Guiding prudent use of antimicrobials in companion animal practice to protect animal and human health

In het Antimicrobial Stewardship and Pets (ASAP) project is onderzocht of door middel van een gerichte aanpak binnen gezelschapsdierenpraktijken er verantwoorder met antibiotica wordt omgegaan. Aan de hand van een literatuurstudie, een interviewstudie en een enquête, is een zgn. Antimicrobial Stewardship Programma (ASP) ontwikkeld en uitgerold in 44 Nederlandse gezelschapsdierenpratijken.

Het ASP bestond o.a. uit nascholing, een informatiefolder voor diereigenaren en individuele feedback per praktijk. Het ASP zorgde voor een significante daling van het antibioticumgebruik bovenop de daling in antibioticumgebruik, die onder invloed van andere maatregelen en factoren al was ingezet in de jaren voorafgaand aan het project. De deelnemers gaven aan bewuster om te gaan met antibiotica en konden individueel verschillende verbeterpunten noemen uit de praktijk m.b.t. antibioticumgebruik.

Tijdens de verdere afronding zullen adviezen gegeven worden voor implementatie op grotere schaal.

Aan de hand van een literatuurstudie, een interviewstudie en een enquête onder gezelschapsdierenartsen, is een zgn. Antimicrobial Stewardship Programma (ASP) ontwikkeld en uitgerold in 44 Nederlandse gezelschapsdierenpratijken. Het ASP bestond o.a. uit nascholing en individuele feedback op voorschrijfgedrag voor gezelschapsdierenartsen en een informatiefolder voor diereigenaren. Het ASP zorgde voor een significante daling van het antibioticumgebruik bovenop de daling in antibioticumgebruik, die onder invloed van andere maatregelen en factoren al was ingezet in de jaren voorafgaand aan het project. De deelnemers gaven aan bewuster om te gaan met antibiotica en konden individueel verschillende verbeterpunten noemen uit de praktijk m.b.t. antibioticumgebruik. Samenvatting van de aanvraag

The use of antimicrobials in veterinary medicine is considered a potential threat for public health. In food producing animals in the Netherlands, a considerable reduction of prescription and use of antimicrobials has been accomplished over the last years. In companion animals, there has been less attention for restricted use. Recent data from the Netherlands show that only 5% of the use belongs to the 1st choice antimicrobials and up to 18% of the total use in cats and dogs is 3rd generation cephalosporins and fluoroquinolones. These animals live in close contact with humans with the risk of transfer of highly resistant microorganisms as has been shown for e.g. methicillin resistant Staphylococcus aureus (MRSA). To reduce this public health risk it is important to reduce unnecessary as well as inappropriate use of antimicrobials in companion animals. We propose to develop an intervention program for companion animal practices to promote and support the prudent use of antimicrobials. In phase 1, a barrier analysis will be performed in 15-20 companion animal practices and an intervention program will be developed. In phase 2 the intervention will be performed in a Stepped Wedge design study in 40 companion animal practices. The effects of the intervention program will be evaluated by collecting data on antibiotic prescription rates before and after the intervention for a period of 18 months to assess changes in quantitative and qualitative aspects of antimicrobial use in companion animals. Secondary parameters are: compliance to available guidelines, cascade use, clinical outcome, pet owner satisfaction and time investment by practices. In phase 3 the feasibility and options for large scale implementation of the intervention program will be evaluated and a guide for implementation and monitoring will be communicated with the stakeholders (practitioners, representatives from public health, policy makers, professional organizations).
