Steeds vaker besteden Nederlandse scholen aandacht aan gezond eet- en beweeggedrag met de Gezonde School aanpak. Schoolprogramma’s beklijven echter moeizaam, waardoor effecten op lange termijn beperkt lijken te zijn.

Onderzoek

Met het creëren van een gezonde school community werd beoogd de Gezonde School aanpak op een praktische, duurzame wijze vorm te geven, uitgaande van de ‘eigen kracht’ in de school. Leerlingen, schoolpersoneel en ouders werkten samen aan een Gezonde School met activiteiten die pasten bij de context en populatie van de school en die het maken van gezonde keuzes in eet- en beweeggedrag op school én thuis stimuleerde.

Uitkomst

Uit interviews bleek dat de ‘eigen kracht’ van interventiescholen was toegenomen. Motiverend en verbindend leiderschap was daarvoor cruciaal en resulteerde in een positievere houding en breder gedragen activiteiten, die pasten bij de school. Scholen hebben ondersteuning nodig om de aanpak beter te borgen in een dynamische omgeving en om netwerken buiten school te benutten. Resultaten op BMI, eet- en beweeggedrag vergeleken met vier controlescholen volgen.

Het doel van het FLASH-project (Fit Lifestyle At School and at Home) was om het proces van capacity building in een school community, waaronder het ontwerpen en implementeren van gezondheid bevorderende activiteiten, te monitoren en om te onderzoeken of dit proces de community capacity versterkte en vervolgens gezond eet- en beweeggedrag bij vmbo-leerlingen bevorderde. De school community bestaat uit leerlingen, schoolpersoneel en ouders.
Steeds vaker besteden Nederlandse scholen aandacht aan gezond eet- en beweeggedrag met de Gezonde School aanpak. Schoolprogramma’s beklijven echter moeizaam, waardoor effecten op lange termijn beperkt lijken te zijn. Te vaak is schoolgezondheidsbevordering afhankelijk van één enthousiaste leraar, voelen de stakeholders onvoldoende eigenaarschap, en zijn ouders niet betrokken. Met het creëren van een gezonde school community werd beoogd de Gezonde School aanpak op een praktische, duurzame wijze vorm te geven, uitgaande van de ‘eigen kracht’ in de school. Leerlingen, schoolpersoneel en ouders werkten samen aan een Gezonde School met activiteiten die pasten bij de context en populatie van de school en die het maken van gezonde keuzes in eet- en beweeggedrag op school én thuis stimuleerde. In FLASH is gebruik gemaakt van 4 strategieën voor capacity building:
1) het identificeren van leiders of sleutelfiguren in de gemeenschap, die ook anderen kunnen motiveren; 2) het creëren van een participatieve schoolcultuur, waarin alle leden van de schoolgemeenschap de mogelijkheid krijgen om mee te denken en/of beslissen; 3) het gezamenlijk en systematisch ontwerpen of kiezen, implementeren en evalueren van gezondheid bevorderende activiteiten, uitgaande van de vier pijlers van de Gezonde School aanpak; 4) het ontwikkelen en benutten van een lokaal netwerk dat bijdraagt aan de realisatie van de activiteiten

Het onderzoeksproject is een samenwerking van de Vrije Universiteit Amsterdam, Hogeschool Windesheim, GGD IJsselland en Landstede Groep. Het project had vier interventie- en vier controlescholen. Iedere interventieschool had een Gezonde School Coördinator (GSC, een docent binnen de school), die werd gecoacht door een expert in onderwijs en een expert in gezondheidsbevordering.
Het project kende 3 delen:
1. Inzicht krijgen in ervaringen, gedrag, context, behoeftes, kansen en belemmeringen van leerlingen, schoolpersoneel en ouders in de school community m.b.t. gezond eet- en beweeggedrag. Data verzameling vond onder meer plaats via vragenlijst onderzoek, meten van lengte/gewicht/middelomtrek, photovoice, focusgroepen en interviews.
2. In co-creatie ontwikkelen van een school-community-based integrale aanpak, uitgaande van de vier pijlers van de Gezonde School. Hiervoor werden o.a. technieken uit design thinking, nudging en educatie ingezet om leerlingen, ouders en schoolpersoneel actief te betrekken. De scholen zelf zijn daarbij ‘in the lead’.
3. Evaluatie van deze integrale aanpak. Vanaf de start van het project is het proces van capacity building in de school community structureel en systematisch gemonitord. Daarnaast is een quasi-experimentele studie uitgevoerd met de 4 interventie- en 4 controlescholen om te exploreren in hoeverre het proces van capacity building, gepaard is gegaan met veranderingen in BMI en eet- en beweeggedrag bij vmbo-leerlingen.

