Characterisation of the influence of (morbid) obesity on CYP3A-mediated clearance and oral absorption using midazolam as a model drug: towards evidence-based dosing in obesity.

Concentraties en effecten van geneesmiddelen kunnen anders zijn bij patiënten met overgewicht in vergelijking met patiënten zonder overgewicht. Daarbij kan dit ook veranderen als zij een maagverkleiningsoperatie ondergaan.

Onderzoek

Tijdens dit onderzoek werden midazolam bloedconcentraties in morbide obese patiënten (BMI > 40 kg/m2) rondom en één jaar na een maagverkleiningsoperatie verzameld. Midazolam is een veelgebruikt slaap- en narcose middel.

Resultaten

De belangrijkste uitkomsten zijn dat de dosering van een tablet midazolam voor patiënten met morbide obesitas niet anders hoeft te zijn dan voor patiënten zonder overgewicht. Echter, als midazolam als narcosemiddel via het infuus wordt gegeven zijn er wel hogere doseringen nodig.

Verder werd gevonden dat een midazolam tablet sneller werkt bij patiënten na een maagverkleiningsoperatie dan bij dezelfde patiënten vóór de operatie en dat midazolam na een tablet of infuus eerder is uitgewerkt. Op basis van deze resultaten zijn midazolam doseeradviezen voor obese patiënten en patiënten na een maagverkleiningsoperatie vastgesteld.

Geneesmiddel concentraties en effecten kunnen anders zijn in patiënten met overgewicht. Daarbij zijn opnieuw veranderingen denkbaar als zij een maagverkleinende operatie ondergaan.
Tijdens dit onderzoek werden midazolam bloedconcentraties in morbide obese patiënten (BMI > 40 kg/m2) rondom en één jaar na een maagverkleiningsoperatie verzameld, gemeten en vergeleken. Midazolam is een veelgebruikt slaap- en narcose middel. De belangrijkste uitkomsten zijn dat de dosering van een tablet midazolam voor patiënten met morbide obesitas niet anders hoeft te zijn dan in patiënten zonder overgewicht. Echter, als midazolam als narcosemiddel via het infuus wordt gegeven zijn er wel hogere doseringen nodig dan bij patiënten zonder overgewicht. Bij een patiënten na een maagverkleiningsoperatie werd gevonden dat een midazolam tablet sneller werkt dan bij dezelfde patiënten vóór de operatie en dat midazolam na een tablet of infuus eerder is uitgewerkt.
Op basis van deze resultaten zijn midazolam doseeradviezen voor obese patiënten en patiënten na een maagverkleiningsoperatie vastgesteld.

Voor veel geneesmiddelen is het nog onduidelijk of en hoe doseringen moeten worden aangepast in patiënten met obesitas of ernstig overgewicht. In dit onderzoek wordt het geneesmiddel midazolam onderzocht. Midazolam is een veelgebruikt slaapmiddel en anesthesiemiddel en kan zowel als tablet als injectie worden toegediend. Er zijn aanwijzingen dat midazolam minder goed wordt afgebroken in obese patiënten wat zou betekenen dat het middel wellicht lager moet worden gedoseerd.
Het doel van dit project is dan ook om te bepalen hoe obesitas de opname en afbraak van midazolam beïnvloedt. Daarom zijn er midazolam bloedconcentraties van zeer obese patiënten rondom een maagverkleiningsoperatie verzameld en gemeten. Een jaar na deze operatie, nadat de patiënten waren afgevallen, hebben we nog een keer midazolam bloed concentraties verzameld en gemeten.
Op basis van de gemeten midazolam concentraties is er een farmacokinetiek model ontwikkeld waarmee parameters zoals klaring en verdelingsvolume geschat kunnen worden. Vervolgens is de variabiliteit tussen patiënten in kaart gebracht en de invloed van obesitas en een maagverkleining op midazolam concentraties bepaald.
Samenvattingen van de projectresultaten zijn inmiddels als twee posterpresentaties geaccepteerd op het Amerikaanse congres opgebied van klinische farmacologie (ASCPT) in Atlanta.

