ESPRIT studie: Zorgvuldige invoering van NIPT

Vraagstuk

De opkomst van non-invasief prenataal testen (NIPT) zal naar verwachting grote veranderingen teweeg brengen in de prenatale screeningspraktijk. Bij prenataal screenen – het bepalen van het risico op een aangeboren afwijking bij een foetus – gaat het onder meer om chromosomale afwijkingen waarvan Down syndroom (trisomie 21) de bekendste is. Sinds kort is een DNA test beschikbaar die trisomie 21 potentieel zonder miskraamrisico kan vaststellen enkel op basis van een bloedmonster van de zwangere. De voordelen lijken evident. De verschillende aandoeningen waarop een foetus getest kan worden zal naar verwachting ook toenemen. Daarmee betekent invoering van NIPT een mogelijk radicale verandering in de manier waarop we naar prenatale screening kijken.

Onderzoek

De ESPRIT studie (Ethical Social Psychological Research for Implementing Screening for fetal Trisomies) heeft door middel van vragenlijsten en (groeps)interviews de mening van zwangeren en haar partner en betrokken stakeholders t.a.v. NIPT in kaart gebracht. Ook is er een ethische analyse gedaan.

Uitkomst

De resultaten van de ESPRIT laten zien dat zwangeren en hun partner barrières voor de huidige deelname aan prenatale screening ervaren. De kenmerken van NIPT (vroege, veilige en accurate test) kunnen volgens zwangeren de drempels voor deelname wegnemen. Vrouwen en hun partners zouden de keuze voor deelname aan prenatale screening meer kunnen baseren op hun wens om te testen, dan vanwege de angst voor een miskraam of onzekere testuitslag. Anderzijds werd ook een keerzijde van deze laagdrempeligheid benoemd. Invoering van NIPT zou mogelijk kunnen leiden tot meer ondoordachte keuzes, routinisering en (sociale) druk. Een breder aanbod van testen voor ernstige aandoeningen werd als acceptabel en zelfs wenselijk gezien, omdat dit veel leed zou kunnen voorkomen. Wel gaf men aan dat dit zwangeren en hun partners voor keuzes zou kunnen plaatsen die zij niet goed kunnen overzien.

De opkomst van niet-invasief prenataal testen (NIPT) zal naar verwachting grote veranderingen teweeg brengen in de prenatale screeningspraktijk. NIPT is een nieuwe testmethode waarbij het bloed van de zwangere wordt gebruikt om in het laboratorium te testen of het ongeboren kind mogelijk trisomie 21 (downsyndroom), trisomie 18 (edwardssyndroom) of trisomie 13 (patausyndroom) heeft. NIPT is sinds 1 april 2014 in Nederland beschikbaar binnen de TRIDENT-studie voor zwangeren die op basis van de combinatietest een verhoogd risico hebben op een kind met een trisomie 21, 18 of 13 (www.meeroverNIPT.nl). NIPT heeft als voordeel dat bij de meeste zwangeren geen vervolgonderzoek (vlokkentest of vruchtwaterpunctie) meer nodig is. Daardoor kunnen die zwangeren de kans op een miskraam vermijden.
De verwachting is dat met NIPT in de toekomst ook op andere erfelijke en aangeboren aandoeningen getest zal kunnen worden. Voor een succesvolle implementatie van NIPT is draagvlak van, en onderzoek naar de mening van zwangeren en hun partners essentieel. In dit ZonMw project (de ESPRIT-studie; Ethical Social Psychological Research for Implementing Screening for fetal Trisomies) is met vragenlijsten en (groeps-)interviews onderzocht hoe zwangeren en hun partner denken over deze nieuwe testmethode. De introductie van deze testen zal zorgvuldig moeten worden voorbereid. Met name is het van belang dat goed geïnformeerde keuzes van zwangeren gewaarborgd blijven. Het onderzoek richtte zich daarom op de mogelijke psychologische, ethische en maatschappelijke gevolgen van deze test. Ook is onderzoek gedaan naar de mening van ouders van een kind met een aangeboren aandoening, zorgverleners, beleidsmakers en overige betrokkenen. Het onderzoek is afgesloten met een stakeholderbijeenkomst met als doel de aanknopingspunten en uitdagingen voor een verantwoorde introductie van NIPT in Nederland in kaart te brengen.

Samenvatting van de aanvraag

Counseling and informed decision making of pregnant women in the field of prenatal screening are about to be challenged. The scope of first trimester screening will widen through rapid developments in non-invasive prenatal diagnostics (NIPD). In NIPD circulating fragments of fetal DNA/RNA in maternal plasma, obtained by a simple maternal blood sample, are analyzed. As soon as NIPD is validated in a large Dutch population-trial (the NITRO study) and its near 100% sensitivity and specificity is proven for the identification of Down syndrome (trisomy 21) in Dutch practice, Down syndrome screening by NIPD will be implemented nationwide. NIPD for fetal Rhesus D status and sex determination is already implemented in the Netherlands. In the future, rapid technical improvement and development of NIPD will make non-invasive screening available for a wide range of (non-)treatable congenital disorders and feto-maternal risk factors. Current prenatal screening in the Netherlands is performed by the so-called combined test (CT), in Dutch referred to as 'combination test'. The CT combines the measurement of the fetal nuchal translucency (a translucent area in the neck of the fetus between the skin and the underlying tissue) by ultrasound and two maternal serum markers (PAPP-A and beta-hCG) analyzed from maternal blood with the maternal age between 11 and 14 weeks’ gestation. Subsequently, a risk for trisomy 21, 18 and 13 is calculated per individual patient. If the risk exceeds 1:200, invasive prenatal testing is offered by chorionic villus sampling or amniocentesis. Both tests can cause a miscarriage of a potentially healthy fetus. Because the test characteristics of the CT are not optimal (at least 10% of trisomy 21 cases are missed, for a false positive rate of 5% (1)) hundreds of women undergo invasive testing while only a few actually carry a fetus with trisomy 21. Many women therefore are reluctant to request even the non-invasive screening, out of fear of having to proceed to the risky invasive test. Given its superior predictive value to the CT, and the absence of the associated miscarriage risk, NIPD is not only expected to be more cost-effective, but may also lead to a higher uptake of prenatal screening. Whether or not NIPD becomes a one-step procedure, it will affect counseling and decision-making dynamics. Furthermore, since NIPD may both be used to detect untreatable disorders relevant for a possible abortion decision and treatable conditions relevant for a healthy pregnancy outcome, the distinction between traditional frameworks for prenatal screening may become blurred. The widening scope and increasing complexity of first trimester screening demands reflection on the ethical, psychological and social consequences. Apart from a need for technical validation of NIPD in Dutch health care practice, there is a need for parallel development of new counseling tools to shape imminent prenatal screening practices that will safeguard future informed decision making in couples, and tools for public debate on ethical, legal and social consequences. We aim to study psychological and societal factors relevant to the responsible implementation of NIPD-based prenatal screening. We will study women’s preferences and assess the impact of NIPD-based prenatal screening on counseling needs and on decision-making dynamics. In an ethical analysis of the findings, we will evaluate the fit between this new development and the accepted normative frameworks for prenatal screening and prenatal care. We will also study the views and preferences of patient representatives (The Dutch Genetic Alliance,VSOP) and healthcare professionals and use these for assessing the expected health care implications of the introduction of NIPD-based prenatal screening. The required empirical data will be collected through personal interviews (pregnant women, health care professionals), focus group interviews (pregnant women, health care professionals), and online questionnaires (pregnant women). A stakeholders conference will be organized to discuss responsible implementation in health care.
