Cost-effectiveness of a Non-degradable Urethral Bulking Agent as compared to Mid-urethral Sling surgery in women with Stress Urinary Incontinence

Dit onderzoek heeft 284 patiënten onderzocht met urineverlies bij hoesten, niezen en lachen (stressincontinentie). Waarvan 153 patiënten een operatie met een kunststof bandje ondergingen, oftewel mid-urethrale sling operatie (MUS-operatie) en 131 patiënten ondergingen een poliklinische behandeling met “bulkinjectie therapie”.
Bij bulkinjectie therapie zorgt een siliconen-achtige substantie, die rondom de plasbuis wordt geïnjecteerd, voor minder urineverlies. Eén jaar na de behandeling bleek dat MUS-operatie effectiever was dan de bulkinjectie therapie. 90% gaf aan dat de klachten minder waren na de MUS-operatie versus 48% na bulkinjectie therapie. Beide behandelingen zijn relatief veilig. De MUS-operatie bleek een stuk duurder. Dit kwam enerzijds omdat de behandeling duurder was, maar ook omdat patiënten na de operatie langdurig niet konden werken. Graag willen wij vervolgonderzoek doen om resultaten op langere termijn te kunnen. Wij kunnen dan de patiënten met stressincontinentie volledig informeren.

RESEARCH QUESTION: Is a non-degradable urethral bulking agent (UBA) cost-effective as compared to mid-urethral sling (MUS) surgery in the treatment of moderate to severe stress urinary incontinence (SUI)? HYPOTHESIS: We hypothesize that a non-degradable UBA is non-inferior in efficacy and superior in cost-effectiveness as compared to MUS surgery in the treatment of moderate to severe SUI. STUDY DESIGN: A multicenter, prospective, matched controlled study with an economic evaluation and a patient preference study alongside. Follow-up will be 12 months. STUDY POPULATION: Women aged 18 and older with moderate to severe (predominantly) SUI and a positive cough stress test. INTERVENTION: The UBA will be a peri-urethral injection of a biocompatible, non-degradable biomaterial (Urolastic) in an ambulatory. USUAL CARE: MUS surgery (performed in an operating room). OUTCOME MEASURES: The primary outcome of this study is the subjective improvement at 12 months, assessed by the patient's global impression of improvement. Secondary outcomes are: - Subjective cure - Objective cure, assessed by standardized cough stress test - Patients global impression of severity - Quality of life related to micturition and sexual functioning - Device and procedure related morbidity - Costs - Patient preference, assessed using a trade-off method to quantify efficacy-differences against procedure related setting-differences. SAMPLE SIZE: For the sample size calculation an expected efficacy for UBA and MUS surgery of 72% and 83% respectively were used. With a significance level of 0.05 (one-sided), power of 80% and anticipated loss to follow-up 10%, 240 patients (120 per group) need to be enrolled in this study.