Samenvatting van de aanvraag

Midazolam is widely used for sleeping disorders (oral use), (pre)anesthesia (i.v. use) or sedation in the Intensive Care (i.v. continuous infusion). Midazolam is primarily metabolized into 1-OH-midazolam by Cytochrome P450 3A (CYP3A). CYP3A is an important enzyme system that is involved in metabolism of 35% of all metabolized drugs, including many drugs that are relevant for obese patients, such as statins, cardiovascular drugs, antipsychotics and oncolytic drugs. As the CYP3A enzyme is located in the liver as well as in the intestines, it determines both systemic clearance and oral drug bioavailability of CYP3A substrates. To date, no evidence-based dosing guidelines for midazolam or other strong CYP3A substrates in obese (BMI>30 kg/m2) or morbidly obese (BMI>40 kg/m2) patients are available. While in obese patients in general higher doses are anticipated, preliminary evidence indicates that absolute clearance of midazolam and other strong CYP3A substrates is lower in obese patients. A reduced absolute clearance is highly relevant, because it may result in prolonged effect of the drug with increased risk of adverse events particularly upon prolonged use. For midazolam, adverse effects comprise respiratory depression, reduced alertness or increased fall incidence. Particularly obese patients are prone for respiratory depression due to their body composition and the high incidence of sleep apnea syndrome. In this project the primary aim is to characterise the influence of (morbid) obesity on CYP3A-mediated drug clearance and oral absorption using midazolam as model drug and to provide practical dosing guidelines for oral and i.v. midazolam in obese and morbidly obese patients and for oral midazolam in patients that underwent a gastric bypass procedure (weight-reducing surgery). The secondary aim of this project is to study whether the derived dosing guidelines for midazolam can be extrapolated to other CYP3A substrates. For the primary aim, a clinical trial will be performed in which oral and subsequently i.v. midazolam are administered to morbidly obese patients (BMI> 40 mg/m2: 100-250 kg) before they undergo weight-reducing surgery. Plasma samples for determination of midazolam parent and the 1-OH-midazolam metabolite will be collected until 8.5 hours after midazolam administration allowing for the characterization of CYP3A activity in this patient population. For the data analysis, both compartmental and minimal physiologic-based pharmacokinetics (PBPK) modeling will be used to fit midazolam and 1-OH-midazolam concentrations from the morbidly obese patients together with oral and i.v. midazolam data from healthy volunteers. These data in volunteers are available within the project group and have been obtained upon the same design of oral and subsequent i.v. dosing. Using this approach, the influence of obesity on CYP3A mediated clearance and CYP3A mediated bioavailability will be characterised and quantified. The developed model is used to derive evidence-based dosing guidelines for oral and i.v. midazolam in (morbidly) obese patients. To identify dosing guidelines for midazolam in morbidly obese patients that underwent a commonly applied weight-reducing surgery procedure (i.e. gastric bypass procedure which results in a bypass of large part of the stomach and approximately 1 meter of intestine), all morbidly obese patients will be re-studied after one year using the same study design of oral and subsequent i.v. dose. As these patients are expected to have lost substantial weight, the results of the re-study can also be used to identify whether obesity-induced changes in CYP3A-mediated midazolam clearance is reversible after weight loss. For the secondary aim of this project, a PBPK simulation study using SimCYP software will be performed to study whether the derived dosing guidelines for midazolam can be extrapolated to other CYP3A substrates. Using SimCYP the influence of obesity on the pharmacokinetics of other CYP3A substrates, with a variety of drug characteristics, can be explored. In this way other CYP3A substrates of which the pharmacokinetics are affected in the same extent as for midazolam will be identified. This principle was applied before in another project on drug glucuronidation in children, in which morphine was used as model drug. Using this approach, it was shown to which UGT2B7 drugs the age-related changes in morphine glucuronidation could be extrapolated to. All dosing advices on midazolam and potentially other CYP3A substrates resulting from this project will be implemented in the G-standard database and Informatorium Medicamentorum of the Royal Dutch Pharmacist Organization (KNMP). The G-standard is the KNMP database which contains all information about drugs, including dosing regimens, interaction, adverse effect, etc. This database is part of software systems, which are used in general pharmacies, hospital pharmacies and by general practitioners.
