Make-It: Medication Adherence Knowledge, Expertise and Implementation Taskforce

Er zijn verschillende succesvolle interventies om therapietrouw te bevorderen. Deze vinden nog niet goed hun weg naar de reguliere zorg. ZonMw gaat daarom inzetten op implementatie van deze interventies in de ambulante zorg. Hiertoe worden 4-8 proeftuinen gefinancierd.
Een kennisconsortium begeleidt deze living labs, ontwikkelt instrumentarium, evalueert de proeftuinen en ondersteunt in het verder verspreiden van de opgedane kennis.

Het Make-It consortium bestaat uit onderzoekers/zorgverleners die al vele jaren samenwerken aan therapietrouwbevordering. Het consortium heeft expertise op het gebied van farmacie, geneeskunde, gedragswetenschappen, zorgonderzoek, communicatiewetenschappen en verpleegkunde. De voorzitter van het nog in te stellen patiëntpanel maakt deel uit van het consortium.

Living labs

De proeftuinen binnen het STIP-programma gaan werken volgens de principes van een living lab. Dit is een afgebakende, levensechte setting waarin verschillende partijen gezamenlijk werken aan een innovatieve oplossing. Een levensechte omgeving is nodig om oplossingen te ontwikkelen die houdbaar zijn binnen de complexiteit van de reguliere zorg.
Co-creatie tussen patiënten, zorgverleners en andere betrokkenen is daarbij essentieel.

Aanpak

Het project heeft vijf werkpakketten:

  • 1: Projectmanagement
  • 2: Inventarisatie van effectieve, geëvalueerde therapietrouwinterventies, incl. voorwaarden voor succesvolle implementatie
  • 3: Ondersteunen van partijen bij indienen aanvraag voor proeftuin
  • 4a: Begeleiden van proeftuinen in uitvoering van de interventies
  • 4b: Evaluatie van effecten van de interventies
  • 4c: Evaluatie van implementatieproces van de interventies
  • 5a: Verspreiding van opgedane kennis naar andere proeftuinen
  • 5b: Rapportage en synthese

De resultaten komen beschikbaar via de website 'Het Therapietrouw Formularium'.

Samenvatting van de aanvraag

Over the past decades, several successful approaches have been found to enhance medication adherence. These do not easily find their way to regular care. The fourth STIP call of ZonMw's Good Use of Medicines program is aimed at using successful approaches to medication adherence promotion in outpatient care. To this end, 4-8 pilot projects will be financed in 2019. A knowledge consortium guides them in the use of interventions, evaluates them, develops instruments and supports further dissemination. The knowledge consortium will be established on July 1, 2019. This proposal describes this consortium. Knowledge consortium The Make-It consortium is a collaboration between researchers and healthcare providers who have been working together on medication adherence for many years. It is multidisciplinary and includes expertise in pharmacy, medicine, behavioral sciences, healthcare research, communication sciences and nursing. The team is supplemented with the chair of a patient panel that will be set up. Living labs We propose that the pilot projects work according to the principles of a living lab. This is a clearly defined, real-life setting where different parties work together on an innovative solution. A real-life environment is needed to develop solutions that are sustainable within the complexity of regular care. Co-creation between patients, caregivers and other stakeholders is essential.
