Analgosedation after cardiac surgery in infants: A "model-based" nation-wide randomized controlled trial.

In Nederland worden jaarlijks 800 grote hartoperaties uitgevoerd bij jonge kinderen. Na de operatie krijgen de kinderen meestal morfine om pijn tegen te gaan. Helaas kan morfine leiden tot een verminderde ademhaling of verlaagde bloeddruk. Mogelijk kan morfine vervangen worden door intraveneuze paracetamol.

Studie

Bij dit onderzoek in de vier kinder-cardiochirurgische centra in Nederland gaan de onderzoekers na of op deze manier de totale morfinedosering in de eerste 48 uur na de operatie minstens 30% lager kan.
In totaal worden 204 kinderen van 0-36 maanden willekeurig verdeeld over twee groepen, hetzij intraveneuze paracetamol hetzij intraveneuze morfine als eerste keus pijnstiller na de operatie. In beide groepen kan extra morfine worden toegediend een kind toch nog pijn heeft.

Verwachte resultaten

De resultaten vormen de basis voor een protocol voor het gebruik van pijnstillers na een grote hartoperatie in de leeftijdsgroep van 0-36 maanden, geldend voor de kinder-cardiochirurgische centra in Nederland.

Samenvatting van de aanvraag

In the Netherlands annually 800 children with congenital cardiac defects undergo cardiac surgery, 55% within the first year of life. Worldwide, dosing of analgesic agents given after surgery differ as no evidence-based guidelines are available and population or model based dosing regimens have not been established. Morphine is the most used analgesic agent worldwide in these patients. Our group has shown that morphine dosages could be successfully reduced or replaced by IV paracetamol after major non-cardiac surgery in infants up to 12 months of age. This is an important finding considering the adverse effects of morphine such as hypotension. The main objective of this research proposal is to develop a model-based protocol for analgesia for pediatric patients after cardiac surgery. After approval from the Medical Ethics Review Board a multicenter double-blind randomized controlled trial will start at intensive care units of all four pediatric cardiac surgery centers in the Netherlands. 200 children between 0-36 months old, will be randomized to receive postoperatively either iv morphine or iv paracetamol as primary analgesic. Additional morphine will be given as required with total morphine dose as the primary endpoint. Rescue morphine will be administered based on pain assessment with the COMFORT-B scale. The results will form the basis for a national guideline on analgesia after cardiac surgery in children aged 0-36 months to be implemented in all four centers.
