Onderzoek

In dit project is het effect van behandeling met schildklierhormoon bij ouderen van 80 jaar en ouder met subklinische hypothyroïdie onderzocht. In totaal zijn 105 deelnemers gerandomiseerd in de IEMO 80+ Schildklier studie. De resultaten zijn geanalyseerd samen met de data van de deelnemers van 80 jaar en ouder (n=146) uit de internationale TRUST studie. De resultaten van de studie worden momenteel geanalyseerd.

Resultaten

De ervaring uit dit onderzoek laat zien dat het opzetten en uitvoeren van medicatie-onderzoek bij ouderen complex en tijdrovend is en dat het een specifieke studie-opzet en financiering vereist. Dit wordt veroorzaakt door vele verschillende factoren.

Samenvatting bij start

Probleemstelling

Een licht verminderde werking van de schildklier kan vele klachten veroorzaken, variërend van vermoeidheid en depressie tot hartritmestoornissen. In sommige studies wordt beschreven dat het bij ouderen zorgt voor een verhoogd risico op hart- en vaatziekten, maar in andere studies gaat het bij ouderen juist samen met grotere overleving. Het is niet duidelijk of de aandoening bij ouderen moet worden behandeld.

Onderzoek

In dit project wordt het effect onderzocht van behandeling met schildklierhormoon bij ouderen (80 jaar of ouder) met een licht verminderde werking van de schildklier. Gedurende vier jaar worden 450 deelnemers vervolgd om te kijken wat het effect is op hart- en vaatziekten, kwaliteit van leven, sterfte en andere uitkomsten. 

Resultaat

Hierdoor zal over een aantal jaar een duidelijk antwoord kunnen worden gegeven op de vraag of ouderen met een licht verminderde werking van de schildklier moeten worden behandeld. De studie wordt uitgevoerd door 6 Nederlandse UMC en gecoördineerd door het IEMO.

Eindverslag

Subklinische hypothyroïdie komt veel voor op hoge leeftijd met een prevalentie toenemend met de leeftijd tot 20%.
De doelstelling van de IEMO 80+ Schildklier Studie waren tweeledig; 1: Het onderzoeken van het effect van behandeling met schildklierhormoon bij ouderen (80 jaar en ouder) met subklinische hypothyroïdie en 2: het uitvoeren van een nationale multi-center studie, het opdoen van expertise en het opbouwen van een nationaal netwerk voor toekomstige trials bij ouderen.

Voor de eerste doelstelling zijn in de IEMO 80+ Schildklier studie 105 deelnemers gerandomiseerd. In de analyse zullend de data van deze deelnemers worden samengevoegd met de data van de 146 deelnemers van 80 jaar en ouder uit de TRUST-studie. De resultaten van de studie worden momenteel geanalyseerd en opgeschreven.

Wat betreft de tweede doelstelling is er ervaring opgedaan met verschillende studieopzetten, zo zijn we begonnen als een multi-center studie in Nederland en geëindigd als een mono-center studie en Nederland waarbij ook deelnemers in Zwitserland zijn geïncludeerd. De voor en nadelen van de verschillende opzetten zijn geanalyseerd. Daarnaast is er gekeken welke factoren een rol spelen tot het besluit niet deel te nemen aan de studie door huisartsen en potentiële deelnemers.

Onze ervaring is dat het uitvoeren van een klinische studie met medicatie bij ouderen complex en tijdrovend is. Tijdens de opzet van toekomstige studies moet hier rekening mee worden gehouden. Hierbij moet voldoende budget en tijd beschikbaar gemaakt worden of moet de studieopzet zo aangepast worden dat de uitvoerbaarheid realistisch is voor de populatie.

Tussenstand per 1 september 2016
De maatregelen uit het tussenrapport in juli 2015 zijn geïmplementeerd, waardoor nu een centrale trial organisatie is ontstaan die deelnemers werft en includeert door heel Nederland. In totaal participeren 16 huisartsen-laboratoria en 1020 huisartsen. Bovendien wordt inmiddels ook in Zwitserland geïncludeerd (op eigen kosten). Op dit moment zijn in Nederland 50 deelnemers en in Zwitserland 2 deelnemer gerandomiseerd. In een recente analyse van de knelpunten geeft het merendeel van de huisartsen aan weinig tijd te hebben voor participatie. Om die reden is recent gestart met intensieve begeleiding van huisartsen bij het benaderen van potentiele deelnemers, hetgeen leidt tot een efficiëntere werving.

