Evaluation of birth centre care in the Netherlands. Evaluation of the effects on quality of care, experiences of clients and caregivers, economic outcomes and implications for further implementation of birth centre care.

In een geboortecentrum kunnen vrouwen bevallen onder leiding van een verloskundige met hulp van een kraamverzorgende. De afgelopen tijd zijn er veel geboortecentra opgericht. Opvallend is de grote diversiteit die voortkomt uit de verschillen in ontstaansgeschiedenis en organisatie van de geboortecentra. Een overkoepelende beleidsvisie over de plaats van geboortecentra en hun mate van integratie in het verloskundige systeem ontbreekt en er is geen inzicht in de bijdrage die geboortecentra leveren aan de kwaliteit van de verloskundige zorg zoals effect op gezondheidsuitkomsten van moeder en kind, ervaringen van vrouwen en zorgverleners, en kosten-effectiviteit. In deze evaluatie wordt op brede en uitlopende wijze gekeken naar de organisatie en integratie van geboortecentra en de effecten van geboortecentrumzorg op de kwaliteit van en ervaring met verloskundige zorg in de regio. Kwalitatieve en kwantitatieve onderzoeksmethoden zullen worden gebruikt en de resultaten zullen in samenhang worden geanalyseerd waardoor concrete aanbevelingen zullen kunnen worden gegeven.

Vrouwen met een laag risico op complicaties tijdens de bevalling kunnen kiezen of ze onder begeleiding van een verloskundige thuis of poliklinisch in het ziekenhuis willen bevallen. Op veel plekken is het nu ook mogelijk om te kiezen voor een bevalling in het geboortecentrum. September 2013 waren er in Nederland 23 geboortecentra. Zij hebben allemaal deelgenomen aan het Geboortecentrum Onderzoek, uitgevoerd door Erasmus MC, Jan van Es Instituut, LUMC, NIVEL, UMC Utrecht, Universiteit van Tilburg en TNO.

Geboortecentra in Nederland zijn een prima plek om te bevallen. De uitkomsten van de bevalling voor moeder en kind, de waardering van vrouwen met een geplande geboorte in een geboortecentrum en de kosten zijn vergelijkbaar met een geplande poliklinische bevalling. Vrouwen die planden om thuis te bevallen hadden betere uitkomsten. Hoewel de geboortecentra in Nederland onderling zeer verschillend zijn in organisatie en inrichting, verschilt de kwaliteit van zorg nauwelijks. Samenvatting van de aanvraag

INTRODUCTION Birth centres are regarded as settings where women with uncomplicated pregnancies can give birth, assisted by a midwife and a maternity care assistant. In case of (threatening) complications referral to a maternity unit of a hospital is necessary. In the last decade up to 20 different birth centres have been instituted in the Netherlands. The sudden increase in birth centres is explained by different parties by referring to various reasons such as a safe and easy accessible place of birth, organizational efficiency in integration of care and direct access to obstetric hospital care if needed, and better use of maternity care assistance. Birth centres are assumed to offer increased quality of care because of improved integrated care and thus contribute to better perinatal and maternal outcomes. So far there is no evidence for that assumption. AIMS The objectives of the study are: 1. To determine process, structure and outcome quality indicators with which to assess the quality of birth and post partum care in a birth centre, in collaboration with the various care providers and clients involved. 2. To develop a typology of birth centres based on the level of integrated care. The structure and process indicators of the quality of care as addressed in objective 1 will be included in this typology. 3. To assess the output in terms of optimality and adverse outcomes of birth centre care in relation to the different types of birth centres (see objective 2). 4 To study the impact of the start of a birth centre on the local adjacent birth and post partum care system by comparing care outcomes before and after the introduction of a birth centre. 5. To study the cost effectiveness of the various forms of birth centre care compared with usual care (home birth and birth in a hospital under midwifery led care). 6. To assess client experiences and experience of care givers working in and with a birth centre. 7. To perform a longitudinal multiple case study investigating the processes of organizational integration in a limited number of selected birth centres from an operational, medical, behavioural and administrative perspective and how these perspectives are intertwined and interact over time, thereby leading to physical, psychological and social health outcomes. 8. To translate results of this study into recommendations for future birth and post partum care. METHODOLOGY To address the various objectives we distinguish three phases in our project: a Definition phase, an Evaluation phase and an Integration Phase. In the Definition phase the birth centres will be described with an agreed upon set of indicators. First (phase 1A) consensus will be reached on a limited set of indicators of quality of care which consists of process, structure and outcome indicators (Objective 1). Phase 1B sets out to describe all birth centres on structure and process indicators to measure the level of integrated care (Objective 2). In the Evaluation phase, the output of the birth centres will be empirically examined. Hereto several different evaluations will be performed. First, in phase 2A for all existing birth centres that participated in phase 1B outcome indicators will be collected (objective 3). In Phase 2B a pre-posttest study is executed for starting birth centres to study the effect of the start of a birth centre in a region (objective 4). In Phase 2C a cost-effectiveness study is performed for a subset of 12 birth centres (objective 5), in which birth centre care is compared to home birth and birth in the hospital under midwifery-led care. The satisfaction of professionals and clients will be assessed in Phase 2D (objective 6). This is done in the same set of birth centres that participate in the cost-effectiveness study (phase 2C). Finally in phase 2E, from the 12 birth centres participating in phase 2C and phase 2D, up to 6 birth centres are selected for an in-depth longitudinal analysis of the implementation process of integrated care (objective 7). RESULTS AND CONCLUSION The insights of phases 1 and 2 will be combined in Phase 3, the Integration phase. The national quantitative results will be combined with the insights of the mirror sessions, the cost effectiveness results, client and professional experiences and the information of the in-depth study and lead to recommendations. In a symposium the results will be presented and the implications for future birth and postpartum care will be discussed (objective 8).
