DUTCH-AF Registry – Prospective evaluation of dosing and adherence of anticoagulant treatment and the risk for bleeding in atrial fibrillation

Projectomschrijving

A nationwide registration of patients with atrial fibrillation (848050007).

Samenvatting van de aanvraag

Atrial fibrillation (AF) is the most common cardiac arrhythmia and can lead to stroke. Preventive drugs comprise of vitamin K antagonists (VKAs) and non-vitamin K-dependent oral anticoagulants (NOACs). Adherence to NOACs is important because of short half-life and lack of anticoagulant control, while VKAs are monitored by lab control. Research shows that 40-60% of patients with AF took a wrong dose or stopped NOACs within 1 to 2 years. An important reason for this misuse or stopping is fear of bleeding. With this research project we want to: 1) investigate daily use (dosing and adherence) and its relationship with the outcomes, 2) determine which patient characteristics are related to improper use and 3) validate and refining existing bleeding models. This information is vital to optimize safety and efficacy of oral anticoagulation therapy. With the results physicians are able: 1) to exactly explain the risk of wrong NOAC dosing to patients, 2) to improve adherence and monitoring, and 3) better predict individual’s bleeding risk, and eliminate modifiable risk factors. We will connect a large, detailed and prospective registration to national pharmacy data and information from clinical outcomes. This study will contribute to the understanding of anticoagulation therapy and improve its efficacy and safety in daily practice throughout the healthcare chain.
