De studie is voortijdig gestopt, omdat de inclusie niet op gang kwam. Reden hiervoor was dat patiënten zich niet wilden laten randomiseren en behandelaren een duidelijke voorkeur hadden voor een bepaald middel in een bepaalde situatie. Hierdoor is de doelstelling niet gehaald en zijn er geen specifieke resultaten ten aanzien van de kosteneffectiviteit van penfluridol.

Aanleiding
De middelen penfluridol, olanzapine en risperidone zijn bedoeld voor de behandeling van psychoses. Penfluridol heeft de unieke eigenschap dat patiënten het maar eens per week hoeven te slikken, terwijl de andere middelen dagelijks moeten worden gebruikt. Penfluridol is bovendien goedkoper. Aangezien dit middel wekelijks kan worden ingenomen is de kans kleiner dat het wordt vergeten, en is de therapietrouw vermoedelijk beter dan bij de anderen middelen. Er bestaat geen recent onderzoek naar de (kosten)effectiviteit van dit middel, en het middel is daarom niet opgenomen in richtlijnen. Doel
Het doel van dit onderzoek is het onderzoeken van de kosteneffectiviteit van een reeds bestaand geneesmiddel genaamd penfluridol (Groep A), in vergelijking met twee andere middelen (olanzapine en risperidone; Groep B). Opzet
180 Patiënten doen mee aan het onderzoek, verdeeld over twee groepen A en B. De primaire uitkomstmaat is gezondheidszorg gerelateerde kosten per Quality Adjusted Life Years (QALY). Duur van de follow-up is 1 jaar.

Het doel van de studie was het bepalen van de kosten-effectiviteit van het middel penfluridol d.m.v. een gerandomiseerde studie waarbij we de kosten-effectiviteit van penfluridol vergelijken met olanzapine en risperidone oraal. Deze doelstelling is niet gehaald aangezien de studie veel vertraging opgelopen heeft om diverse redenen (o.a. wisseling onderzoekers, medisch ethische toetsingsprocedures, eisen aan geneesmiddelentrials) en uiteindelijk de inclusie niet op gang kwam omdat patienten zich niet wilden laten randomiseren en behandelaren een duidelijke voorkeur hadden voor een bepaald middel in een bepaalde situatie. Daarom werd de studie vroegtijdig beeindigd.

Samenvatting van de aanvraag

Background Penfluridol (Semap) is the only long acting neuroleptic today that can be taken orally once a week for the treatment of schizophrenia and other psychotic disorders. Due to its weekly dosage scheme, it can be used for the treatment of psychotic disorder patients who have compliance problems. Compliance problems are present in 50-75% of psychotic disorder patients. Other oral antipsychotic drugs, including olanzapine and risperidone, have to be taken daily. In addition, penfluridol is cheap as compared to olanzapine and risperidone. Since penfluridol can be taken once weekly, we expect better compliance as compared to olanzapine and risperidone. Better compliance may be associated with less relapses, better quality of life, less crises and less or shorter hospitalisations, leading to lower costs. There have been no studies comparing penfluridol and second generation neuroleptics. Given the absence of evidence on these matters, penfluridol is not recommended in any guidelines on psychiatric treatment of psychosis, although costs may be lower and compliance and outcome better. Aims The aims of this multicenter study are to compare penfluridol and second generation oral neuroleptics (olanzapine, risperidone) for their effects on health related costs and Quality Adjusted Life Years (QALY;s) (primary outcome variables), compliance, (side)effects, functioning, subjective well-being under neuroleptics and time to first discontinuation of the neuroleptic (secondary outcome variables). Design: open label randomized controlled trial in 180 adult psychotic disorder patients. Duration of follow-up is 12-months. There will be two assessments by independent raters at baseline and at 12-month follow-up. Hypotheses We hypothesize that penfluridol as compared to oral second generation neuroleptics (olanzapine and risperidone), will show lower total costs, more QALY’s (primary outcomes), and better compliance and as a consequence better symptomatic and functional outcome (secondary outcomes). Furthermore, we hypothesize that there will be no differences in overall frequency and severity of side effects, subjective well-being under neuroleptics and time to first discontinuation of the neuroleptic between penfluridol as compared to oral second generation neuroleptics. Analyses The hypotheses are tested using Cost-Effectiveness Analyses (CEA). Costs and effects will be discounted according to the Dutch guideline for economic evaluation in Health Care. The cost-utility is evaluated by relating the difference in direct medical costs per patient to the difference in terms of QALY's, which yields a cost per QALY estimate (using the SF-36 as well as the CORE-6D). The incremental cost-effectiveness ratio (ICER) will also be calculated. In addition a budget impact analysis with a time horizon of five years will be done. Secondary outcome measures will be analysed using multilevel analyses, controlling for the effects of site.