In het najaar van 2016 worden deelnemers geworven in gebieden waarin dat eerder niet gebeurde en met intensievere ondersteuning van huisartsen, laboratoria en potentiele deelnemers. De verwachting is dat daardoor in inclusie van nieuwe deelnemers fors zal toenemen. Bovendien zullen ook in Zwitserland meer deelnemers worden geworven. De resultaten van deze studie (sample size berekening n=145) worden uiteindelijk gecombineerd met de oudste ouderen uit de TRUST studie (inclusie inmiddels gesloten: n=146 80-plus), waarmee het een van de grootste analyses zal zijn op dit vakgebied. Bovendien worden ervaringen opgetekend voor toekomstige 'investigator-driven' medicatie trials in een rapport met aanbevelingen.

Graag stellen we voor de inclusie en daarmee de looptijd van het project met 6 maanden te verlengen. Bovendien willen we de trial versterken met een extra budget dat beschikbaar kan komen uit het programma Evidence-based Medicine bij Ouderen.

Samenvatting van de aanvraag

In old age, subclinical hypothyroidism is a common finding, with a prevalence increasing with age to up to 20%. In a recent review, which included over fifty thousand individual patients over the age of 65 years, subclinical hypothyroidism was associated with a significantly increased risk of cardiovascular disease and mortality. On the other hand, observational studies suggest that subclinical hypothyroidism may associate with better survival in the oldest old, but these results are unequivocal. Clinical guidelines are inconsistent with respect to advocation of treatment, because large scale randomized controlled trial are lacking in all age groups. This lack of evidence in older age is "symptomatic" for the shortage of clinical trials performed in old age. Elderly are systematically excluded from clinical trials. Performing clinical trials in older age is demanding and complex and collaboration between multiple institutions is necessary to perform such trials. The aims of the present study are 1) to investigate the effect of thyroid hormone supplementation in persons with subclinical hypothyroidism aged 80 years and older 2) to perform the study in a national multi-center collaboration, to serve as a working example, building expertise and a nation-wide infrastructure for future trials in older age The Institute for Evidence-based Medicine in Old Age (IEMO) was founded with the aim to improve the healthcare for older people by substantiating the evidence for their treatment. The IEMO 80-plus thyroid trial will recruit and retain 80-plus year old participants in five University Medical Centers throughout the Netherlands. The trial will be performed in close cooperation with the European Union funded TRUST thyroid trial, a pan-European multi-center RCT on subclinical hypothyroidism in participants over 65 years of age. The Leiden University Medical Center is one of the participants of this consortium, and is also a part of the present project. This cooperation allows for efficient use of study infrastructure and maximizes the impact of resources committed. The TRUST thyroid trial does not have sufficient power for a formal subgroup analysis of the oldest old. The present project will comprise a Dutch multi-center RCT specifically for 80-plus year olds, allowing (in a combined analysis with the TRUST 80-plus year olds) for a powerful subgroup analysis in the oldest old. We will tailor the trial for the oldest old, maximizing inclusion and retention of representative older people, studying end-points relevant in old age. This national trial in old age sets an example and builds expertise and infrastructure for future trials in old age. Patients will be identified through laboratories, where thyroid function tests have revealed SCH. Minimal exclusion criteria will be applied to allow generalization to the elderly population at large. Participants are randomized to receive active treatment or placebo during a mean follow-up of 3 years. Primary outcome measures will be the risk of fatal and non-fatal cardiovascular disease and disease specific quality of life. Secondary outcomes will include overall quality of life, cognitive function, muscle strength, functional ability, and total mortality. Participants will be visited at their place of residence throughout the study to ensure minimal burden for the older participant and hence maximal participation. Inclusion of 450 patients and combining with the TRUST 80-plus participants will provide enough power to detect a 25% reduction of cardiovascular disease and mortality in the 80-plus year old subgroup (IEMO and TRUST combined). Furthermore, ample power is present for analyses of the second primary and secondary end points. The results of this study will lead to specific guidelines for the treatment of sub-clinical hypothyroidism in the oldest old, studied in representative elderly participants on relevant end points.
