Tranexamic acid to prevent OpeRation in Chronic subdural Hematoma (TORCH)

Het chronisch subduraal hematoom is een bloeduitstorting tussen het harde hersenvlies en de hersenen, dat vooral bij ouderen optreedt. Door druk op het hersenweefsel ontstaan hoofdpijn en neurologische uitvalsverschijnselen. Door de vergrijzing komt dit ziektebeeld steeds vaker voor, met in 2030 in Nederland 3600 nieuwe patiënten per jaar.

Bij de helft van de patiënten kan vanwege milde symptomen spontaan herstel worden afgewacht. Driekwart moet echter alsnog worden geopereerd omdat de klachten verergeren. Hoewel deze neurochirurgische behandeling effectief is, leiden ziekenhuisopname, narcose en operatie bij deze oudere patiënten vaak tot slechter lichamelijk en geestelijk functioneren, waardoor verlies van zelfstandigheid ontstaat. Het voorkomen van een operatie is daarom gewenst.
Enkele kleine studies beschreven het effect van tranexaminezuur: bij alle 39 behandelde patiënten werd een operatie voorkomen. Deze studie onderzoekt het effect van deze behandeling vergeleken met placebo.

Samenvatting van de aanvraag

Research question A chronic subdural hematoma (cSDH), caused by a bleeding into the space between the dura and arachnoid membranes, develops mainly in elderly, due to brain atrophy and the frequent occurrence of antiplatelet or anticoagulant co-medication. With an expected increase of elderly people, cSDH is expected to become the most frequent neurosurgical condition by 2030, with an incidence of 20/100,000 per year, resulting in 3600 newly-diagnosed patients per year in the Netherlands. Although 50% of the patients is primarily treated conservatively, 75% still need surgery for neurological deterioration. Prevention of surgery might be beneficial as hospital admission, anesthesia and surgery in these frail, elderly patients, often with comorbidities, frequently leads to decreased physical and cognitive functioning, and loss of independence. Tranexamic acid (TXA) treatment in a few small studies showed that surgery could be prevented. The aim of our study is to evaluate whether TXA can prevent surgery for cSDH. Hypothesis: TXA significantly reduces enlargement of the cSDH by blocking hyperfibrinolysis. Study design: superiority, double-blind, multicenter, placebo-controlled RCT. Study population: adults > 50 years with a cSDH. Intervention: TXA treatment (TXA group) or placebo (control group). Outcome measures: surgery for cSDH within 12 weeks after diagnosis; clinical outcome (modified Rankin Scale) (4, 8 and 12 weeks); residual cSDH volume on CT-scan (4, 8 and 12 weeks); quality of life, resource use and associated costs. Sample size/data analysis: expecting a relative risk reduction of 33%, a total of 116 patients is needed (power 80%, alpha 5%). Taking into account some withdrawals, 130 patients will be included. Statistical analyses will be by intention-to-treat.
