Individualized PeMetRexed dOsing in lung cancER and mesothelioma patients to improVE treatment response and allow treatment of patients with impaired renal function – The IMPROVE study (previous acronym: PROPER study).

Projectomschrijving

Het antikankermiddel pemetrexed wordt vaak gebruikt voor de behandeling van longkanker en longvlieskanker. De dosis van pemetrexed wordt zo ingesteld dat er een zo hoog mogelijke concentratie in het bloed komt zonder dat er ernstige bijwerkingen ontstaan. Omdat pemetrexed door de nieren uitgescheiden wordt, kan bij mensen met verminderde nierfunctie stapeling plaatsvinden met ernstige bijwerkingen als gevolg. Andersom kan er bij mensen die een beter dan gemiddelde nierfunctie hebben mogelijk een te lage en ineffectieve blootstelling zijn. Er wordt echter op dit moment nog geen rekening gehouden met de nierfunctie bij het doseren van pemetrexed.

Dit project onderzoekt of de behandeling met pemetrexed veiliger en effectiever kan door ook rekening te houden met de nierfunctie bij het doseren. Hiermee wordt mogelijk de behandeling veiliger voor mensen met een verminderde nierfunctie, maar mogelijk ook veiliger en effectiever bij mensen met een goede nierfunctie.

Samenvatting van de aanvraag

RESEARCH QUESTIONS 1. Can pemetrexed be effectively and safely given to patients with renal dysfunction? 2. Can treatment response to pemetrexed be improved by dose individualization based on renal function in patients with adequate renal function? HYPOTHESIS Pemetrexed treatment response (efficacy and toxicity) correlates well with systemic exposure. The most important determinant for pemetrexed exposure is renal function. However, pemetrexed is dosed based on body surface area and thus renal function is ignored. Therefore, patients with renal dysfunction are withheld effective therapy due to the high risk of toxicity and patients with adequate renal function have an unnecessarily high incidence of inefficacy and toxicity. We postulate dat dose individualization based on renal function overcomes these problems. STUDY DESIGN We will perform a phase II safety/toxicity study of pemetrexed studying individualized dosing of pemetrexed in patients with renal dysfunction and a randomized open label study inevstigating the superiority of individualized dosing of pemetrexed versus standard of care in patients with adequate renal function. STUDY POPULATION Patients eligible for treatment with pemetrexed, irrespective of renal function. INTERVENTION Individualized dosing of pemetrexed based on renal function. OUTCOME MEASURES Primary: pharmacokinetic target attainment and safety Secondary: The population pharmacokinetics of pemetrexed and the comparative performance different algorithms to estimate GFR to predict pemetrexed pharmacokinetics, estimated reduction in treatment costs due to prevention of neutropenia, quality of life. SAMPLE SIZE/DATA-ANALYSIS In the phase II safety and pharmacokinetic study in patients with renal impairment we need to include a maximum of 35 patients and in the randomized study in patients with adequate renal function we need to include 94 patients.
