Prediction model for colonization with zoonotically acquired Clostridium difficile in patients on admission to a hospital.

De darmbacterie Clostridium difficile kan infecties veroorzaken, waarbij diarree meestal de belangrijkste klacht is. Er zijn echter ook personen, waarbij deze darmbacterie onderdeel is van de darmflora, zonder klachten te veroorzaken. Dit noemen we dragerschap.

In onze studie werd aangetoond dat 3.1% van de patiënten die opgenomen worden in het ziekenhuis drager was van C. difficile. Er werden veel verschillende C. difficile typen gevonden. Dragerschap van C. difficile en dragerschap van meticilline resistente Staphylococcus aureus werd nooit in dezelfde patiënt gevonden. Wel hadden de dragers een andere samenstelling van de darmflora. Deze waarneming betekent dat er eerst veranderingen in de darmflora ontstaan voordat C. difficile zich daar kan vestigen. Dragerschap werd vooral gevonden bij patiënten met een inflammatoire darmziekte (colitis ulcerosa, ziekte van Crohn), orgaantransplantatie en bij patiënten die een hond als huisdier hielden.

Uit onze studie bleek dat geen van de dragers symptomen van een darminfectie ontwikkelde. Ook werd er geen overdracht van de bacterie van dragers naar patiënten met een C. difficile infectie aangetoond. Dragerschap van C. difficile gaat dus wel samen met een veranderde darmflora maar heeft verder geen directe conseqenties voor de patiënt of zijn omgeving. Dit betekent voor de Nederlandse situatie dat onderzoek naar dragerschap van C. difficile bij opname in het ziekenhuis op dit moment niet nodig is om infecties met deze bacterie in het ziekenhuis te voorkomen.

Het doel van onze studie is om een model te ontwikkelen dat voorspelt of patiënten die opgenomen worden in het ziekenhuis, Clostridium difficile bij zich heeft. We zijn vooral in het PCR ribotype 078 geinteresseerd omdat dit ook vaak bij dieren wordt aangetroffen. De studie bestaat uit twee fases. Eerst is er een screening-fase waarin ontlasting of rectale uitstrijkjes worden afgenomen van patiënten die net zijn opgenomen in het ziekenhuis. In de tweede fase, de case-control fase, wordt informatie verzameld van C. difficile gekoloniseerde patiënten (cases) en niet-gekoloniseerde patiënten (controles) over mogelijke risicofactoren voor C. difficile kolonisatie. Op dit moment is de inclusie in 3 van de 4 deelnemende ziekenhuizen voltooid. Er hebben totaal 4350 patiënten ingestemd met deelname aan de studie en van 2165 patiënten hebben we ontlasting of rectale uitstrijkjes inderdaad verkregen. Van deze monsters waren 107 positief voor C. difficile; de C. difficile kolonisatie bij opname was dus 4,9%. Er zijn verschillende ribotypen gevonden, maar zeer opvallend was het geringe aantal patienten met Ribotype 078. Patiënten die gekoloniseerd door C. difficile waren jonger dan niet-gekoloniseerde patiënten. Achtenvijftig gekoloniseerde patiënten en 174 controles hebben ingestemd met deelname aan de case-controlefase die nu is begonnen.

Samenvatting van de aanvraag

BACKGROUND. Both methicillin-resistant Staphylococcus aureus clonal complex 398 (ST398 MRSA) and Clostridium difficile Type 078 have livestock as a common source. A recent study revealed that C. difficle Type 078 and ST398 MRSA pigs coexist in pigs. When admitted to a hospital, colonized patients may develop an endogenous infection or disseminate these pathogens to susceptible individuals who may develop exogeneous infection, both difficult to recognize and treat. OBJECTIVES. The primary aim is to develop a model for patients admitted to the hospital to predict carriage of C. difficile, in particular Type 078. Secondary aims are to establish the risk of colonized patients to developed C. difficile infection (CDI) associated with Type 078, to assess the burden of disease in patients with infection by Type 078 compared to other Types, and to establish the relation of carriage with ST398 MRSA. STUDY DESIGN. A prospective case-control study will be performed in five hospitals (Leiden Medical Centre, Erasmus Rotterdam Medical Centre, Radboud Medical Centre, Canisius Hospital, Amphia Hospital Breda) in various regions in The Netherlands. WORKPLAN. The study will be performed in five hospitals who already participate in the National Sentinel Surveillance for C. difficile infections and the National Surveillance for MRSA infections. All adult patients admitted to wards of Internal Medicine or Surgery will be screened for presence of C. difficile in faeces. Assuming a carriage rate of (at least) 2% and an inclusion percentage of 70%, we need approximately 16,300 screened individuals to obtain 228 cases of patients colonized with C. difficile. To demonstrate that contact with farm animals is a risk factor for C. difficile (Odds Ratio of 3, power of 80% and a type 1 error of 5%) combined with inclusion of other candidate predictors will require a total of 228 cases. From patients tested positive for C. difficile and their negative tested controls, nasal, perineum and throat swabs will be obtained for culturing and typing of MRSA. A case-control study will be performed to associate carriage of C. difficile with risk factors, such as contact with pets or farm animals, recent contact with healthcare, professional contact with children, previous episodes of diarrhoea and previous medications during 3 months before admission in the hospital. Geographic Information Systems (GIS) software will be used to link the colonization data for cases and controls with potential sources of environmental exposures (proximity of livestock farms and farm density) and geodemographic characteristics of populations. Multivariable logistic regression analysis using all defined candidate predictors will be performed to develop a prediction model. Colonized patients and controls will be observed for the development of C. difficile and/or MRSA infection and the study data will be linked with the national databases of C. difficile surveillance (LUMC) and MRSA surveillance (RIVM) to which all participating hospitals contribute. Standardized questionnaires will be used for cases and controls to assess the impact of carriage on costs, mortality and quality of life during hospital stay and until 1 year follow-up after discharge. To determine the relatedness of study isolates and clinical isolates, C. difficile and MRSA strains will be typed by single-nucleotide polymorphism analysis and by whole genome mapping, respectively. EXPECTED OUTCOME. The results of the study will provide evidence-based information whether C. difficile, in particular Type 078, has to be considered as a zoonotical pathogen. This will be complementary to studies currently performed as Livestock Farming and Neighbouring Residents’ Health (VGO) and to an ongoing intervention study among pig farmers, family members and employees from breeding pig farms (BACTOPATH). The results will also provide useful recommendations to prevent development and spread of zoonotically acquired C. difficile infection in hospitals, similar as has been done for ST398 MRSA.
