Na een polsbreuk kan de pols in een verkeerde stand vastgroeien. Deze scheefstand kan operatief  gecorrigeerd worden door een wig in het bot te zagen, waardoor de pols in een goede stand kan worden gezet en vastgezet met plaat en schroeven. Het ontstane botdefect wordt veelal opgevuld met bot uit de bekkenkam. Deze operatie wordt in de meeste ziekenhuizen uitgevoerd en wordt gezien als de standaardbehandeling. Het voordeel van het opvullen van het botdefect is dat het mogelijk de botgenezing kan bevorderen, maar er kunnen ook complicaties optreden zoals pijnklachten, infectie en nabloeding ter hoogte van de oogstplek. Op basis hiervan wordt in sommige ziekenhuizen het botdefect niet opgevuld. Momenteel is het onduidelijk welke behandeling de beste is. Daarom wordt in deze studie onderzocht of het beter is om het botdefect op te vullen of juist niet. Om dit te kunnen bepalen wordt na de operatie gekeken naar kwaliteit van leven, complicaties, herstel in polsfunctie, botgenezing en kostenbesparing.

Samenvatting van de aanvraag

Nederlandse samenvatting VRAAGSTELLING Zal er een betere kwaliteit van leven en kostenbesparing zijn wanneer er geen bot uit de bekkenkam gehaald wordt bij patiënten die een operatieve correctie ondergaan van hun pols in vergelijking met patiënten die de standaardbehandeling ondergaan met het verkrijgen van bot uit de bekkenkam tijdens de operatieve standscorrectie van hun pols? HYPOTHESE We verwachten dat de patiënten, bij wie geen bot uit de bekkenkam is gehaald tijdens de operatieve standscorrectie, een betere kwaliteit van leven ervaren. Zij zullen geen complicaties hebben ten gevolge van het oogsten en het gebruik van bekkenkambot. Hiermee zullen ook kosten bespaard worden aangezien er geen noodzaak is voor een langere operatie- en opnameduur, heroperaties en/of langdurig gebruik van pijnmedicatie. De studieresultaten zullen ook relevant zijn voor operatieve correcties in andere metafysaire botgebieden, waarbij het gebruik van bekkenkambot de standaardbehandeling is. STUDIEOPZET Dit is een gerandomiseerde, prospectieve, multicenter studie. Bij de patiënten zullen de volgende onderzoeken eenmalig voor de operatie en vijf keer na de operatie gedaan worden: 1) het invullen van 3 vragenlijsten, 2) het noteren van eventuele complicaties, 3) het testen van de polsfunctie en 4) het maken van röntgenfoto’s en CT scans. STUDIEPOPULATIE Patiënten met de leeftijd van 18 jaar of ouder die klachten hebben van een afwijkende stand van hun pols ten gevolge van een breuk en die geschikt zijn voor een operatieve standscorrectie van hun pols. INTERVENTIE Standscorrectie van de pols ofwel open wig correctie osteotomie en plaatfixatie. GEBRUIKELIJKE ZORG Open wig correctie osteotomie en plaatfixatie met verkrijgen van bot uit de bekkenkam. UITKOMSTMATEN Primaire uitkomstmaten zijn: complicaties en kwaliteit van leven. Secundaire uitkomstmaten zijn: botgenezing, functionele uitkomstmaten en kosteneffectiviteit. SAMPLE SIZE BEREKENING/DATA-ANALYSE We hebben berekend dat er tenminste 104 patiënten nodig zijn in deze studie. KOSTENEFFECTIVITEITSANALYSE/ BUDGET IMPACT ANALYSE Voor de economische evaluatie zal een kosten-utiliteitsanalyse uitgevoerd worden, waarbij ‘quality adjusted life years’ (QALY’s) als uitkomstmaat wordt meegenomen. Hierbij worden kosten vanuit het ziekenhuis, maar ook vanuit het maatschappelijk perspectief meegenomen. Een budget-impact analyse zal volgens de Nederlandse richtlijnen uitgevoerd worden. TIJDPAD 0-3 maanden: studievoorbereiding 3-33 maanden: inclusie van patiënten 33-45 maanden: follow-up 45-48 maanden: analyse en disseminatie English summary RESEARCH QUESTION Does surgical correction without bone grafting in patients with malunited distal radius fractures lead to a better quality of life and reduce the costs compared to the standard procedure, in which bone is harvested from the iliac crest during the surgical correction? HYPOTHESIS We hypothesize that patients will have a better quality of life when the surgical correction of their malunited distal radius fracture does not involve the harvesting of bone from the iliac crest, as they will be spared the complications associated with bone grafting. Further, we expect this approach to be more cost effective than the standard treatment, as patients will be able to avoid a longer operation and hospital stay, reinterventions and/or prolonged use of pain medication. The study findings will also be relevant for surgical corrections in other metaphyseal bone areas, where bone grafting from the iliac crest is also the standard treatment. STUDY DESIGN This is a prospective, randomized, controlled multicenter study. Patients will undergo the following examinations once before the operation and five times afterwards: 1) the patients will fill out three questionnaires, 2) complications will be noted, 3) the wrist function will be measured, and 4) radiographs/CT scans will be made. STUDY POPULATION All patients over the age of 18 years who have a symptomatic malunion after distal radius fracture and are eligible for surgical correction. INTERVENTION Surgical correction in the form of open wedge corrective osteotomy and plate fixation without bone grafting. USUAL CARE Open wedge corrective osteotomy and plate fixation with harvesting bone from the iliac crest. OUTCOME MEASURES Primary outcomes: complications and quality of life. Secondary outcomes: time to complete bone healing, functional outcomes, and cost effectiveness. SAMPLE SIZE/DATA-ANALYSIS We calculated that at least 104 patients are needed in this study. COST-EFFECTIVENESS ANALYSIS/BUDGET IMPACT ANALYSIS A cost-utility analysis will be performed expressing health improvements in quality-adjusted life years (QALYs), including healthcare costs and societal costs. In addition, a budget-impact analysis will be conducted in accordance with the Dutch guidelines. TIME SCHEDULE 0-3 months: study preparation 3-33 months: patient inclusion 33-45 months: follow-up 45-48 months: data analysis and implementation
