Induction of labour in hospital with a Foley catheter or oral misoprostol versus induction of labour at home with Foley catheter or oral misoprostol (PROBAAT-III): a multicentre randomized controlled trial with cost-effectiveness analysis.

Inleiden van de bevalling is de meest voorkomende interventie in de verloskunde. Het is belastend voor patiënten. Ook geeft het in ziekenhuizen logistieke uitdagingen. Het is daarom van belang om te zoeken naar de meest veilige, efficiënte, patiëntvriendelijke en kosteneffectieve methode. De meest gangbare methoden van inleiden zijn de Foley katheter en een tablet misoprostol. Beide zijn effectief en veilig.  

Doel van deze studie is te onderzoeken of het rijpen van de baarmoederhals thuis even effectief en veilig is als in het ziekenhuis. Ook wordt gekeken naar de kosteneffectiviteit en patiënt tevredenheid bij beide methoden. 2828 zwangere vrouwen worden gevraagd om deel te nemen in meerdere Nederlandse ziekenhuizen. Na deelname worden zwangeren verdeeld over 4 groepen op basis van loting. De vier groepen zijn: rijpen van de baarmoederhals thuis met Foley katheter, thuis met misoprostol tabletten, in ziekenhuis met Foley katheter of in het ziekenhuis met misoprostol. De totale duur van de studie bedraagt 36 maanden.

Samenvatting van de aanvraag

Samenvatting: DOEL Is het rijpen van de cervix, als onderdeel van inleiden van de baring, -thuis kosten-effectief in vergelijking met het ziekenhuis? -met orale misoprostol kosten-effectief in vergelijking met Foley-catheter? HYPOTHESES Rijpen van de cervix bij vrouwen met een onrijpe cervix: -is thuis even effectief en veilig als in ziekenhuis. -is thuis kosten-effectief en verbetert patiënttevredenheid. -is met misoprostol even effectief en veilig als bij gebruik van Foley. -is met misoprostol kosten-effectief en verbetert patiënttevredenheid. STUDIE OPZET Multicenter gerandomiseerde open-label studie met 2 bij 2 factorieel design. Analyse van veiligheid op basis van non-inferioriteit. Tevens zal kosteneffectiviteit en patiënttevredenheid onderzocht worden. STUDIEPOPULATIE Zwangeren in de à terme periode met indicatie tot inleiding van de baring en een onrijpe cervix. INTERVENTIES -Rijpen van de cervix thuis -Rijpen van de cervix met misoprostol GEBRUIKELIJKE ZORG/ VERGELIJKING -Rijpen van de cervix in het ziekenhuis -Rijpen van de cervix met Foley katheter UITKOMSTMATEN Primaire uitkomst voor veiligheid is ongunstige neonatale uitkomst gedefinieerd als perinatale sterfte, 5 min. Apgar score < 7 en/of navelstreng art. pH < 7.05 en/of opname op intensive care voor pasgeborenen. Primaire uitkomstmaat voor effectiviteit is het percentage vaginale bevallingen. SAMPLE SIZE BEREKENING/ DATA ANALYSE Om non-inferioriteit van zowel rijpen van de cervix thuis versus ziekenhuis, als rijpen van de cervix met misoprostol versus Foley katheter aan te tonen zijn 2828 deelnemers nodig. Met incidentie van de samengestelde slechte neonatale uitkomst van 4.8% met Foley katheter met 80% power, eenzijdige alpha van 0.05, en non-inferioriteitsmarge van 2% zijn 2828 deelnemers nodig om incidentie van 6.8% of hoger met misoprostol uit te sluiten. KOSTENEFFECTIVITEITSANALYSE/ BUDGET IMPACT ANALYSE Omdat rijpen van de cervix thuis opnamekosten bespaart zal de nadruk liggen gezondheidszorgperspectief. In de economische analyse worden deze kosten d.m.v. 'incremental cost-effectiveness planes' en 'cost-effectiveness acceptability curves' vergeleken met klinische uitkomstmaten. Een budget impactanalyse (BIA) zal financiële gevolgen van de invoering van het rijpen van de cervix in een thuissituatie in het Nederlandse zorgstelsel inzichtelijk maken. TIJDPAD Studie voorbereiding 5 maanden, 24 maanden voor inclusie 2828 deelnemers. Follow-up en analyse 7 maanden. Totale duur 36 maanden. Summary OBJECTIVES Is priming of the cervix, as part of induction of labour, -at home cost-effective as compared to the hospital? -with oral misoprostol cost-effective as compared to a Foley-catheter? HYPOTHESIS Induction of labour of women with an unripe cervix: -at home is equally effective and safe compared to ripening in the hospital. -at home cost-effective and resulting in higher patient satisfaction as compared to the hospital. -with misoprostol equally effective and safe compared to ripening with Foley catheter. -with misoprostol cost-effective and resulting in higher patient satisfaction than ripening with Foley catheter. STUDY DESIGN A nationwide multicentre open label 2 by 2 factorial randomized controlled trial with a cost-effectiveness analysis alongside it. STUDY POPULATIONS/ DATASET Pregnant women with an indication for induction of labour at term and an unripe cervix. INTERVENTIONS We will investigate the following experimental interventions: -Cervical ripening at home. -Cervical ripening with misoprostol. USUAL CARE / COMPARISON -Cervical ripening in hospital. -Cervical ripening with Foley catheter. OUTCOME MEASURES Primary outcome measure for safety is a composite of adverse neonatal outcome defined as perinatal mortality, 5-minute Apgar score <7 and/or umbilical artery pH < 7.05 and/or NICU admission. Primary outcome measure for effectiveness is the percentage of vaginal deliveries. SAMPLE SIZE CALCULATION / DATA ANALYSIS To assess non-inferiority of priming of the cervix at home versus versus hospital, and priming of the cervix with oral misosprostol we need 2828 participants. Based on an adverse neonatal outcome rate with Foley catheter of 4.8%, a predefined non-inferiority margin of 2%, we need 2828 women (1414 per group) to exclude an incidence of 6.8% per group. COST-EFFECTIVENESS ANALYSIS / BUDGET IMPACT ANALYSIS. Because cervical ripening in a home setting mainly reduces admission costs a health care perspective is used. In the economic analysis, these costs are compared with clinical outcome measures by means of 'incremental cost-effectiveness planes' and 'cost-effectiveness acceptability curves'. A BIA will be conducted to inform decision-makers about the financial consequences of the adoption and diffusion of cervical ripening in a home setting in the Dutch healthcare system. TIME SCHEDULE Study preparation 5 months. 24 months to recruit 2828 participants. Follow-up and analysis 7 months. Total duration of 36 months.
