Aanleiding

Jaarlijks worden 20.000 ouderen met een gebroken heup opgenomen in het ziekenhuis. Een kwart van deze patiënten raakt verward (delier), hierdoor liggen zij langer in het ziekenhuis, komen vaker terecht in een verpleeghuis en hebben een grotere kans om te overlijden. Delier is een ernstige aandoening. Risicofactoren voor het krijgen van een delier zijn niet goed behandelde pijn en het toedienen van opioïden (bv morfine) via het infuus. Voor het optimaal behandelen van de pijn kunnen de zenuwen rondom de heup verdoofd worden met een verdovingsvloeistof (lokaal zenuwblok), hierdoor vermindert het gebruik van morfine.

Opzet

Op de Spoedeisende Hulp loot de patiënt met een gebroken heup voor standaard behandeling met pijnmedicatie via het infuus of een continu zenuwblok; een lokaal zenuwblok met achterlaten van een slangetje, zodat gedurende de ziekenhuisopname continue pijnstilling kan wordt toegediend.

Samenvatting van de aanvraag

DOEL/VRAAGSTELLING Leidt een op de SEH echogeleid toegediend continu Fascia Iliaca Compartment Zenuwblok (FICB) met achterlating van een catheter voor continue toediening van locaalanesthetica (LA) tot een vermindering van het optreden van een delier bij oudere patienten met een heupfractuur? HYPOTHESE Vroegtijdige, efficiente, continue regionale pijnbestrijding vermindert de behoefte aan systemische analgetica (minder systemische bijwerkingen) en reduceert de incidentie van een delier gedurende het klinisch beloop bij patienten met een heupfractuur STUDIE OPZET Open multi-center RCT met follow-up tot 3 maanden na ontslag STUDIEPOPULATIE 340 patienten (>55 jaar) met een heupfractuur op de SEH. Exclusie criteria zijn meerdere letsels; delier bij presentatie en onvermogen tot het geven van informed consent. INTERVENTIE Op de SEH < 2uur met echogeleide plaatsing van een continu FICB, de geplaatste catheter blijft tot 2 dagen postoperatief zitten voor continue pijnbestrijding d.m.v. titrateren van LA VERGELIJKING Gebruikelijke zorg met systemische analgetica (nationale richtlijn): opioiden, paracetamol, NSAIDs UITKOMSTMATEN Primaire uitkomst: optreden van een delier tot 3 maanden na ontslag. Secundaire uitkomsten: pijn, patient- en staftevredenheid, ziekenhuisopname karakteristieken, gezondheidsstatus, kwaliteit van leven, economische evaluatie SAMPLE SIZE BEREKENING Superioriteitsanalyse voor vinden minimaal klinisch relevant verschil van 13% in delier incidentie met power van 80%, een alfa van 5% en een mogelijke loss to follow-up van 10%. Hiervoor zullen in totaal 340 patienten nodig zijn KOSTENEFFECTIVITEITSANALYSE/BUDGET IMPACT ANALYSE (BIA) Hypothese: het gebruik van een continu FICB bij ouderen met heupfracturen op de SEH leidt tot vermindering van kosten. De potentiele impact van deze behandelstrategie op de kosten van de gezondheidszorg van Nederland wordt vast gesteld middels een BIA. Deze wordt gebaseerd op klinische data, inclusief effectiviteit en gerelateerde kosten van een continu FICB, evenals de grootte en karakteristieken van de bevolking TIJDPAD Na 6 maanden voorbereiding start inclusie in 2016 (29 mnd) gevolgd door 6 maanden analyse en verslaglegging OBJECTIVE/RESEARCH QUESTION Does an ultrasound guided Fascia Iliaca Compartment Block (FICB) employing an intra-compartmental catheter (continuous FICB, providing continuous anesthesia), applied in the Emergency Department (ED), reduces delirium in elderly hip fracture patients? HYPOTHESIS Due to superior and continuous regional pain treatment from a very early starting point in the clinical course, patients experience less pain and require less systemic analgesics. Incidence of delirium during the complete clinical course will decrease STUDY DESIGN Prospective open multi-center RCT with follow-up until 3 months after discharge STUDY POPULATION Consecutive elderly hip fracture patients (>55 years) presenting to the ED. Exclusion criteria: polytraumatic injuries; delirious state at presentation and inability to provide informed consent. All patients eligible will be asked to participate in the study. INTERVENTION Using ultrasound guidance, a continuous FICB is placed in the ED <2 hours after presentation. The catheter placed will remain in place until 2 days after surgery to treat pain continuously by titration of LA COMPARISON Usual care (national guideline): opioids, combined with paracetamol and NSAIDs OUTCOME MEASURES Primary outcome will be occurrence of delirium during entire hospital admission, until 3 months after discharge. Secondary outcomes will be pain, patient and staff satisfaction regarding analgesic effects and logistics of the allocated pain treatment, hospital stay characteristics, functional, cognitive and generic and specific Health Related Quality of life (HRQol), cost-effectiveness. SAMPLE SIZE CALCULATION Based on assumed superiority of a continuous FICB in acute pain management in comparison with usual care, registering a minimal clinically relevant difference of 13% in incidence of delirium, using a 5% significance level and power of 80% and allowing for 10% loss to follow-up, we need to recruit 340 patients COST-EFFECTIVENESS ANALYSIS / BUDGET IMPACT ANALYSIS (BIA) We hypothesize that using a continuous FICB in elderly hip fracture patients in the ED is a more tailored approach in acute pain management associated with less health related costs. In addition a BIA will be performed to determine potential financial impact of this more adequate pain strategy for elderly patients with a hip fracture in the ED on national health care costs in the future. This will be based on (clinical) data that reflect size and characteristics of the population, the effectiveness of a continuous FICB and the related resource use and costs TIME SCHEDULE The project will start in 2016 after 6 months preparation with recruiting (29 months) followed by 6 months of analysis and reporting
