De ziekte van Crohn is een chronische ontstekingsziekte van de darm met een wisselende prognose. De standaard behandelstrategie is een stapsgewijze introductie van steeds krachtigere geneesmiddelen. Hierdoor wordt het starten van TNF-blokkers, die het ziektebeloop bij ernstige ziekte gunstig kunnen beïnvloeden, pas in een later stadium als behandeloptie ingezet.
Het onmiddellijk na de diagnose starten van de behandeling met een thiopurine in combinatie met een TNF-blokker zorgt daarentegen voor overbehandeling van groepen met een mild beloop. Er is niet goed onderzocht of en wanneer deze combinatiebehandeling weer kan worden gestopt.

Doel

Het doel van de CoCros-studie is de effectiviteit, veiligheid en kosten van een tijdelijke behandeling met de TNF-blokker adalimumab als eerstelijns therapie te vergelijken met de standaard stapsgewijze behandelstrategie.
Deelnemers aan deze gerandomiseerde studie worden 2 jaar strikt gevolgd met de web-based toepassing ‘mijnIBDcoach’ en met een ontlastingthuistest, zodat bij terugval de behandeling tijdig kan worden geoptimaliseerd.

Samenvatting van de aanvraag

Crohn's disease (CD) is a chronic progressive inflammatory bowel disease with a heterogeneous disease course and treatment response. No markers are available to predict a severe or favourable disease course. Increasing evidence shows that early initiation of biological drugs protects against irreversible damage and improves quality of life. Prospective disease modification trials, designed to also prevent unnecessary exposure of low risk patients to combined immunosupressives are however lacking. Furthermore cost-effectiveness analysis of treatment strategies is warranted. Objective: Since introduction of TNF-blockers immediately after diagnosis is potentially disease modifying compared to conventional step-up care our aim is to compare the long-term outcome of step-up care starting with corticosteroids to episodic treatment with adalimumab monotherapy as first-line therapy in CD on the primary outcome quarters of corticosteroid free clinical and biochemical remission after 96 weeks. Safety endpoints are: disease progression on MRI-enterography, side effects and tolerability. Additional cost-effectiveness, and patient reported outcomes are assessed. Study Design: Open label randomized multicenter trial of episodic adalimumab monotherapy compared to step-up care. Treatment discontinuation of adalimumab is based on endoscopic remission at week 24. Recurrence is tightly monitored with myIBDcoach and faecal calprotectin. Study population:146 adult newly diagnosed CD patients.
