Are ursodeoxycholic acid, mesalazine, thiopurines, and non-steroidals efficacious and cost-effective in primary sclerosing cholangitis? A nationwide study.

Primary sclerosing cholangitis (PSC) is een ernstige galwegziekte die jonge mensen treft.
Het meest voorgeschreven medicijn is ursodeoxylcholzuur. Maar na 20 jaar onderzoek is de waarde hiervan nog immer onduidelijk. PSC gaat vaak gepaard met colitis ulcerosa (UC), wat de kans op dikke darmkanker 10 keer verhoogt vergeleken met UC. Bij PSC/UC is weinig bekend over beschermende medicatie hiervoor.
In de afgelopen 5 jaar hebben wij de EpiPSC database (DB) gebouwd, die 695 PSC-patiënten bevat. Deze krijgt koppelingen met landelijke farmaciedatabases, die sinds 2003 95% van de voorgeschreven medicatie in Nederland bevatten.
Met dit ideale platform willen wij observationele studies uitvoeren naar het effect van vaak gebruikte medicijnen bij PSC-patiënten op late uitkomsten, wat nooit met klinische trials kan. Budgeteffecten van veranderingen in voorschrijfgedrag volgend op nieuwe richtlijnen die voortkomen uit onze studie, zullen worden bestudeerd vanuit overheids- en verzekeraarsperspectief.

Samenvatting van de aanvraag

Primary sclerosing cholangitis (PSC) is a disease of the biliary tree affecting young adults with a median time to death or liver transplantation of 21 years. The most widely prescribed drug in PSC is ursodeoxycholic acid (UDCA). However, its use is after 2 decades of clinical trials still debated. PSC/Colitis (UC) patients have a 10-fold increased risk of developing colorectal carcinoma (CRC) as compared to UC patients. For UC, there are concordant data that thiopurines are chemopreventive with regard to CRC. Data on mesalazine are controversial. For PSC/UC, there are only a few data on UDCA. In the past 5 years we have built the Epi PSC cohort, containing 695 well-ascertained PSC patients, which will be expanded to a web-based nationwide level. This data warehouse will have links to pharmacy database systems covering 95% of prescriptions in the Netherlands since 2003. With this ideal platform we intend to perform observational studies on the impact of commonly used drugs on late clinical outcomes that can never be studied in randomized trials. The short- and mid-term budget impacts of changes in prescription guidelines following the results of the observational study and clinical trial will be assessed from governmental and insurer perspective.
