Efficacy of paracetamol, NSAIDs and advice for patients with acute low back pain: a placebo controlled trial (PACE-plus)

Deze studie beoogde het effect van zowel paracetamol als NSAIDs bij acute lage rugpijn te onderzoeken in een placebo gecontroleerde gerandomiseerde klinische trial. De vraagstellingen waren als volgt:
  • Wat is het effect van paracetamol in vergelijking met placebo voor patiënten met acute lage rugpijn?
  • Wat is het additionele effect van paracetamol of NSAIDs in vergelijking met alleen advies bij patiënten met acute lage rugpijn?
  • Wat is het effect van paracetamol in vergelijking met NSAIDs voor patiënten met acute lage rugpijn?

Om deze vraagstellingen te kunnen beantwoorden moesten 800 patiënten worden geïncludeerd die zich met acute a-specifieke lage rugpijn presenteren in de huisartsenpraktijk. Zij werden gerandomiseerd over vier interventies:
1.    alleen advies
2.    advies + paracetamol
3.    advies + NSAID
4.    advies + placebo
De primaire uitkomstmaat was pijnintensiteit, gemeten over een follow-up periode van 12 weken.

Vanwege niet genoeg deelnemende patiënten is dit onderzoek vroegtijdig stopgezet (juni
2017). Het bleek erg moeilijk voor huisartsen om geschikte patiënten tijdens hun spreekuur uit
te nodigen voor deelname aan het onderzoek. Hierdoor is de nog steeds relevante
vraagstelling niet beantwoord.

Vanwege niet genoeg deelnemende patiënten is dit onderzoek vroegtijdig stopgezet (juni 2017). Het bleek erg moeilijk voor huisartsen om geschikte patiënten tijdens hun spreekuur uit te nodigen voor deelname aan het onderzoek. Hierdoor is de nog steeds relevante vraagstelling niet beantwoord.

Samenvatting van de aanvraag

Low back pain is common and associated with a considerable burden to patients and society. Their is uncertainty regarding the relative benefit of paracetamol and NSAIDS in the management of acute low back pain. The objectives of the present study are: 1. What is the efficacy of paracetamol compared to placebo for patients with acute low back pain? 2. What is additional benefit of paracetamol or NSAIDs plus advice versus advice only for patients with acute low back pain? 3. What is the efficacy of paracetamol versus NSAIDs for patients with acute low back pain? Design: placebo controlled randomized clinical trial in primary care. Study Population: patients with acute non-specific low back pain presenting in general practice. Study treatments (group size 4 times 200 patients) : Group 1: Standardized advice: (to avoid bed rest and stay active) and reassurance of a favorable prognosis. Group 2: Standardized advice + paracetamol (time contingent) Group 3: Standardized advice + NSAID (time contingent) Group 4: Standardized advice + placebo. Outcomes: The primary outcome will be pain intensity over a 12 week follow up period. Secondary outcomes time to recovery, disability, function, global perceived effect and sleep quality. These data will be captured at weeks one, two, four and twelve post randomisation.
