DUTCH-AF Registry – A nationwide registration of patients with atrial fibrillation

In Nederland is atriumfibrilleren (AF) met ca 400.000 patiënten de meest voorkomende hartritmestoornis. AF gaat gepaard met een verhoogd risico op een herseninfarct. Om dit te voorkomen hebben patiënten met AF vaak antistollingsmedicijnen nodig. Dit zijn nieuwe (NOAC’s) of al langer bestaande middelen (VKA).

Dit project bestaat uit twee delen. Om voor de dagelijkse praktijk in kaart te brengen hoe deze nieuwe middelen werken, worden alle patiënten met AF in Nederland in een nationaal register, DUTCH-AF registry genaamd, samengebracht. De patiënten in dit DUTCH-AF registry zijn zowel afkomstig van de huisarts als van de specialist.
Daarnaast wordt vanuit het register in het onderzoek aandacht besteed aan therapietrouw met en juiste dosering van NOAC’s, omdat 40-60% van de patiënten met AF een verkeerde dosis krijgt voorgeschreven of de behandeling met NOAC’s binnen 1 tot 2 jaar stopt. Een belangrijke reden hiervoor is angst bij artsen en patiënten voor bloedingen. Een nieuwe bloedingsscore zal worden geëvalueerd zodat er een betere schatting kan worden gemaakt.

Samenvatting van de aanvraag

In the Netherlands the prevalence of atrial fibrillation (AF) is 400,000 and is still rising. AF is strongly associated with ageing and underlying disease such as hypertension, heart failure, type 2 diabetes and coronary atherosclerosis. AF may cause symptoms, and is associated with an increased risk for developing ischemic stroke (annual risk 5-7%). Therefore, patients with AF often need oral anticoagulant therapy (either NOACs or VKAs). In 2010, the World Health Organization (WHO) stated that focus should be on i) patient empowerment, ii) care provided by multi-disciplinary teams of healthcare professionals working collaboratively, and iii) better registration of patient-relevant outcomes.For this reason the Dutch Society of Cardiology (NVVC) started with the Connect AF initiative in 2015 improve the quality of care by optimization of the regional ‘transmural organization of AF-care’, including patient empowerment. These aims will be achieved through multi-disciplinary networks (including patients’ associations), and by initiation of a registry. Given that oral anticoagulation for stroke prevention in AF is the mainstay of treatment goals, safe oral anticoagulation management is our first goal (see our research part of DUTCH AF). Moreover, both will facilitate the transition of care from hospital to primary care bearing in mind “technically sophisticated high-tech care” if needed, and “integrated low-complex care in the community” when possible’, in shared-decision.
