Cryobiopsy for diagnosing interstitial lung diseases.
Projectomschrijving
De studie betreft patiënten met diffuse longaandoeningen (interstitiële longziekten (ILD)), waarbij het noodzakelijk is longweefsel te verkrijgen om een zekere diagnose te stellen. Tijdens deze studie zal een loting beslissen of een chirurg tijdens een operatie longweefsel wegneemt (huidige standaard zorg) of dat er door middel van een nieuwe endoscopische techniek vriesbiopten worden genomen. Middels een kijkonderzoek van de long (bronchoscopie) worden via de nieuwe vriestechniek (cryobiopt), een aantal kleine stukjes long verwijderd. Deze procedure kan in dagbehandeling worden uitgevoerd. Alleen wanneer hier geen diagnose uitkomt (20% van de patiënten), zullen er alsnog chirurgische hapjes van de long worden genomen. In deze studie wordt onderzocht welke van de 2 strategieën (operatie of vriesbiopt) het meeste voordeel biedt voor de patiënt (zoals het aantal opnamedagen, hoeveelheid pijn, complicaties) en tevens de laagste kosten met zich meebrengt.

DOEL/VRAAGSTELLING
Wij beogen een innovatieve diagnostische strategie te onderzoeken voor patiënten met longfibrose waarbij de noodzaak bestaat voor het verkrijgen van longweefsel. In gerandomiseerd onderzoek zullen bronchoscopisch long cryo (vries)biopten (nieuwe minimaal invasieve techniek) vergeleken worden chirurgische open long biopten (huidige standaard).
HYPOTHESE
Een diagnostische strategie met bronchoscopische cryobiopten (interventiegroep) heeft een vergelijkbare diagnostische waarde met lagere morbiditeit en kosten, in vergelijking met een chirurgisch longbiopt (huidige standaard).
STUDIE OPZET
Gerandomiseerde studie
STUDIEPOPULATIE/DATABRONNEN
Patienten met interstitiele longziekten/ longfibrose en een indicatie voor een longbiopt.
INTERVENTIE
Bronchoscopische long cryobiopten gevolgd door chirurgische longbiopten (alleen in geval van een non-diagnostische cryobiopsie)

Primaire uitkomstmaat: Opname-dagen ziekenhuis
Secundaire uitkomstmaten: Dagen met thorax-drain, pijn, complicaties, kosten, patienten preferentie, diagnostische waarde van de strategie.

KOSTENEFFECTIVITEITSANALYSE/BUDGET IMPACT ANALYSE
De economische evaluatie zal worden uitgevoerd als een kostenutiliteitsanalyse vanuit een maastschappelijk perspectief en een kostenminimisatieanalyse vanuit een ziekenhuis perspectief met een tijdshorizon van 2 maanden. Bij geleidelijke implementatie wordt een besparing van 3.4 miljoen euro voor een budget periode van vier jaar verwacht.

OBJECTIVE(S)/RESEARCH QUESTION(S)
We aim to investigate a diagnostic strategy starting with bronchoscopic lung cryobiopsy compared to immediate surgical lung biopsy (SLB) for patients with interstitial lung disease with a need for tissue sampling.
HYPOTHESIS
We hypothesize that a diagnostic strategy starting with lung cryobiopsy (intervention group) provides similar diagnostic yield and reduces morbidity and costs in comparison to SLB (current standard of care).

Randomized controlled trial
STUDY POPULATION
Patients with interstitial lung disease and an indication for lung biopsy
INTERVENTION
Bronchoscopic lung cryobiopsy followed by surgical lung biopsy (only in case of non-diagnostic cryobiopsy)
USUAL CARE/COMPARISON
Immediate surgical lung biopsy
OUTCOME MEASURES
Primary outcome measure: Hospitalization days.
Secondary outcomes: Days with chest tube, pain, complications, costs, patient preference, diagnostic yield of strategy.

COST-EFFECTIVENESS ANALYSIS/BUDGET IMAPCT ANALYSIS
The economic evaluation will be performed as a cost-utility analysis from a societal perspective and a cost-minimisation analysis from a hospital provider perspective with a time horizon of 2 months. Gradual implementation during a four year budget period may generate a cost saving of 3.4 million euro in total.
