The attributable burden and costs of infections caused by antibiotic-resistant Gram-negative bacteria in Dutch hospitals

Wereldwijd, inclusief Nederland, is er een evidente toename van antibiotica-resistente Gram-negatieve bacteriën. Tot nu toe ontbreekt een adequate en nauwkeurige berekening van de ziektelasten en -kosten van infecties veroorzaakt door deze antibiotica resistente bacteriën. In een prospectieve cohort studie zullen we patiënten met een kweek-bewezen infectie met antibiotica-resistente Gram-negatieve bacteriën vergelijken met patienten met een infectie met antibiotica-gevoelige bacteriën. We beogen om met dit onderzoek een nauwkeurige schatting te maken van de ziektelasten en -kosten van antibiotica resistentie in de Nederlandse ziekenhuizen. We zullen variabelen die zijn geassocieerd met antibiotica resistentie identificeren en bacterie-specifieke Disability-Adjusted Life Years [DALY] berekenen. Ten behoeve van deze doelstellingen passen we innovatieve onderzoeksmethoden toe die we vergelijken met de huidige methoden die worden gebruikt om te corrigeren voor verstorende variabelen.

Gram-negatieve bacteriën zoals Escherichia coli en Pseudomonas aeruginosa zijn belangrijke ziekteverwekkers, zowel binnen als buiten het ziekenhuis. Hierbij kan gedacht worden aan bijvoorbeeld urineweginfecties en buikinfecties. In de laatste jaren blijken deze Gram-negatieve bacteriën vaker resistent voor belangrijke antibiotica te zijn. Bij een infectie met deze bacteriën zijn er altijd nog reserve-antibiotica beschikbaar om de infectie te behandelen. Het kan alleen wel langer duren voordat de patiënt deze krijgt, omdat aan het begin van de infectie niet bekend is dat het om een resistente bacterie gaat. De vraag is of patiënten hier problemen door ondervinden.

Binnen dit project is een studie opgezet waarin patiënten met een infectie met resistente Gram-negatieve bacteriën vergeleken worden met patiënten met een infectie met normale varianten van deze bacteriën. In 8 ziekenhuizen is gedurende een jaar veel informatie verzameld over patiënten met Gram-negatieve infecties. Zo konden wij een nauwkeurige schatting maken van de ziektelasten en -kosten van antibioticaresistentie in de Nederlandse ziekenhuizen. Dat houdt in dat we bijvoorbeeld extra sterfte als gevolg van resistentie in kaart brengen, maar ook andere gevolgen zoals een toename aan complicaties bij infecties (denk aan opname op de intensive care of een operatie), zorggebruik en ziekteverzuim.

Dat houdt in dat we bijvoorbeeld extra sterfte als gevolg van resistentie in kaart brengen, maar ook andere gevolgen zoals een toename aan complicaties bij infecties (bijvoorbeeld IC-opnames of operatief ingrijpen), zorggebruik en ziekteverzuim.

Samenvatting van de aanvraag

It is extremely difficult to accurately quantify the attributable burden of infections caused by antibiotic-resistant bacteria. Yet, all initiatives for prevention and policy making are based on such estimates. Up till now, a holistic approach to accurately quantify burden and costs of infections caused by antibiotic-resistant Gram-negative bacteria and of their preventive measures is lacking. Current estimates are prone to overestimation and foreign estimates are not transferable to the Netherlands, as the Dutch situation differs in many aspects. In a prospective parallel matched cohort design of consecutive patients with culture-proven infections caused by antibiotic-susceptible and antibiotic-resistant Gram-negative bacteria [ASGNB and ARGNB] and matched uninfected controls we will determine the incremental burden and attributable costs of ARGNB compared to ASGNB. This design and adaptation and optimization of existing analytical methods will allow optimal adjustment for confounding and clustering effects. By means of a pathogen based Disability-Adjusted Life Years approach all sequelae will be related to their infectious cause. The combination of a rigorous study design and the comparison of innovative analytical methods to current state-of-the-art methods allow optimal adjustment for confounding and clustering, and will provide an accurate quantification of the burden of antibiotic resistance in Dutch hospitals.