Samenvatting van de aanvraag

When growing up, adolescents tend to adopt unhealthy dietary and physical activity behavior that is associated with a rising prevalence in overweight and obesity. Especially adolescents attending prevocational schools are a vulnerable population at risk for unhealthy behavior and overweight. Schools are an opportune setting to encourage a healthy lifestyle in adolescents because of their pedagogical task and potential to reach youth. The WHO advocates for a research-based, whole school health approach in which curricular health education is combined with a favorable environment, a school health policy and partnerships with stakeholders such as parents, organizations for youth health care and sports. In the Netherlands, many schools have adopted the integrated Healthy School Approach to some degree, but experience a challenge in implementing the approach in every day life at school in a sustainable way. In this regard a prerequisite is the presence of sufficient community capacity, that needs to be built through external support. Another prerequisite is the involvement of stakeholders including teachers, parents and adolescents in program design, implementation and evaluation from the start, so their values and concerns will be met, which will lead to support, understanding and ownership. The aim of this study is to develop a method to increase community capacity in a school by means of a participatory action approach in order to improve the sustainable implementation of a school health promotion program based on the Dutch Healthy School Approach. The purpose of the school health promotion program is to improve healthy dietary and physical activity behavior and a healthy weight in adolescents in prevocational schools. The process of building the school community capacity and the impact of the school health promotion program will be studied in order to learn whether this method is feasible and effective in stimulating healthy behavior in adolescents. Key elements in the project are: 1. Implementing a whole school approach: the ‘Healthy School Approach’, applicable to the Dutch school system; 2. Increasing community capacity of the school; 3. Involving stakeholders in design, implementation and evaluation of a school health promotion program, targeting healthy dietary and physical activity behavior; 4. Taking the pupils’ motives, needs and ideas as starting point, which will increase the commitment of other pupils, parents and teachers 5. Selecting and implementing strategies that fit with the opportunities and capacities of the school, such as establishing social norms, applying nudges or prompts to encourage healthy behavior and making healthy choices easier. In the project 4 intervention and 4 control schools participate. The project consists of three phases. In a preliminary phase a healthy school working group will be established and led by a healthy school coordinator. Subsequently, in the first phase a needs assessment will be carried out to gain insight in barriers, benefits and needs of pupils, teachers and parents regarding (stimulating) healthy dietary and physical activity behavior. A baseline measurement will be carried out to collect data on (determinants of) health behaviors, length and weight on all schools. In addition a photovoice study, lesson-observation study and in depth group discussions among various stakeholders will be carried out in the intervention schools and school health goals will be set. In phase 2 the designed health promotion program, including the initiation of activities and the evaluation, will be implemented. The program will be managed by the healthy school coordinator and the healthy school working group coached by experts in health promotion and education. Other stakeholders in the school community will be actively involved. Teachers in health and physical education will improve their health promotion competences by means of practice development. Communication of stakeholders proceeds via social media. In phase 3, the working method will be evaluated on process and outcome level. The feasibility, acceptability, fidelity and satisfaction will be evaluated applying mixed methods by using questionnaires, focus group discussions and a digital registration system (study 1). Effects of the method on (determinants of) dietary and physical activity behavior and BMI in adolescents (study 2) and on motivation, capabilities and opportunities of teachers and parents to stimulate healthy behavior of adolescents (study 3) will be evaluated in a quasi-randomized controlled trial. This project is a unique collaboration between representatives of education (ranging from prevocational schools to university) practice, research and policy. Therefore, scientific and practical expertise on health promotion, education and policy is combined and will be used to support schools to develop a feasible method for school health promotion in real-life conditions.
