Efficacy of AlbenDazol to inDuce mUcosal healing in Patients on anti-TNF monotherapy (ADD UP)

Bij de behandeling van de ziekte van Crohn is genezing van het darmslijmvlies belangrijk. M2 macrofagen nemen toe na behandeling met anti-TNF en zijn cruciaal voor de heling van het slijmvlies. De combinatietherapie van anti-TNF met een thiopurine geeft de sterkste toename in M2 macrofagen. De behandeling met thiopurines moet echter vaak gestopt worden omwille van bijwerkingen.
Uit een screening van 1600 geregistreerde medicijnen bleek dat albendazol ook de toename van M2 macrofagen versterkt. Een muismodel voor chronische darmontsteking toonde aan dat albendazol inderdaad de effectiviteit van anti-TNF significant verhoogt. Albendazol behoort tot de groep van anti-helmintica en heeft een goed veiligheidsprofiel.

Doel

Het doel van dit multicenter, gerandomiseerde onderzoek is de veiligheid en effectiviteit vast te stellen van het toevoegen van albendazol aan anti-TNF monotherapie gedurende een periode van 12 weken bij volwassen patiënten met de ziekte van Crohn met zweren in hun darm.

Samenvatting van de aanvraag

Crohn’s disease is a chronic inflammatory bowel disorder (IBD) of the intestinal tract with a considerable impact on the quality of life of affected patients. The introduction of antiTNF therapy has significantly improved the treatment options. Unfortunately, a large proportion of patients still do not respond (sufficiently) as complete mucosal healing in seen in only 30% of patients after 26 weeks of therapy. This can be increased from 30% to 44% if it is combined with the thiopurine azathioprine. However, thiopurines often have to be discontinued due intolerance or serious side effects. Our preclinical work shows that albendazole potentiates antiTNF both in vitro and in an IBD mouse model. Albendazol is antihelminthic agent with a well known safety profile. In the current project we want to evaluate the overall benefit of the addition of albendazole to antiTNF therapy in patients with anti-TNF monotherapy with incomplete mucosal healing. Patients will be randomized to either albendazole or placebo in addition to ongoing antiTNF treatment for 12 weeks, after which the endoscopic response is determined. We expect that albendazole in combination with antiTNF is safe and well tolerated and that combined treatment will increase the percentage of patients that achieve complete mucosal healing on antiTNF therapy hereby increasing clinical outcomes and reducing medical costs. Furthermore, results of this project can easily be implemented as albendazol is a registered drug.
