De laatste jaren is het aantal diagnoses van ADHD en het gebruik van geneesmiddelen voor de behandeling van ADHD gestegen. Dit roept de vraag op of er terecht wordt voorgeschreven. Methylfenidaat is het meest gebruikte geneesmiddel bij ADHD.
Onderzoek
Er is systematisch dossieronderzoek gedaan bij kinderartsen en GGZ-jeugdinstellingen of er terecht methylfenidaat is voorgeschreven en of de bestaande richtlijnen goed worden opgevolgd. Verder is onderzocht bij jongeren die al langer dan twee jaar methylfenidaat gebruiken of het medicijn dan nog steeds werkzaam is. Hier is gekeken wat er gebeurt als de medicatie langzaam wordt afgebouwd.
Resultaten
Uit de dossieraudit bleek dat ook kinderen zonder ADHD vaak methylfenidaat voorgeschreven krijgen en ook dat lang niet altijd de juiste psychosociale zorg wordt geboden. Verder bleek methylfenidaat ook na twee jaar gebruik nog effectief. Opvallend feit was dat bijna de helft van de kinderen ook na afbouw van de medicatie goed bleef functioneren.

Aanleiding
Methylfenidaat is verreweg het meest gebruikte geneesmiddel bij kinderen met ADHD. De laatste jaren is het gebruik van ADHD enorm gestegen. Dit roept de vraag op in hoeverre er zorgvuldig wordt voorgeschreven en of er misschien sprake is van ten onrechte, te snel en/of te lang voorschrijven van methylfenidaat aan kinderen.
Onderzoek
We gaan daarom systematisch dossieronderzoek doen bij kinderartsen, GGZ-jeugdinstellingen en huisartsen of er terecht methylfenidaat is voorgeschreven en of de controles op orde zijn. Verder onderzoeken we bij jongeren die al langer dan twee jaar methylfenidaat gebruiken of het medicijn dan nog steeds werkzaam is. We doen dit door te kijken naar wat er gebeurt als de medicatie langzaam wordt gestopt.
Doel
Dit project beoogt te komen tot belangrijke verbeteringen in het gebruik van methylfenidaat bij kinderen en jongeren in Nederland.

Samenvatting van de aanvraag
Recently, attention-deficit/hyperactivity disorder (ADHD) has received much and almost exclusively negative coverage by media, politicians, and government due to concerns about overdiagnosis and overtreatment with medication, in particular psychostimulants. Over the past decade, we have witnessed a rapid and tremendous increase in the diagnosis of ADHD and in the use of medication treatments for ADHD, in particular methylphenidate. It is unclear whether overdiagnosis is a fact, i.e. whether children are being diagnosed (and medicated) in spite of falling below the diagnostic threshold set by the DSM-IV system. A second issue is that the increased and longer use of methylphenidate contrasts heavily with the lack of long term data on effectiveness. In fact, Dutch multidisciplinary guidelines state that there is currently only evidence for the long term effectiveness of methylphenidate for a treatment duration between three months and two years. A related problem concerns uncertainty about the current prescription practice of methylphenidate, which has resulted in increased levels of public concern and several reviews by professionals working in the field who have attacked the widespread prescribing of methylphenidate. This proposal describes a series of studies to address these concerns. Our proposal has two main parts: 1. A large scale audit to study the current long term prescription practice and policies of methylphenidate in daily clinical practice. The goal is investigate how well existing national and international professional guidelines are being adhered to. 2. A double-blinded placebo-controlled discontinuation trial to investigate the effectiveness of treatment continuation with methylphenidate beyond two years of treatment. The current proposal forms a perfect match to the recently funded ZonMw project Methylphenidate Treatment and the Developing Brain in Attention-Deficit/Hyperactivity Disorder (funded under the ZonMw program Priority Medicines Kinderen), which focuses on the long term neurobiological safety aspects of methylphenidate treatment in children. Moreover the recently funded EU FP7 study ADDUCE in which the current applicants are participating, addresses the issue of long term safety of psychostimulant treatment; that is why we here focus on long term effectiveness and prescription practice. As methylphenidate is not protected by patents anymore, it is not to be expected that the pharmaceutical industry will initiate the type of studies that we propose. Yet, we have been able to obtain co-financing by the pharmaceutical industry (Shire) through an unrestricted research grant. Our proposal will generate new knowledge with direct relevance for daily clinical practice and will lead to much needed improvements on the long term prescription practice of methylphenidate in the Netherlands. An implementation plan will be a dedicated part of the project.
