andere bronnen in gezondheidszorg, kosten ouders, kwaliteit van leven en neurologische uitkomst. SAMPLE SIZE BEREKENING / DATA-ANALYSE Met een inclusie van 690 premature pasgeborenen is het mogelijk om een absolute toename in intacte overleving van 10% (van 72% naar 82%) te detecteren met een power van 0.8 en een alpha van 0.05 met 10% non-compliance/cross-over. KOSTENEFFECTIVITEITSANALYSE / BUDGET IMPACT ANALYSE De economische evaluatie vanuit een maatschappelijk perspectief zal bestaan uit een trial-based kosten-baten analyse (kosten per additioneel kind met intacte overleving) en een model-based kosten-utiliteits analyse (levenslange kosten per QALY) volgens de Nederlandse richtlijnen voor economische evaluaties. Evaluatie van Implementatie scenario's vanuit relevante perspectieven (maatschappelijk, zorgverlener, verzekeraar), overeenkomstig de Nederlandse BIA richtlijnen. TIJDPAD Juli -Nov 2018: voorbereidingen Dec 2018-Dec 2020: includeren Dec 2020-Dec 2022: rapportage primaire uitkomst, follow up Jan 2023-Maart 2023: rapportage neurologische uitkomst en kosteneffectiviteit OBJECTIVE? To compare the effect of umbilical cord clamping after cardiopulmonary stabilization (Physiology Based Cord Clamping; PBCC) to time-based cord clamping (TBCC) on intact survival and health care costs in preterm infants. HYPOTHESIS PBCC in preterm infants increases intact survival (survival until discharge without cerebral injury and/or necrotizing enterocolitis) and decrease costs when compared to standard approach (TBCC). STUDY DESIGN Multicentre randomized controlled clinical trial? STUDY POPULATION Infants < 30 weeks of gestation? INTERVENTION PBCC: Stabilization of the infant with the umbilical cord intact and only clamp the cord when the infant is cardiopulmonary stable. For this a purpose-built resuscitation table (Concord) will be used, which makes it possible to stabilize the preterm infant in close proximity to the mother while the cord remains attached. USUAL CARE/COMPARISON Infants are clamped first before they are moved to the standard resuscitation table for further treatment and stabilization. Clamping is time-based and performed immediately or delayed at 30-60 seconds, depending on the clinical condition of the infant. OUTCOME MEASURES Intact survival (survival until discharge without cerebral injury and/or necrotizing enterocolitis), morbidities, (re)admission days, other healthcare resource use, parental costs, quality of life, neurodevelopment outcome.? SAMPLE SIZE/DATA-ANALYSIS.? With inclusion of 690 infants we will be able to detect an absolute increase in intact survival of 10% (from 72% to 82%) with beta 0.8 and alpha 0.05 and assuming a 10% non-compliance/cross-over. COST-EFFECTIVENESS ANALYSIS/ BUDGET IMPACT ANALYSIS? The economic evaluation from a societal perspective will consist of a trial-based cost-effectiveness analysis (costs per additional infant with intact survival) and a model-based cost-utility analysis (lifelong costs per QALY) according to the Dutch guidelines for economic evaluations. Implementation scenarios will be evaluated from the relevant perspectives (societal, healthcare provider, healthcare insurer), in accordance with the Dutch BIA guidelines.
