Diagnostic tool to safely CEASE anti-TNF therapy in Crohn’s Disease (CEASE-trial)

Rond 35.000 mensen in Nederland lijden aan de ziekte van Crohn, een chronische darmontsteking. De ziekte heeft een grote impact op het dagelijks leven van Crohn-patiënten.

Ongeveer een kwart van de patiënten wordt langdurig behandeld met anti-TNF medicijnen (ontstekingsremmers). Deze behandeling is vaak effectief; er kunnen echter (ernstige) bijwerkingen optreden en er zijn hoge kosten aan verbonden (€ 12.500/patiënt/jaar).
Het stoppen met anti-TNF medicijnen lijkt bij bepaalde patiënten een reële (veilige) optie. Recent onderzoek duidt op het bestaan van indicatoren die het risico op opvlamming van de ziekte na stopzetten van de behandeling voorspellen.

Doel

Het doel van de CEASE studie is het ontwikkelen van een voorspellend diagnostisch instrument op basis van weging van deze indicatoren. De voorspellende waarde van het instrument zal getoetst worden in een klinische studie.
Het gerichter kunnen inzetten van anti-TNF medicijnen zal gezondheidswinst opleveren voor de patiënt en leiden tot kostenbesparing.

Samenvatting van de aanvraag

PROBLEM DEFINITION/OBJECTIVE In the Netherlands, approximately 8,000 patients with Crohn’s Disease (CD) receive anti-TNF medication. Long-term anti-TNF treatment may result in side-effects and is associated with high costs. Tools for patient stratification are urgently needed to effectively use anti-TNF medication. This project will develop, validate and implement a predictive diagnostic tool to ensure a personalized anti-TNF treatment approach in CD. STUDY DESIGN A meta-analysis on risk factors for cessation of anti-TNF treatment is performed as basis to develop a prediction model using individual patient data (n=500, >100 events). A cluster step wedge randomized trial will be performed to validate performance of the diagnostic tool and confirm cost-effectiveness. STUDY POPULATION Patients with quiescent CD using anti-TNF therapy. DIAGNOSTIC INTERVENTION & COMPARATOR Cessation of anti-TNF therapy will be compared with standard care (continuation of anti-TNF). OUTCOME MEASURES Primary outcome measures: relapse of CD Secondary measures: cost-effectiveness, QALY, quality of life and productivity loss. SAMPLE SIZE 194 patients DATA-ANALYSIS Predictive performance will be evaluated with cross-validation between cohorts. Comparisons will be made on relapse rates in the control groups receiving continuing medication and groups that stop anti-TNF medication. Factors related to time to relapse will be studied through univariable and multivariable Cox proportional hazards regression. ECONOMIC EVALUATION Effects of using the diagnostic tool to stop anti-TNF treatment of CD patients will be compared. Therefore, cost-effective analysis, cost-utility analysis and budget impact analysis will be performed. IMPLEMENTATION The diagnostic tool will be implemented as a score in the electronic system to be used in daily clinical practice, this will make the model easily available for all Dutch gastroenterologists. In addition, a guide for shared decision making will be created.
